PHARMACOGENETIC FEATURES OF THE USE OF FLUVOXAMINE IN THE TREATMENT OF DEPRESSIVE DISORDER
Abstract
The article discusses the features of the use of fluvoxamine for the treatment of depressive disorder from the point of view of pharmacogenetics. The aim of the study was to evaluate the efficacy of fluvoxamine in patients with various polymorphic variations of the CYP2D6 gene and suffering from newly diagnosed depressive disorder. Methods. For the study, 48 patients with newly diagnosed DR and who had not previously received antidepressant therapy were selected. Fluvoxamine was prescribed at a dosage of 100 mg/day with the possibility of dose adjustment for a period of 3 weeks. The effectiveness of the drug was assessed by evaluating the results of a survey on the Galminton Scales (HAMD) and the Hospital Anxiety and Depression Scale (HADS). Polymorphisms were determined using genotyping. Results. It has been established that patients with heterozygous GA variation have a slower metabolism of the drug, which is expressed in greater resistance to treatment, while GG homozygotes, which are represented in the population in a much smaller number, demonstrate a good therapeutic response. The results of this study can be used to select tactics for prescribing antidepressants.
WATERJET DISSECTION OF HARD TO ACCESS LIVER SEGMENTS
Abstract
Surgical oncology of the liver over the past decades has significantly increased the surgical aggressiveness in relation to tumors. It has become possible to perform surgical interventions on hard-to-reach segments of the liver, thanks to the progress in the field of pharmacology and the improvement of medical equipment, for example, the advent of water-jet vascular-saving dissectors.
The main reasons for the complexity of the posterior-superior segments are poor visualization, limitations in manipulation, as well as the proximity of large hepatic veins at their confluence with the inferior vena cava. For these reasons, parenchyma-sparing resections of the posterior-superior segments of the liver, according to the IWATE scale, are on the same level of complexity as extensive resections.
The advantages and effectiveness of water-jet vessel-sparing dissection of hard-to-reach segments of the hepatic parenchyma during enucleation of liver hemangiomas in segments II and IVA were evaluated. Resection using a water scalpel required less time and suture material to achieve final hemostasis of the wound surface of the liver stump. The study of the dynamics of scar formation showed that the water-jet dissector leads to the formation of a wider area of damage when crossing due to the impregnation of the liver parenchyma with a working solution, but, despite this, the disappearance of the scar occurs earlier and, as a result, the patient is discharged faster.
THE STATE AND TRENDS IN THE INCIDENCE OF MENTAL DISORDERS IN THE KURSK REGION
Abstract
Relevance. On the territory of the Russian Federation, about 40% of the population have signs of mental activity disorders, which affect all segments of the population, including children [1, 2]. These disorders entail serious consequences for the social spheres of human life. As a result, monitoring the incidence of mental disorders in dynamics remains an urgent task. The purpose of the study: to analyze the state of the incidence of mental disorders in the Kursk region and make a forecast for 2021-2022. Materials and methods of research. Based on the data of the Federal State Statistics Service, the Unified Interdepartmental Information and Statistical System (EMISS), an analysis of the state of general and primary morbidity of mental disorders in the Kursk region for 2011-2020 was carried out. The following methods of analysis were used: the method of averages, tabular, calculation of indicators of dynamics series.
Results. According to the results of the analysis, it was found that the direction of the indicators of general and primary morbidity of mental disorders changed significantly during the analyzed period. Thus, the average growth rate of the overall incidence of mental disorders in the Kursk region was 0.8%, while against the background of a dynamic decline in indicators in 2013-2016, a significant increase was noted in 2017-2019. During the assessment of primary morbidity, a general trend was established to reduce morbidity by an average of 2.0% over the analyzed period. Among the possible reasons for the increase in the indicators of general and primary morbidity can be identified improvements in the organization of psychiatric care, changes in diagnostic criteria, etc. The reasons for the decrease in morbidity include improvements in the system of development of preventive and rehabilitation measures, improvement of pharmacotherapy and drug provision, reduced availability of medical care, etc.
The calculation of the forecast of possible morbidity showed a downward trend in both the general (2667.1 and 2688.4 cases per 100 thousand people) and primary morbidity (218.9 and 214.5 cases per 100 thousand people) of mental disorders in 2021-2022. Conclusions. Thus, dynamic changes in the indicators of general and primary morbidity of mental disorders indicate the need for a systematic analysis aimed at timely identification of alarming trends in the field of mental health of the population.
