Grade 2 VUR predominated (86.7%). Bacteriuria was confirmed in only 15.6% of cases. Furagin was the most frequently prescribed agent (20.0%), followed by co-trimoxazole (8.9%) and nitroxoline (6.7%). No statistically significant association was found between the choice of co-trimoxazole and confirmed bacteriuria (p=0.11), indicating an empirical approach. Analysis of compliance with the 2021 clinical guidelines revealed that in half of the cases, therapy was not fully rational: data on uropathogen resistance were insufficiently considered, and the rationale for long-term prophylaxis was lacking. These findings necessitate a revision of pharmacotherapy approaches towards strict consideration of microbial sensitivity, in accordance with the updated 2024 guidelines.&lt;/p&gt;
<kwd-group xml:lang="en"><kwd>pharmacoepidemiology</kwd><kwd>children</kwd><kwd>urinary tract infections</kwd><kwd>vesicoureteral reflux</kwd><kwd>uroseptics</kwd><kwd>clinical guidelines</kwd><kwd>antibiotic resistance</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>фармакоэпидемиология</kwd><kwd>дети</kwd><kwd>инфекции мочевыводящих путей</kwd><kwd>пузырно-мочеточниковый рефлюкс</kwd><kwd>уросептики</kwd><kwd>клинические рекомендации</kwd><kwd>антибиотикорезистентность</kwd></kwd-group></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Sohail AM, Satapathy AK, Agrawal K, et al.
