The effectiveness of biomaterial self-sampling devices in detecting human papillomavirus

Abstract

Cervical cancer currently ranks fourth in the structure of malignant oncological diseases in the world. Breast cancer is one of the most preventable and treatable forms of cancer if it is prevented by vaccination against human papillomavirus (HPV), detected at an early stage and effectively controlled. Russia is the leader among newly identified cases of breast cancer, as indicated by data for 2022, when 127 cases of the disease per 100,000 population [1]. According to the WHO Expert council, about 530,000 new cases of breast cancer are registered annually in the world. Russia is considered the leader among European countries in the number of detected cases between the ages of 29 and 34, as indicated by data from the Global Cancer Observatory [2, 3]. In 2020, WHO developed three strategies for the elimination of breast cancer. These include vaccination of at least 90% of girls against HPV under the age of 15, screening coverage of 70% of women using the highly effective test by the age of 35 and again by the age of 45, as well as treatment of at least 90% of precancerous and breast cancer. Detection of high–risk HPV is an effective screening tool recognized by large international randomized trials. Modern screening programs for the detection of breast cancer face two main problems that require immediate solutions: insufficient coverage of the female population and the need to improve diagnostic methods. Currently, the main focus is on increasing the number of women undergoing screening and optimizing breast cancer screening methods, the most promising of which is the detection of high-risk human papillomavirus (HPV) [4]. Effective screening requires not only a sensitive diagnostic method, but also a broad coverage of the female population by the existing program. However, the unavailability of medical facilities or the fear of diagnostic methods eventually set the task of creating a method of individual diagnostic methods without the direct intervention of a doctor. For this purpose, many vaginal secretion self-sampling test systems have been implemented, which have shown their effectiveness in cohort studies, having high predictive value and the greatest comfort for women.

Modern views on the etiopathogenesis of endometriosis: clinical demonstration of problematic issues

Abstract

Endometriosis is a disease directly related to the proliferation of endometrial-like tissue and affecting both the genitals and the abdominal cavity, pelvic cavity, in some cases the intestines, pleura, kidneys and other distant localizations. Since the term "endometriosis" appeared, many theories have been proposed to explain the occurrence and development of endometriosis, but the complex underlying causes of its development remain uncertain, and none of them can explain all types of endometriosis in general and is not generally recognized and proven. The aim of the study is to highlight the problems of the etiopathogenesis of endometriosis in the context of the presented case of extragenital endometriosis of mammary localization. As part of the work, an assessment of the medical history of a patient with a clinical case of endometriosis of the right mammary gland was made. For histological evaluation of the affected tissue, both routine histological methods and immunohistochemical ones (CK7, p16, p53, vimentin, Ki67, GATA3, GCDFP15, mammoglobin, Cdx2) were used. Thus, to date, the mechanism of occurrence and spread of endometriosis has not been fully studied, and none of the theories has provided an exhaustive explanation for all aspects of this disease. In some clinical cases, the development of extragenital endometriosis cannot be explained within the framework of one or even several related theories. The etiopathogenesis of endometriosis is complex and includes many factors and processes, and only through a deeper understanding can effective treatment methods be developed.

Dysmenorrhea and its impact on women's quality of life

Abstract

Dysmenorrhea (algodismenorrhea, algomenorrhea) is a syndrome that occurs on the eve or during menstrual bleeding, the leading clinical manifestation of which is pain in the sacrum and lower back. Goal. To study the relevance of dysmenorrhea and its impact on the quality of life among women of different ages. Materials and methods. In this study, 280 respondents were surveyed, representing 2 age groups: 150 women (53.6%) from 16 to 20 years old and 130 women (46.4%) from 21 to 50 years old. Results. In order to differentiate the age for which the first episodes of dysmenorrhea are most characteristic, the respondents needed to indicate when the painful sensations were first recorded. At the age of 12, this syndrome was noted by 15 respondents (6.1%); from 12-14 years – 102 respondents (41.8%); from 15-17 years – 78 respondents (32%); from 17-25 years – 42 respondents (17.2%); after 25 years – 7 respondents (2.9%). The results show that dysmenorrhea is more likely to occur between the ages of 12 and 17. Conclusions. Dysmenorrhea remains one of the leading gynecological problems of women's health, 244 respondents noted the presence of pain syndrome. Girls between the ages of 16 and 20 experience menstrual pain in 92.6% of cases.

Labor outcomes for mother and fetus in case of anatomically contracted pelvis

Abstract

Abstract.

Introduction. The issue is concerned with the labor outcomes for mother and fetus in case of anatomically contracted pelvis. The goal of this study is to learn the labor outcomes for mother and fetus in women from the Voronezh Region. Materials and methods. From June 2024 to February 2025 it was analyzed 442 birth histories at the Maternity hospital Emergency medical Hospital №10. Results. It was studied 442 labor histories, 60 of them was with anatomically contracted pelvis (13,5%). Among them there are generally contracted - 43 (71,7%), transverse-contracted - 14 (23,3%), coxalgic pelvis  - 3 (5%). Primiparas - 25 (41,6%) and  multiparas - 31 (51,6%). Complications among women of the experimental group: premature rupture of the membrane (PPROM)  – 22 (36,6%), labor anomalies – 17 (28,3%), clinically contracted pelvis – 8 (13,3%), shoulder dystocia – 1 (1,6%), CNS depression – 5 (8,3%), acute fetal hypoxia – 3 (5%), labor fetal injury – 4 (6,6%), fetal infection – 3 (5%). In the control group there were 310 cases of PPROM among all women (70,1%), 91 (20,5%) cases of labor anomalies. Conclusions. The incidence of anatomically contracted pelvis was 13,5%, which is the prognostic index for detecting the possibility of labor complications.

