Infants under one year of age, especially unvaccinated children under 3 months, are the most vulnerable group, often experiencing atypical disease progression and severe complications like pneumonia. Objective. identify the specific features of pertussis complicated by pneumonia in infants.
Materials and methods.A retrospective analysis was conducted on 24 medical records of patients aged 1 month to 1 year with laboratory-confirmed pertussis and pneumonia, treated in a hospital from 2016 to 2025. Results. The average patient age was 5 months, with 42% being under 3 months old. In 100% of cases, pertussis was complicated by pneumonia. Mixed infections (viral, bacterial, CMV) were detected in 58% of children. Respiratory failure in 79% of cases was caused by broncho-obstructive syndrome. A characteristic laboratory finding was lymphocytic leukocytosis, with hyperleukocytosis (50109/L) in 18% of young infants serving as a poor prognostic marker.
Conclusion. Pneumonia is the dominant complication of pertussis in infants. Children in the first three months of life are at the highest risk of severe illness. A white blood cell count exceeding 50109/L is a criterion for extremely severe cases and requires careful monitoring.

<kwd-group xml:lang="en"><kwd>pertussis</kwd><kwd>infants</kwd><kwd>pneumonia</kwd><kwd>broncho-obstructive syndrome</kwd><kwd>hyperleukocytosis</kwd><kwd>cytomegalovirus infection</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>коклюш</kwd><kwd>дети до года</kwd><kwd>пневмония</kwd><kwd>бронхообструктивный синдром</kwd><kwd>гиперлейкоцитоз</kwd><kwd>цитомегаловирусная инфекция </kwd></kwd-group></article-meta></front>

<back><ref-list><ref id="B1">1.<mixed-citation>Xu, M Lei, Y & Tan, K & Deng, J.