STUDY AND HYGIENIC ASSESSMENT OF THE ACTUAL NUTRITION OF STUDENTS
Abstract
Rational nutrition is an important component for maintaining the health of every person, including students, as they constantly need to keep a high concentration of attention and efficiency. Recent statistical investigations show that diseases provoked by disturbed nutrition tend to increase. Purpose: to research and comparatively evaluate the actual nutrition of students, using the examples of high education institutions in Uzbekistan and Russia. Methods: to study the problem was created the questionnaire, including questions that allow to determine the quality of nutrition and the regimen of students. 206 students from Russia (101) and Uzbekistan (105) were interviewed. Results: an analysis of the received responses shows that almost all students in both countries have poor-quality and unbalanced nutrition. There are also certain differences in some aspects of nutrition among students from different countries, mainly due to geographical location, cultural features and differences in the educational process. It was determined that due to disturbed nutrition, students can be at risk of developing diseases of the digestive system and other nutrition-related diseases. Conclusion: measures are needed that will help in solving this problem and will help prevent the increase in diseases associated with the gastrointestinal tract, not only among students, but also among the entire population. Scientific novelty lies in the comparison of the data obtained from students of universities in the two countries, which allows a more comprehensive assessment of the problem of food hygiene.
RESULTS OF A MORPHOLOGICAL STUDY OF THE REGULATION OF POSTNATAL OSTEOGENESIS
Abstract
Objective: to study the morphological features of bone tissue regeneration in postnatal ontogenesis under the condition of implantation of corral granules.
Methods: experimental. The study was performed on laboratory animals - rats, in which coral granules were implanted in the area of the bone defect. They were removed by overdosing of drugs for inhalation anesthesia. The criterion for evaluating changes was descriptive morphology.
Results. Changes were revealed: the cortical plate or compact substance was thinned for a considerable length. At the same time, there is some division of it into two zones. It should be noted that in most osteons of the cortical part of the bone, an expanded central channel is detected, thus a positive, so-called "porosity symptom" or "cortical osteopenia".
Conclusions. Thus, the conducted experiment showed the presence of qualitative and quantitative changes in the structural organization of bone tissue and a change in the rate of its neoplasm.
OXIDATIVE STRESS IN THE CURE OF CHRONIC OSTEOMYELITIS WITH THE APPLICATION OF MARAL PANTS
Abstract
Relevance: The recurrence rate in acute osteomyelitis ranges from 20% to 30%, and disability of patients reaches 25% of cases. Complications in the treatment of this pathology can be traced in 14-32%, and according to some sources in 54.6% of patients.Goal: Determination of the effect of deer antlers in combination with amikacin on the course of chronic osteomyelitis.Materials and methods: The study was conducted at the Research Institute of Experimental Biology and Medicine on laboratory rats of the Wistar line with a mass of 270-290 grams. The study was carried out on 180 laboratory animals, which were divided into 5 groups: 2 control and 3 experimental: in the first control group, treatment was not carried out, in the remaining groups, 2-stage surgical rehabilitation was performed. In the first experimental group, powdered deer antlers with a particle size of 200-250 microns were introduced into the formed cavity before it was filled. In the second experimental group - a semi-synthetic antibiotic amikacin at a dose of 15 mg/ kg, and an inorganic component - calcium hydroxyapatite until the cavity is completely filled. In the third experimental group - amikacin at a dosage of 15 mg / kg and maral antlers in a ratio of 2:1, the remaining volume of the cavity was filled with hydroxyapatite. The effectiveness was evaluated by evaluating the indicators of oxidative stress and X-ray methods of investigation on 7, 14, 28, 60, 90- the e and 120th days of the study. Research results: in the 1st control group, there was no decrease in the indicators of DFNG and MDA during the entire experiment. In the 2nd control group, the indicators decreased by 53.8% and 28.12% by the 120th day. In the 1st experimental group, the indicators of MDA and DFNG decreased by about 6%, by the 120th day. In the 3rd experimental group, the indicators returned to normal by the 90th day. Conclusions: The most effective method of relieving chronic osteomyelitis is a technique based on the use of surgical sanitation, amikacin at a dosage of 15 mg / kg / day and deer antlers in a ratio of 2:1.