Compliance with Birth Interval Among Residents of Bishkek

Abstract

In the Kyrgyz Republic, the proportion of women with a birth interval of less than three years is increasing. Addressing the issue of short birth intervals requires identifying the practical aspects of non-compliance with the interdelivery interval, including ensuring universal access to high-quality, comprehensive, and detailed information about the optimal birth interval. Objective. To assess the knowledge and attitudes of postpartum women in Bishkek regarding the optimal birth interval. Materials and Methods. After obtaining written consent for participation in an anonymous study, we conducted an anonymous online survey among 98 postpartum women at Clinical Maternity Hospital No. 2 in Bishkek. Among them, 55 had a birth interval of less than 33 months, while 43 had an interval of 33 months or more. The collected data were processed using Microsoft Office Excel 2016. Results. 25% of respondents deliberately did not follow the recommended interdelivery interval. When asked about the duration of a safe birth interval, the majority of respondents (74%) stated that it should be three years. Conclusion. The reasons for short birth intervals are not only medical (e.g., not having the opportunity to consult a doctor regarding contraception) but also social (e.g., a woman's desire to give birth at a younger age, or the sex of the previous child—when a family has more daughters, women may try to conceive sooner to have a son).

Course of pregnancy and birth outcomes in the age aspect

Abstract

In the Kyrgyz Republic, there has been an increase in the number of births in the group of women of late reproductive age (40 years and older) in the age structure of fertility, from 2.9% in 2019 to 3.8% in 2023. “Pregnancy after 40” is growing due to changes in lifestyle, the status of women in society, and the emergence of assisted reproductive technologies. At the same time, it is considered that this category of patients is exposed to many risks associated with pregnancy and childbirth. Objective. To study the course of pregnancy and birth outcomes in women aged 40 years and older.Materials and Methods. A retrospective analysis was conducted on 198 birth histories of women who gave birth in the clinical maternity hospital № 2 in Bishkek, of which the main group consisted of 46 women of late reproductive age (40 years and older), and the control group consisted of 152 women of optimal reproductive age (20-25 years). Inclusion criteria: age, gestational age between 22-42 weeks. Exclusion criteria: in vitro fertilization. Ethical approval was obtained before the data collection process. Relative indicators (absolute numbers, %) were used to present qualitative features, and Pearson’s χ2 criterion was applied for their evaluation.Conclusion. Late reproductive age (40 years and older) affects the development of complications such as preeclampsia, premature labor, and operative delivery, which should be anticipated by the obstetrician.

Relevance of urine analysis for HPV as a screening method for preventing and detecting precancerous and cancerous conditions of the cervix

Abstract

Abstract.

Introduction. Cervical cancer remains a serious medical problem, especially in developing countries where approximately 500 thousand new cases are registered annually. The main cause of the disease is long-term infection with human papillomavirus (HPV). Despite the importance of screening, many women avoid visiting doctors, which complicates early diagnosis. The development of urine analysis methods for HPV allows simplifying the detection process of cervical lesions by providing convenience and cost-effectiveness. Objective: To assess the relevance and effectiveness of using laboratory urine analysis for HPV as a screening method to detect precancerous and cancerous conditions of the cervix and reduce mortality rates. Materials and Methods: Based on VGKMP No. 1 under the Ministry of Health of the Russian Federation, we conducted research on the effectiveness of urine analysis for HPV to identify cervical pathologies. We evaluated the results from 30 patients divided into two groups based on PCR test systems used: HPV Quant 15 and HPV Quant 21 produced by DNA Technology, Russia. Results. The study showed that in the first group, full correspondence of results was observed in 22%, partial correspondence in 45%, and discrepancy in 33%. In the second group, partial correspondence was noted in 25% of patients, full correspondence in 33%, and discrepancies in 42%. The proportion of low virus titers in smears from the cervical surface among patients from Group 1 with discrepancies amounted to 58%, while high titers were 42%; 60% and 40% respectively for those examined in Group 2. Abnormalities during colposcopy in Group 1 were detected in 10 patients (55.55%). Among 12 patients with positive urine HPV tests, an abnormal picture was found in 8 (66.66%), increasing diagnostic sensitivity by 11.11% compared to the method of taking cervical samples. Among the 18 HPV-positive cervical samples, the first group showed abnormal cytology in 5 patients (27.77%). Four of the abnormal cytological samples (80%) had both positive HPV tests, with 2 being classified as ASC-US, 1 as LSIL, and 1 as HSIL. In Group 2, abnormal colposcopic findings were noted in 6 patients (50%). Out of 7 positive urine tests, 4 (57.14%) also revealed anomalies, enhancing the efficiency of colposcopic diagnostics by 7.14% when HPV status in urine was positive. In the second group, abnormal cytology was detected in 3 patients (25%), including 2 (28.57%) out of 7 HPV-positive urine tests showing ASC-US and LSIL. Conclusion. The results demonstrated a correlation between the presence of HPV DNA in urine and high concentrations of the virus in smears. The frequency of abnormalities during colposcopy and deviations in oncocytology with a positive urine HPV test turned out to be significant. This confirms the potential use of urine analysis for HPV as a possible screening method for preventing and detecting precancerous states of the cervix.

Topographic justification for cesarean section in the lower uterine segment

Abstract

Abstract.According to WHO, the average frequency of cesarean section (CS) in the world is about 20% and is steadily increasing. Every year, more than 1 million women give birth by CS. The frequency of cesarean section in Russia increased from 5% in 1970 to 31.9 percent in 2023. CS reduces the risk of perinatal mortality. A transverse incision in the lower uterine segment is used in most women who are indicated for abdominal delivery. Other incision options are used extremely rarely. The aim of the study is to substantiate the advantages of the technique of cesarean section in the lower uterine segment. Materials and methods. The data from statistical and annual reports of the obstetric departments of the perinatal center as part of the State Healthcare Institution of the Vologda Regional Clinical Hospital No. 1, literary sources, Internet resources, and WHO statistical indicators were used. Results. The method of CS in the lower uterine segment allows the surgeon to obtain adequate access to the uterus, perform the necessary manipulations with a minimal risk of postoperative complications, is less traumatic, has a smaller volume of blood loss, favorable conditions for peritonization of the suture on the uterus, and better reproductive prognosis. Conclusions. A transverse incision in the lower uterine segment is used in most women who are indicated for abdominal delivery. Other types of incisions (corporal, in the bottom of the uterus) are used extremely rarely for certain indications.

