The aim of the study was to assess the effect of MS on the indicators of autonomic regulation in hypertension. 95 patients with hypertension were examined, 47 of them men, 48 women; the average age was 58.7  6.1 years. Patients were divided into two groups depending on the presence or absence of MS: the 1st group (comparison) included people with hypertension without MS (n = 49), the 2nd (main) - with hypertension and MS (n = 46). The analysis of heart rate variability (HRV) was applied according to a cardiointervalogram recorded over a short (five-minute) period of time. HRV indices were significantly lower in patients with hypertension with MS than in those without MS. In the presence of MS in patients with hypertension, vegetative disorders are aggravated in the direction of relative sympathicotonia, combined with parasympathetic insufficiency against the background of reduced functional capabilities of the heart, which forms unfavorable prognostic sings.&lt;/p&gt;</abstract><kwd-group xml:lang="en"><kwd>arterial hypertension</kwd><kwd>metabolic syndrome</kwd><kwd>autonomic regulation</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>артериальная гипертония</kwd><kwd>метаболический синдром</kwd><kwd>вегетативная регуляция</kwd></kwd-group></article-meta></front><body>&lt;p&gt;&lt;strong&gt;Relevance&lt;/strong&gt;. The relevance of AH and MS is beyond doubt. The significant prevalence and great medical and social significance of hypertension, its high level of comorbidity are well known [1-3]. Emerging pathological changes on the part of organs and systems in patients with hypertension are aggravated against the background of MS [4-7]. At the same time, the features of autonomic regulation in patients with comorbid hypertension and MS remain insufficiently studied.&lt;/p&gt;&#13;
&lt;p&gt;&lt;strong&gt;The purpose of the study.&lt;/strong&gt; To evaluate the effect of MS on the indicators of autonomic regulation in patients with hypertension.&lt;/p&gt;&#13;
&lt;p&gt;&lt;strong&gt;Materials and methods.&lt;/strong&gt; 95 patients with hypertension were examined, 47 of them men, 48 women; the average age was 58.7  6.1 years. HRV analysis by cardiointervalogram was used to assess the state of autonomic regulation [8,9].&lt;/p&gt;&#13;
&lt;p&gt;&lt;strong&gt;Results.&lt;/strong&gt; A comparative analysis of HRV in the time domain revealed differences in the groups for most parameters. Thus, SDNN, RMSSD and pNN50 were significantly lower (p 0.05) in patients with AH and MS than in AH, indicating a more pronounced tension of regulatory systems against the background of MS. In the frequency domain, differences between the groups in HF and LFn values (p  0.05) were revealed, reflecting an increase in sympathetic influence in hypertension with MS. Intergroup differences were obtained in the frequency of sympathicotonia: 23 (46.9%) patients patients of the main group and in 34 (73.9%)  the comparison group (2 = 7.19; p  0.001) and vagotonia: in 22 (44.9%) and in 7 (15.2%), respectively (2 = 10.81; p  0.001).&lt;/p&gt;&#13;
&lt;p&gt;&lt;strong&gt;Conclusions.&lt;/strong&gt; In patients with hypertension with MS, HRV indicators are significantly reduced than in hypertension, which indicates a significant weakening of the adaptive capabilities of the body. MS, occurring comorbidally with hypertension, exacerbates autonomic disorders in the direction of sympathicotonia, combined with parasympathetic insufficiency against the background of reduced functional capabilities of the heart. These disorders form unfavorable prognostic signs, which must be taken into account for carrying out therapeutic and preventive measures in this category of patients.&lt;/p&gt;</body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Попов, В.В.
