The aim of the study was to evaluate these parameters taking into account concomitant pharmacotherapy. Material and methods. This retrospective study enrolled 67 patients aged 39 to 91 years (Me 63 [54; 73]), hospitalized at the E.E. Volosevich City Clinical Hospital Hospital in 20242025. Residual platelet reactivity (RPT) was assessed using the VerifyNow point-of-care aggregometry system (ARU for aspirin, PRU for P2Y12 receptor inhibitors). Results. The therapeutic range was achieved for ARU (550) in 87.8% of patients and for PRU (95208) in 59.1% of patients. Stent thrombosis was recorded in two cases (during aspirin therapy, ARU = 475, and during clopidogrel therapy, PRU = 118). Hemorrhagic syndrome was observed more frequently with P2Y12 inhibitors (14 cases) than with aspirin (3 cases). Statistically significant drug-drug interactions were identified: concomitant use of P2Y12 inhibitors with proton pump inhibitors (PPIs) led to an increase in PRU (p = 0.012), while concomitant use of beta-blockers with aspirin led to a decrease in ARU (p = 0.032). A correlation was found between the PPI dose and PRU values (rs = 0,281; p = 0,045), as well as between the beta-blocker dose and ARU values (rs = 0,283; p = 0,039). Conclusion. Monitoring RPT and accounting for drug-drug interactions enable treatment personalization and reduce the risk of complications.</p>
<kwd-group xml:lang="en"><kwd>antiplatelet therapy</kwd><kwd>residual platelet reactivity</kwd><kwd>drug-drug interactions</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>антитромбоцитарная терапия</kwd><kwd>остаточная реактивность тромбоцитов</kwd><kwd>межлекарственные взаимодействия</kwd></kwd-group></article-meta></front>

<back><ref-list><ref id="B1">1.<mixed-citation>Лукьянец, К.