Effectiveness of TruScreen in diagnosing precancer and cervical cancer

Abstract

Cervical cancer (CC) is a malignant gynecological neoplasm and has long been ranked 5th (5.2%) in the structure of oncological morbidity in women, second only to 2 gynecological localizations and one of the leading causes of cancer mortality among young women. In the Russian Federation, according to 2018 data, 17,766 cases of the disease were identified, which is 22.3% more than in 2008 (13,807). In 2018, mortality from CC was 6,392, which puts this disease in 10th place in the structure of causes of cancer mortality in women. The peak incidence of CC, unlike other malignant neoplasms of the female genital tract, occurs at a younger age of 40 - 49 years (42.8 per 100 thousand of the corresponding population). Mortality rates from cervical cancer are steadily increasing, so there is a need to create new diagnostic methods that would allow determining changes in cervical tissues with high accuracy, for the early detection of pathological precancerous changes. TruScreen is a portable device for detecting cervical cancer, which was recognized by WHO in May 2019. This innovative scanner is capable of detecting cervical tissue damage, precancerous changes and cancerous processes with high accuracy in real time.

The impact of human papillomavirus -infection awareness on the psycho-emotional status of women

Abstract

Introduction. Every year more than 630 thousand cases of HPV-associated cancer are registered worldwide, which causes a psycho-emotional disturbance in infected patients due to the uncertainty of the prognosis. The goal of this study is to determine the impact of HPV-status awareness on the level of subjective well-being. Materials and methods. The study involved 434 patients aged 18-30 who filled out the survey (A. Perue-Badu questionnaire) and gynecological examination with PCR-diagnosis of HPV. Results. The results showed that patients with HPV-infection experience and awareness of its risks showed increased levels of tension, sensitivity, and self-esteem of health, while no significant differences were found in the clusters "importance of the social environment" and "satisfaction with daily activities". The highest scores on the mood swings cluster were found in women with HPV-negative status, but who were aware of a previous infection, which may be related to anxiety about relapses. Conclusions. Thus, awareness of HPV-status significantly affects subjective well-being, emphasizing the need for psychological support to improve the quality of life of patients.

Cytological screening in the Voronezh region: relevance, statistical data, problems and ways of improvement

Abstract

A steady increase in the incidence of cervical malignancy (cervical cancer) and the rejuvenation of the age group of patients actualize the need to optimize preventive programs and methods of early detection of the disease. Special attention is paid to the annual increase in the number of newly diagnosed pathologies among women of reproductive age (21-35 years). According to the statistics of the Voronezh Region, in 2023, 242 patients with newly diagnosed ZNO were registered, and 50% of them had the disease detected at stage I. This pathology remains a significant factor in female mortality: mortality in the first year after diagnosis reaches 12.7% in the Russian Federation, demonstrating a steady negative trend. The persistence of human papillomavirus (HPV), especially highly oncogenic types (HPV-16 and HPV-18), is recognized as a key factor in the development of breast cancer. Despite the possibility of preventing breast cancer through timely diagnosis of precancerous conditions, the problem is compounded by low coverage of screening programs and insufficient effectiveness of preventive examinations. The article discusses regional statistical data and modern cytological screening strategies, including two-stage diagnosis. An analysis of 3 years of data (270,748 women were examined) revealed an HPV-positive status in 15.63% of the surveyed. Distribution by serotypes: 29.7% — types 16/18; 44.4% — 2-3 serotypes; 8.4% — 4 or more serotypes. These data confirm the importance of HPV screening in the prevention of breast cancer, and combined diagnosis (PAP test and HPV typing) has proven effective in reducing morbidity and mortality. The successful implementation of screening requires the combined efforts of medical professionals, health authorities and the public.

Epidemiology of fat metabolism disorders in pregnant women in an obstetric hospital

Abstract

Introduction: Women's and children's health, as well as reproductive health, remain important topics in the healthcare system. The successful realization of a woman's reproductive potential depends on many factors, including metabolic balance, a healthy lifestyle, and genetic aspects. However, exogenous factors can negatively affect the process of fertilization and the course of pregnancy. One of the urgent problems is obesity, which affects the reproductive function and health of women. According to WHO, in 2022, 44% of women were overweight, which increases the risks to the health of the mother and fetus. Pre-pregnancy training, including normalization of body weight and rejection of bad habits, can reduce obstetric and perinatal risks.

 The aim of the study: was to determine the prevalence of obesity in obstetrichospitalpatientsand the frequencyof pre-pregnancy trainingamongwomenwithmetabolicdisorders.

Materials and methods: A survey of 69 pregnant women in an obstetric hospital was conducted. The study included an analysis of exchange cards, birth history, and survey results. The participants were divided into groups by body mass index (BMI), and weight gain was assessed. Results: The average age of the participants was 29.4 years, the average BMI was 25.6 kg/m2. 39.1% of the patients were overweight or obese. Full-term pregnancies were more common in women without obesity. The average weight gain during pregnancy was 11.9 kg. 80.6% had planned pregnancies. Only 16.1% of the participants were tested for infections. Most of the women started taking vitamins, but the level of awareness about the need for comprehensive training was low.

Conclusion: The studyrevealed a highproportion of overweightandobesepregnant women,whichhighlights the need for weightcontrolprograms.Obesity is associatedwith a lowchanceof full-termpregnancyand a highrisk of gestationaldiabetes.Despitethefactthatmanypregnant women are aware of the importance of planning, the level of preventiveexaminationsandtrainingwasinsufficient.Measures are neededtoraisehealthawarenessandprepareforpregnancy,especiallyamongoverweightwomen.