SERUM ESTRADIOL TO TESTOSTERONE RATIO AS A PREDICTOR OF DRUG-INDUCED QT INTERVAL PROLONGATION IN MALE PATIENTS
Abstract
Aim. To establish the prognostic value of the serum estradiol to testosterone (E/T) ratio for the development of drug-induced long QT syndrome (LQTS).
Materials and methods. 59 male patients were examined, mainly with ischemic heart disease, hypertension and cardiac arrhythmias, 29 (49.2%) of them with drug-induced LQTS and 30 (50.8%) with normal values of the QT interval while receiving antiarrhythmic therapy.
Results. Patients with drug-induced LQTS have higher E/T ratio values compared to patients without LQTS (median 9.18 vs 4.72, p<0.001). An E/T ratio value ≥6.08 can be used to predict the development of drug-induced LQTS in male patients taking amiodarone and sotalol (OR=3.43 [95% CI 2.27; 5.19]).
Conclusion. Our data indicate the important physiological role of sex hormones, in particular, estradiol and testosterone, as well as their ratio in the genesis of drug-induced LQTS in male patients.
CORRECTION OF ENDOGENOUS RISK FACTORS FOR CORONARY HEART DISEASE
Abstract
Relevance: for a long time, coronary heart disease (CHD) has been the main cause of death of the population worldwide [1]. In 2016, for the first time, "revolutionary" lipid-lowering drugs, monoclonal antibodies- inhibitors of the PCSK9 protein were used in practical medicine, allowing achieving the target values of low-density lipoprotein cholesterol of LDL cholesterol in more than 90% of patients [2]. Studies by various scientists have proved that the concentration of PCSK9 in women is higher than in men [3]. In addition, with age, the level of plasma PCSK9 in men decreases, and in women, on the contrary, increases, which is most likely due to the level of estrogens in the body [4].Objective: to evaluate the effect of estradiol on the expression of PSCK9 in patients with IHD Materials and methods: 39 men (average age 60.95±5.89 years) participated in the study IHD sufferers from a group of very high cardiovascular risk. Prior to the start of the study, for 8 weeks, all patients took a combination of the maximum tolerated dose of HMG-CoA Reductase inhibitor (Atorvastatin 80mg 1p / day) with an inhibitor of intestinal cholesterol absorption (Ezetemib) and did not reach the target values of LDL cholesterol < 1.4 mmol. After that, Alirocumab 150mg 1 injection per 14 days was added to the treatment (PRALUENT, SANOFI, France), followed by monitoring of LDL cholesterol and estradiol levels every 4 weeks. The median of the study was months. The program SPSS 23.0 (IBM USA) was used for statistical data processing. Results: at the start of the study, the average LDL cholesterol level was 2.28±0.08 mmol, the estradiol level was 149.13±87.3 pg/ml. A large variation in estradiol indicators (standard deviation) was associated with individual characteristics of the endocrine status of men. The use of Alirocumab showed high statistical significance after 4 weeks of using the drug (2 injections), the level of LDL cholesterol decreased by 34.2% and amounted to 1.5±0.1 mmol (p<0.001), the level of estradiol in the same control increased by 8.39% and did not show statistical significance (p=0.64). After 6 months of treatment, the LDL cholesterol level decreased by 43.86% 1.28±0.14 mmol (p<0.001), the estradiol level increased by 15.15% after 6 months of treatment (p=0.47). At the end of the study, after 12 months of treatment with PCSK9 inhibitors, the level of LDL cholesterol decreased by 49.56% from the baseline level and amounted to 1.15±0.15 mmol (p<0.001); the target values of LDL cholesterol reached 97.43% of patients (n=38). The estradiol level after 12 months of treatment with PCSK9 inhibitors was 162.64±86.14 pg/ml, increased by 19.37% from the baseline values and showed no statistical significance (p=0.39). In the correlation analysis, no significant relationship was found between the dynamics of LDL cholesterol reduction and an increase in estradiol levels. Conclusions: the revealed features of changes in endogenous risk factors in patients with coronary heart disease expand modern ideas about the role of PCSK9 inhibitors in the context of correction of atherogenic lipoprotein fractions.
BIOCHEMICAL FEATURES OF THE EFFECT OF INTRAHEPATIC ADMINISTRATION OF DRUGS ON POST RESECTION LIVER REGENERATION
Abstract
Relevance. Liver diseases affect about 500 million people in the world, of which 1 million die annually. Liver resections are an effective way of treating both a tumor lesion of the liver and many other non-tumor diseases, however, with an increase in resectability, the risk of developing post-resection liver failure increases annually. Thus, the search for new methods aimed at activating the reparative regeneration of the liver remains an urgent task.
