Disturbances of autonomic regulation occurring in the acute period of stroke are associated with worsening of neurological prognosis and mortality, which underlines the relevance of the study of mechanisms of autonomic dysfunction in STEMI. Objective. To study possible correlations of the parameters of heart rate variability with the severity of the state of patients with ONMK. Materials and methods. A study of 40 patients with STEMI was carried out. Cardiorhythm was determined using VNS-micro by Neurosoft company, with the analysis of temporal (SDNN, RMSSD) and spectral (LF, HF, LF/HF) HRV parameters. Results. The analysis of data from patients with acute cerebral circulatory failure (ACBF) demonstrated statistically significant correlations between key parameters of heart rate variability (HRV), such as time (SDNN, RMSSD) and spectral (LF, HF) indices, and specific characteristics of neurological status, including severity of motor deficit, cognitive impairment and NIHSS scores. Conclusion. The correlation data covering both sympathetic and parasympathetic components of regulation make it possible not only to quantitatively assess the degree of autonomic nervous system dysfunction, but also to use HRV data as an additional prognostic marker for early detection of the risk of secondary complications, such as arrhythmias or autonomic crises, which is especially relevant for individualisation of therapy in the acute period of stroke.&lt;/p&gt;</abstract><kwd-group xml:lang="en"><kwd>heart rate variability</kwd><kwd>stroke</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>вариабельность сердечного ритма</kwd><kwd>инсульт</kwd></kwd-group></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>1.