Peculiarities of pregnancy management in Rh-isoimmunisation, assessment of perinatal outcomes after spontaneous delivery and caesarean section

Abstract

The problem of diagnosis and treatment of fetal and neonatal haemolytic disease (FHD) caused by Rhesus-isoimmunisation has not lost its relevance to date, despite a well-organised system of immunoprophylaxis, and remains one of the causes of perinatal mortality and disability. Perinatal mortality due to foetal haemolytic disease (FHD) in our country reaches 15-16 ‰.According to the current clinical recommendations of the Ministry of Health of the Russian Federation ‘Rhesus-isoimmunisation. Haemolytic disease of the fetus’ 2024, after intrauterine transfusion of washed red blood cells to the fetus, abdominal delivery is preferable, which avoids additional traumatisation and distress of the fetus during labour. However, modern studies suggest that the choice of the method of delivery should be based on obstetric indications. Objective. To demonstrate the possibility of choosing the method of delivery based on obstetric indications in pregnant women with GBP after multiple VPKs. Materials and Methods. Two clinical examples of the course of pregnancy complicated by Rh-isoimmunisation are presented. Labour histories, anamnesis data, clinical, laboratory and instrumental investigations were analysed. Results. Two observations of Rh-isoimmunisation complicated by GBP were described in the presented clinical cases, which necessitated fourfold MIC in both pregnant women. The method of delivery did not affect the outcome of labour, both children were born in satisfactory condition. Conclusion. Current management of pregnant women with Rh-isoimmunisation tends to intrauterine EMOLT transfusions at a later date in order to reduce perinatal complications associated with prematurity. In addition, compensation of intrauterine anaemia on the eve of delivery makes it possible to choose the method of delivery based on the obstetric situation.

Toxicity of breast cancer drug treatment during antiretroviral therapy

Abstract

 

Breast cancer, like HIV infection, represents global challenges for healthcare. The combination of HAART (highly active antiretroviral therapy) and chemotherapeutic treatment poses risks of increased toxicity due to drug interactions and comorbidities. The relevance of this topic stems from the need to optimize treatment regimens to reduce toxicity and improve patient survival rates. Objective: To analyze current data on cross-toxicity in HIV-infected patients with breast cancer undergoing HAART and chemotherapy. Materials and Methods. The literature review utilized data from clinical studies focused on breast cancer treatment in the context of HIV therapy. Research was conducted using epidemiological data from the WHO and UNAIDS websites, as well as PubMed and eLibrary databases. Results. The study revealed that the risk of increased cross-toxicity during HAART and antitumor drug use depends on pharmacokinetic and pharmacodynamic interactions. Modern evidence-based online tools for assessing pharmacokinetic interactions and cross-reactions between antiretroviral and cytotoxic anticancer drugs enable the development of personalized treatment plans aimed at mitigating risks associated with these drug combinations. Conclusion: Addressing the issue of increased toxicity during antitumor treatment and HIV therapy is a critical healthcare challenge that requires a personalized approach.

Morphological diagnosis of breast malignancies: from macroscopic imaging to immunohistochemical examination methods

Abstract

The significant prevalence of breast cancer and high mortality from this disease necessitate further scientific research in the field of morphological diagnostics aimed at practicing clinical specialists. In scientific research devoted to oncomorphology, an important role is played by the epithelial-mesenchymal transition – a reversible process of transformation of epithelial cells into mesenchymal ones, accompanied by the disintegration of intercellular connections, loss of cellular polarity, reorganization of the cytoskeleton and changes in gene expression, which increases cell mobility and their invasive capacity. In some types of malignant neoplasms, a program of incomplete epithelial-mesenchymal transition is realized, in which the cells retain some epithelial markers. Mesenchymal-epithelial transition is the process of returning cells to the epithelial phenotype with suppression of mesenchymal genes and activation of epithelial ones. Epithelial-mesenchymal transition is also realized in the oncogenesis of breast cancer. There are several subtypes of breast cancer: less aggressive – luminal subtypes, and more aggressive – triple-negative subtype.

To identify cells that have undergone epithelial-mesenchymal transition, the p66Shc marker can be used, as well as classical markers for determining the cell phenotype – vimentin and E-cadherin. The aim of the study was to substantiate the diagnostic value of complex immunohistochemical examination in breast cancer in light of the phenomenon of epithelial-mesenchymal and mesenchymal-epithelial transitions. For the study, 10 mammary glands were selected after resection for breast cancer, of which 6 samples were of the luminal A subtype, 4 – triple-negative.

The results of the study showed that with the luminal A subtype, the expression of the p66Shc protein is focal, with the triple-negative – diffuse.

Experimental model of inducible breast cancer in the context of dynamic assessment of vasculogenic mimicry: a pilot study

Abstract

One of the fundamental tasks of experimental oncology today is to detail the necessary characteristics of the tumor process, which will help to most accurately determine specific targets for targeted therapy and increase its effectiveness. It has been found that tumor cells under certain conditions have the ability to create limited basal membrane high-permeability, matrix-rich vasculogenic mimicry channels, supporting the growth of tumor tissue and its metastasis. They become the main suppliers of energy substrates. Vasculogenic mimicry correlates with a high degree of tumor malignancy and is more common in patients with an unfavorable course of the disease, making the study of this phenomenon on experimental models a relevant and promising direction for modern scientists. The mechanism of vasculogenic mimicry is not completely understood, but a stable connection between its initiation and hypoxic conditions of the tumor microenvironment is noted. It is under these conditions that the expression of genes responsible for the epithelial-endothelial transition of tumor cells, the production of matrix metalloproteinases increases. Dynamic monitoring of tumor progression in experimental models, serial sections of biomaterial can be carried out thanks to the use of trepanobiopsy - a modern, minimally invasive method that is distinguished by sufficient informativeness, availability, and mobility of equipment, allowing to maintain the viability of laboratory animals.