The aim of the study was to improve the results of treatment of postresection hepatic insufficiency by intrahepatic administration of ademethionine and cyanocobalamin in an experiment.
Materials and methods. The experiments were performed on 72 mature male Wistar rats, which were divided into 4 groups. In all groups, a typical resection of ~ 70% of the liver volume was performed, according to the model proposed by G. Higgins and R. Anderson. In the 1st control group, prevention of postresection hepatic insufficiency was not carried out; in the 2nd control group, 0.9% sodium chloride solution was intrahepally administered; in the 1st experimental group, cyanocobalamin (patent RU 2720451); in the 2nd experimental group, ademethionine. Blood sampling was performed on the 1st, 7th and 14th days after surgery, and the indicators of cytolytic, cholestatic syndromes, as well as hepatic cell insufficiency syndrome were evaluated. Statistical processing was performed using the Statistica 6.0 program.
Results. By the 14th day after the operation in the 1st experimental group, the indicators of cytolytic and cholestatic syndromes in 92.04± 6.14% of animals, liver cell insufficiency syndrome in 83.12± 3.09% of animals were normalized, in the 2nd experimental group, this indicator was lower by 27% and 21%, respectively. In the control groups, normalization of biochemical parameters was observed in less than 70% of animals.
Conclusion. Intraoperative administration of ademethionine or cyanocobalamin during liver resection are effective ways to increase the reparative potential of the liver. The most optimal method is intrahepatic administration of cyanocobalamin, which significantly reduces the indicators of cytolysis, cholestasis and hepatic cell insufficiency syndromes in more than 75% of operated animals.

PHARMACEUTICAL CARE FROM THE POSITION OF A THREE-LEVEL ANALYSIS
Abstract
Target. Study of Pharmaceutical Care (PC) from the perspective of a three-level analysis.
Methods. In the course of a three-level analysis, from the position of a product, the essence of the PC, the actual product, the added product.
Results. At the first level of analysis, the essence of the PC was considered: PC is a system for providing drug treatment that provides for a set of measures to improve the quality of life of the patient, which provides for the interaction of the pharmacist-patient-physician and their responsibility for the patient's health based on the actions and decisions made by the doctor and a pharmacist, and the main result of the PC is the preservation of health and improvement of the quality of life of patients. At the second level, the elements of the PC are defined from the perspective of the actual product: 1) the consumer properties of the PC include the timeliness, availability and quality of drug treatment; 2) the category of "quality" includes the functioning of the logistics service, pricing, assortment policy, the absence of falsified, low-quality drugs, the presence of highly qualified personnel; 3) the "design" of the PC includes the appearance of the pharmacist - his neatness, the presence of a uniform, compliance with the code of corporate culture, etc.; 4) “packaging” of the PC implies the correspondence of the personal (empathy, tact, politeness, etc.) and professional qualities of the pharmacist to the requirements of buyers; 5) “brand name” (patients are often adherents of not only one manufacturer, but also one specialist pharmacist, and turn to him specifically for help). ANALYSIS OF PHARMACOLOGICAL THERAPY OF A NEW CORONAVIRUS INFECTION IN KSMU STUDENTS
Abstract
With the appearance of the Omicron strain, the prevalence of coronavirus infection among a younger group of the adult population has increased significantly [1-3], which means that statistical data on the incidence of a new coronavirus infection, their absolute and relative growth determine the relevance of improving approaches to CoViD-2019 therapy within the framework of current temporary clinical recommendations. According to the results of the study of this sample, it can be said that special attention deserves the fact that most cases of transmission of a new coronavirus infection in the studied population are not confirmed by PCR testing for the detection of SARS-CoV-2 RNA, which may affect the incidence statistics, as well as the treatment protocol in each individual clinical case. The highest frequency of use in the treatment of NKVI was observed in such drugs as umifenovir (27.9%), gripferon in the form of nasal drops (27.9%), vitamin C (60.5%), vitamin D (44.2%), and vitamin B (20.9%). The therapy of a new coronavirus infection used by students corresponds to the schemes described in the temporary clinical recommendations for the treatment of NCVI [2], and is also effective.