Morphological assessment of bone tissue changes under the influence of cytostatic therapy: clinical and morphological observation

Abstract

In clinical practice, both in the treatment of malignant neoplasms and in inflammatory diseases of various etiologies, cytostatic drugs are used. Given the statistics that confirm the increase in cases of rheumatic diseases, the use of cytostatic therapy and the study of its side effects are becoming increasingly relevant. Methotrexate is an immunosuppressive agent with antifolate properties, which has been used since the end of the last century and is the "gold standard" in the treatment of rheumatoid arthritis. However, methotrexate has a fairly large list of undesirable drug reactions, among which "methotrexate osteopathy" is noted, presumably associated with a violation of the vital processes of osteocytes, which is accompanied by a decrease in bone density with an increased risk of bone fractures of the lower extremities. The recommended combination of the drug with folic acid, aimed at preventing the antifolate effect of methotrexate does not guarantee the absence of methotrexate osteopathy during treatment with this cytostatic agent. Lack of awareness about this side effect of methotrexate therapy leads to misinterpretation of clinical symptoms and continuation of therapy, exacerbating side effects from the musculoskeletal system. It should be mentioned that the exact mechanism of development of osteoporosis caused by methotrexate is still unknown, which necessitates a comprehensive study of this phenomenon, including a morphological assessment of bone changes. Based on the above, the purpose of this work is to determine the features of the morphological picture of bone tissue after exposure to a cytostatic drug.

"Hydronephrosis associated with abnormalities of the pelvic-ureteral junction: clinic and diagnosis"

Abstract

Introduction. Hydronephrosis associated with uropelvic junction anomalies (N13.0) is a pathological condition leading to progressive urinary outflow obstruction and renal tissue atrophy, which can result in chronic kidney failure and various complications. Early diagnosis and timely intervention are crucial for preventing complications such as urinary tract infections, pyelonephritis, and renal hypertension. The aim of this study is to analyze the clinical presentation and symptoms of hydronephrosis caused by uropelvic junction anomalies, as well as to review modern diagnostic and treatment methods, including surgical intervention. The study utilizes data from relevant scientific publications, prenatal ultrasound screenings, and clinical observations, which help in developing effective patient management algorithms. Hydronephrosis in (N13.0) can be asymptomatic or present with symptoms such as urinary tract infections, abdominal pain, hematuria, and elevated blood pressure. In severe cases, progression of kidney failure and development of urosepsis are observed, requiring surgical correction. Treatment includes conservative observation with regular monitoring and surgical intervention when indicated. Early diagnosis and timely surgical intervention are essential measures to prevent irreversible renal tissue changes and preserve kidney function.

Neuroendocrine breast tumors: difficulties in classifying and diagnosing focal neuroendocrine changes

Abstract

Neuroendocrine tumors are rare oncological diseases. Their diagnosis is difficult, since the classifications are different and not universal. The only way to confirm the diagnosis of neuroendocrine carcinoma (NEC) is histological examination, which requires the expression of neuroendocrine cell markers. A particular difficulty is the diagnosis of primary breast tumors with focal neuroendocrine differentiation, which is caused by the lack of a formulated approach to differential diagnosis and the rare use of appropriate immunohistochemical markers. The aim of the work was to describe the main approaches to the classification of neuroendocrine neoplasms in the context of describing focally differentiated neuroendocrine tumors of the mammary gland using a clinical case as an example. Biopsy material from the breast area with the neoplasm was processed within the framework of routine histotechnics with hematoxylin and eosin staining followed by evaluation using light microscopy, immunohistochemical study was also carried out with antibodies to estrogen and progesterone receptors, to epidermal growth factor 2, to Ki-67, as well as to synaptophysin, chromogranin A and NCAM. This paper presents a brief overview of neuroendocrine neoplasms, methods of their classification, diagnostic methods, data on the pathogenesis of clinical symptoms and pathomorphology.

Abstract

The subareolar lymphatic plexus of Sappey is a part of the dense lymphatic network of the breast, located in the area of ​​the areola of the nipple, formed by deep parenchymatous, intradermal and superficial subcutaneous lymphatic vessels. Numerous literary sources show that over the centuries, the study of the lymphatic system of the breast led various authors to contradictory concepts, which complicated the functional interpretation of the typical and variant lymphangioarchitectonics of the breast, in particular the subareolar lymphatic plexus, first fully described by Constant Marie Philibert Sappey and named in his honor. The study and clinical interpretation of the topographic and anatomical features of the formation of lymphatic outflow links from the breast is extremely important, since lymphogenous metastasis in breast cancer is one of the main ways of tumor dissemination. The aim of the work is a clinical and anatomical substantiation of lymphogenous metastasis of breast cancer, topographically associated with the areolar region. This paper describes the history of the development of views on the lymphangioarchitectonics of Sappey's plexus, describes methodological approaches to its study, concepts describing the formation of the network structure of the plexus, as well as clinical aspects of interpreting the significance of Sappey's lymphatic plexus at the present stage.

On the issue of using the biopsy technique of signaling lymph nodes in breast cancer in patients with GBUZ TOKOD

Abstract

In the statistics of diseases among women caused by oncological aspects, breast cancer is a leading group worldwide. According to the American Cancer Society, the incidence of breast cancer continues to increase by an average of 1% between 2012-2021. Goal. Analysis of the technique of biopsy of signaling lymph nodes in breast cancer in patients with GBUZ TOKOD. Materials and methods. In this work, the object of study was the clinical cases of patients of the Tver Regional Clinical Oncological Dispensary of the Oncological department No. 4 (Department of breast and Skin tumors). Results. In the first case, using the technique of signal lymph node biopsy (BSLU), it was possible to preserve the lymph nodes, while avoiding complications such as lymphostasis, lymph outflow disorders along the cephalic vein, and others. In the second case, it was also possible to preserve the axillary lymph nodes and perform an organ-preserving operation, which minimized the occurrence of complications in the postoperative period. Conclusion. The BSLU method allows surgeons and oncologists to assess the status of lymph nodes, while minimizing the amount of surgery and reducing the risk of postoperative complications.

Recurrent soft tissue sarcoma. A clinical case

Abstract

The treatment of soft tissue sarcoma (SMT) has gone through a long and difficult development path — from amputation operations to complex and extensive interventions aimed at preserving organs. However, regardless of the type of surgery performed, the problem of relapses still remains relevant. Goal. To analyze recurrent soft tissue sarcoma using a clinical case example. Materials and methods. In 2011, patient D. independently discovered a neoplasm in the middle third of her right shin, and after a few months the formation disappeared. 10 years later, an education in the same field appeared. Results. In May 2022, a recurrence occurred in the area of the postoperative scar. In March 2023, another recurrence of the soft tissue area of the right shin. In August 2024, a recurrent lesion was detected in the inguinal-femoral region. Conclusions. Information about these rare cases of recurrent lesions in soft tissue sarcoma contributes to an in-depth understanding of the dynamics of malignant tumors and helps to develop more effective and personalized approaches to the treatment of patients with this pathology.

Primary multiple cancer: a combination of gynecological and other types of cancer. Results of 5-year survival rate.

Abstract

Introduction. Malignant neoplasms are a serious problem in Russia, where more than 400,000 new cases of cancer are registered annually, with high mortality.. The relevance of the study of primary multiple malignant tumors of the female genital organs is increasing against the background of an increase in morbidity and the absence of trends towards a decrease in mortality from genital cancer.

 The goal of this study is to analyze the 5-year survival rate among patients with primary multiple cancers, one of which is gynecological.

Materials and methods. A retrospective analysis of the medical histories of 32 women of different age groups with established primary multiple forms of stage I-IV cancer was carried out. The study analyzed the five-year survival rate of patients with this pathology.

 Results. The survival rate in patients with the studied pathology is on average 76.6%. The average time interval between the detection of the first and second tumors was 5.6 years, which indicates the need for long-term dynamic monitoring of patients, especially postmenopausal patients. The most common localization of the primary process is the ovaries, the uterine body, the cervix and the vulva. The primary cancer of the female reproductive system was most often represented by grade III- IV.

 Conclusions. : This study, despite a relatively small sample (n=32), makes a significant contribution to understanding the course and prognosis of primary multiple malignancies in women.

Features of choriocarcinoma in a man with extragenital lesion: description of a clinical case

Abstract

Choriocarcinoma is a rare, typically very aggressive malignant tumor that develops from trophoblastic cells. There are two main types of choriocarcinoma: gestational (associated with pregnancy) and nongestational (outside of pregnancy). The gestational form occurs only in women and is typically linked to pregnancy, whereas the nongestational form affects both sexes. In this case, choriocarcinoma is a type of germ cell tumor that most commonly arises from the cells that form the human reproductive system. In men, it typically develops at a young age and affects the testes.

Previous cases of unique clinical case of extragenital liver involvement with choriocarcinoma in a 36-year-old male. This disease presentation is rare, which complicated both the diagnostic process and the selection of treatment methods. The patient was admitted to the hospital with signs of rapidly developing multiple organ failure, which led to the diagnosis being confirmed post-mortem through histological and immunohistochemical studies. Autopsy material was analyzed, and accompanying medical documentation was reviewed, along with all 11 cases of liver choriocarcinoma published in the international literature to date.

Justification of the use of the preparation ‘Fibrinostat’ for stopping bleeding from the liver wound and the effect of low-intensity laser radiation on the organ parenchyma

Abstract

В современной хирургии важное значение имеют методы гемостаза и улучшения регенерации тканей. Особенно это важно при операциях на печени в связи с анатомическими особенностями сосудистого русла этого органа. Цель. Изучить эффективность гемостаза при ранениях печени с использованием препарата «Фибриностат» и низкоинтенсивного лазерного излучения (НИЛИ). Материалы и методы. Опыты проведены на 16 белых крысах, у которых моделировали рану (0,5х0,3х0,2 см) на левой доле печени. Вскрытие проводили через 28 суток после операции и оценивали состояние раневой поверхности с последующей микроскопией места раны печени. Результаты. Результаты показали, что все животные хорошо перенесли операции. На 3-и сутки после вмешательства у крыс не наблюдалось выпота, признаков кровотечения и гематом. В группе с «Фибриностатом» наблюдалась выраженная инфильтрация гранулоцитами, тогда как в группе, подвергшейся воздействию низкоинтенсивного лазерного излучения, она была выражена меньше. В более поздние сроки наблюдались различия в макрокартине печени — в группе с «Фибриностатом» преобладал адгезивный процесс. Заключение. Таким образом, «Фибриностат» продемонстрировал хорошие гемостатические, а в сочетании с низкоинтенсивным лазерным излучением регенераторные свойства печеночной ткани.

Severe Foot Injury. Amputation or Reconstruction? Review

Abstract

Relevance. Severe foot injuries resulting from high-energy trauma remain a significant challenge in both military and civilian surgery due to high disability rates, treatment costs, and the complexity of decision-making between amputation and reconstruction. Objective. To evaluate modern treatment approaches for severe foot injuries and draw conclusions based on a literature review. Materials and Methods. A review of scientific publications was conducted using data from PubMed, Web of Science, e-Library, and ScienceDirect, focusing on injuries of the forefoot, midfoot, and hindfoot, as well as combined injuries. Results. The choice of treatment strategy depends on the extent of tissue, vascular, and nerve damage. Reconstruction using microsurgery, negative pressure therapy, and 3D printing is preferable when functional recovery is possible, whereas amputation is justified in cases of irreversible damage and leads to faster rehabilitation. Limb injury severity scores (e.g., MESS) assist in decision-making but do not guarantee an accurate prognosis. Conclusion. The issue of amputation versus reconstruction in severe foot injuries remains unresolved within the trauma surgery community. An individualized approach, based on the clinical presentation, surgeon’s experience, and available resources, remains crucial. Further research is needed to develop precise treatment protocols.

Features of repositioning in pediatric trauma: key aspects

Abstract

Fractures in children are frequent injuries that require a comprehensive approach to treatment.

Repositioning - restoration of anatomical integrity and correct bone position - is an important part of fracture treatment and has a significant impact on further recovery and functional status of the limbs [2].

The aim of the study was to analyse the types of injuries that required repositioning in childhood.  The study was carried out on the basis of the trauma centre of the DOKB in Belgorod. The study group included 227 cases of various types of repositioning in the course of a particular injury. The age and sex of the patients in the study group, the localization of the injury and the type of repositioning, the need for hospitalisation and recurrence were analysed. The study group included 161 (70.9%) boys and 66 (29.1%) girls.

The largest number of cases was associated with fractures of the upper extremities. In the majority of cases 71 (34.6%) were injuries of both forearm bones.

 The most frequent injuries were sustained in the age period from 6 to 12 years - 155 cases (68.3%). This is the age when the child has little or no parental control and self-control is far from perfect.

 Closed reposition as a method of treatment for injuries in children and adolescents is a priority, but in our study, in 7 cases (3.1%), repeated repositioning was necessary - open repositioning was performed after closed repositioning.

Surgical correction of the posterior abdominal wall using the Tar technique

Abstract

Each year, the frequency of hernias is more than 20 million cases, and postoperative ventral hernias are found in 22% of all hernias of the anterior abdominal wall. Currently, methods of dissociation of muscular-aponeurotic structures, which are called separation plastic surgery methods, are of great interest. Anterior and posterior separation plastics have been identified in the global surgical community. The most common and relevant method at present is the method of back separation plastics. Goal. To evaluate the results of surgical interventions (posterior separation plastic surgery, TAR method) for postoperative hernias. Materials and methods. In the course of the work, a statistical analysis of patients' indicators was carried out before surgery and in the postoperative period. Surgical interventions were performed on the basis of the surgical department of GBUZ GKB No. 7 in Tver. To evaluate the surgical technique, the medical history data of 12 operated patients was used. Results. No deaths were detected among the operated individuals. In 11 patients (91.7%), surgery was performed without surgical complications, with a characteristic increasing positive dynamics in the postoperative period. Conclusions. The death rate based on the analyzed surgical interventions is 0%, the incidence of complications in the form of PE is 0%, complications were detected in only 1 patient out of 12 (8.3%), which positively characterizes the quality of the intervention and indicates a detailed study of the medical history of each patient.

Mortality analysis among patients with perforated ulcer, depending on the duration of hospitalization

Abstract

A perforated ulcer is a serious complication of peptic ulcer disease that requires emergency surgical treatment. It is logical to assume that the results of treatment and the mortality rate among patients in this group are influenced by many factors, including the timing of hospitalization. In 2023, the proportion of patients diagnosed with a perforated ulcer who were admitted to the hospital later than 24 hours after the onset of the disease in different regions of the Russian Federation ranged from 21% to 39.4%. The purpose of the study. To conduct a mortality analysis taking into account the duration of hospitalization among patients diagnosed with perforated gastric and duodenal ulcers admitted to GBUZ VOKB No. 1 for 2020-2023. Materials and methods. We studied 48 medical records of patients diagnosed with perforated ulcer who were hospitalized in GBUZ VOKB No. 1 for 2020-2023, of which 15 were fatal. Results. During the systematization of the material, two groups were identified: those with hospitalization up to 24 hours from the onset of the disease (early) and later than 24 hours (late hospitalization). The subjects of comparison of the two groups are: mortality rate, causes of death. The result of the analysis is a direct relationship between the duration of hospitalization and the mortality rate. Conclusion. In the case of a perforated ulcer, the timing of hospitalization is an important factor that determines the clinical picture, decompensation of concomitant pathology, and the presence of complications, which affects the risk of death.

Study of differences in the biomechanical properties of samples of postoperative scars

Abstract

It is known that any tissue of the human body is capable of displaying plastic properties, i.e. it can stretch or contract [1,2]. Under the influence of the force that acts on the tissue, tissue deformations occur, represented by the following stages: 1 the zone of initial deformation; 2 the zone of plastic deformation; 3 the zone of terminal deformation; 4 the average critical point; 5 the rupture zone. [3]. When performing operations on the anterior abdominal wall after suturing the wound and reducing the load on the tissue, stress relaxation phenomena occur: the tissue demonstrates the properties of elasticity and firmness. These processes occur due to the peculiarities of the histoarchitectonics of tissues of different topographic anatomical regions [4]. The presented processes have found a response in the scientific literature, however, no studies have been found on the differences in the biomechanical properties of postoperative scars. Goal. To identify differences in the integrity of the postoperative scar in the studied samples in the conditions of an infected and clean wound. Materials and methods. For this purpose, a study of 180 samples of postoperative scars previously operated on at the VSMU (Voronezh) rats, on a bursting electromechanical machine in accordance with GOST 8847-85 in the laboratory of KSMU (Kursk). Results. The tensiometry of samples obtained using various methods of hernial defect plastic (plastic with local tissues, PP (polypropylene mesh), DBT (decellularized bioengineered graft) showed that in the samples of "clean" and "infected" wounds with a standard load (16 N) in the longitudinal and transverse directions, the highest extensibility was observed in the case of using DBT. Conclusions. These data indicate that the use of DBT promotes the formation of scar tissue with high-quality mechanical properties, which may be a key factor in improving the effectiveness of postoperative recovery and reducing the risk of complications.

Variants of Hepatic Arterial Supply and Their Impact on Surgical Strategy

Abstract

The arterial blood supply of the liver is a crucial aspect of surgical anatomy, significantly influencing the strategy of surgical interventions. The variability of the vascular system is determined by embryonic development and can substantially complicate surgical manipulations, increasing the risk of intraoperative injuries and postoperative complications. Understanding anatomical variations of the hepatic arteries is essential for successful surgical planning, particularly in transplantation, oncology, and reconstructive surgery. Objective. To analyze anatomical variations in the arterial blood supply of the liver and their impact on surgical strategies in various types of operations. Materials and Methods. A literature review was conducted, including an analysis of anatomical and clinical studies on hepatic artery variations and their significance in liver, pancreatic, and biliary surgery. Results. It was found that the most widely used classifications (Michels, Hiatt) allow for the systematization of hepatic artery variations. Vascular anomalies influence techniques in liver transplantation, pancreaticoduodenal resection, gastrectomy, and gastric fundoplication. The use of modern imaging techniques, including MSCT angiography and 3D reconstruction, enables the identification of variations during preoperative planning, reducing the risk of complications.. Conclusion. Anatomical variations of hepatic arteries have clinical significance in surgical planning and require an individualized approach. The use of advanced diagnostic methods contributes to improved surgical outcomes.

Study of vital signs of rabbits after hernioplasty for median abdominal hernias

Abstract

Introduction. Hernias of the anterior abdominal wall reach 7% of all surgical diseases. The risks of developing postoperative complications after auto- or alloplasty of the hernial orifice are very high.

The goal of this study is to study the main indicators of the vital activity of rabbits with various hernioplasty techniques in order to establish possible postoperative complications in an experiment.

Materials and methods. We conducted an experimental study on 21 rabbits. Then we divided the rabbits into three groups (n=7) depending on the hernioplasty method (control group – autoplasty according to K.M. Sapezhko, 1 main group – prosthetic hernioplasty “sublay preperitoneal”, 2 main group – prosthetic hernioplasty “IPOM”). We studied the main vital signs of the animals to establish the pain symptom: body temperature, respiratory rate, and pulse before the operation, immediately after the surgery and on the 3rd day after its completion.

Results: The body temperature indicators after the operation in the study groups were approximately the same at all stages of the experiment. The respiratory rate in the control group rabbits on the third day after the operation was minimal than in the rabbits of the main groups, and the pulse was maximal. Non-traumatic rupture of the bladder (clinical case)

Abstract

Bladder injury is one of the most severe injuries to the organs of the genitourinary system and occupies a special place in the problem of injury (1-3% of cases among injuries to other organs). Based on the anatomical structure and localization of the bladder, we can say that it is a hollow muscular organ located in the intraperitoneal space and thus protected from external influences, and therefore injury. Goal. to analyze a non-traumatic rupture of the bladder using the example of a clinical case. Materials and methods. In the course of this study, a clinical case of the urology department at the Tver Emergency Hospital was analyzed. Results. At the time of the examination, the patient noted abdominal pain, most pronounced in the lower regions (suprapubic region). An ultrasound examination and a CT scan were performed, which revealed a large amount of fluid in the abdominal cavity. After examination by the therapist and the surgeon, a preliminary diagnosis was made – "Closed abdominal injury, intraperitoneal rupture of the bladder," after which it was decided to urgently close the rupture with drainage of the abdominal cavity. Conclusions. Intraperitoneal bladder injury is a rather dangerous injury that can be accompanied by a number of complications. The complexity of the clinical picture makes diagnostic examination difficult.

Risk factors and frequency of postoperative atrial fibrillation after coronary artery bypass grafting in conditions of artificial circulation

Abstract

Coronary heart disease occupies a leading place in the structure of mortality from diseases of the cardiovascular system. Coronary artery bypass grafting is the gold standard of surgical treatment for coronary artery disease. One of the most common complications after coronary bypass surgery is cardiac arrhythmia [1]. Objective: to identify risk factors and the incidence of early postoperative atrial fibrillation (AFF) after coronary artery bypass grafting in conditions of artificial circulation. Materials and methods: the study involved 96 patients who had no history of atrial fibrillation. Taking into account the coronary angiography data, all patients underwent coronary artery bypass grafting in IC conditions. Results: postoperative atrial fibrillation was registered in 30 men (31.25%) in the first two days after surgery, aged 58 to 75 years with an average BMI of 30 kg/m2, 18 patients have a history of AMI combined with type 2 diabetes mellitus. According to the ECHO-KG data, all patients had an enlargement of the left atrium cavity. Conclusion: postoperative atrial fibrillation is a common complication of CABG in IC conditions. It occurs in 30% of cases, and the risk factors for its development are: male, BMI >30 kg/m2, age over 65 years, diabetes mellitus, dilation of the left atrium.

Basic approaches to surgical treatment of thyroid diseases

Abstract

. The most common pathology of the thyroid gland is macrofollicular and microfollicular colloid goiter, malignant neoplasms, thyroiditis, as well as benign neoplasms of the thyroid gland (adenoma, angioma, lymphoma). Surgical methods are often used to treat thyroid pathology.
  Objective: to study the basic approaches to surgical treatment of thyroid diseases.
  Materials and methods: a review of data in PubMed, ScienceDirect, eLIBRARY was performed using the keywords "endocrinology", "thyroidectomy", "hemithyroidectomy", "thyroid gland", "thyroid cancer", "intraoperative neuromonitoring".
Of the initially identified 56 sources, 15 were included in the review.
  Results: total and subtotal thyroidectomy, hemithyroidectomy are the most common surgical interventions.
Current guidelines recognize hemithyroidectomy as an acceptable method of initial treatment for cytologically indeterminate nodules and papillary thyroid carcinoma. According to the recommendations of the American Thyroid Association and the National Comprehensive Cancer Network, if thyroid nodules less than 4 cm in size are detected without concomitant signs of malignancy, it is advisable to consider hemithyroidectomy. Recently, active surveillance of papillary thyroid microcarcinomas smaller than 100 mcg in the absence of signs of malignancy and metastases to the lymph nodes has become an increasingly acceptable treatment method.
  Conclusion: the choice of surgical tactics depends on the nature of the thyroid disease, the prevalence of the pathological process and should take into account the capabilities of modern surgery and the patient's wishes.
