The aim. To conduct a comprehensive analysis of the clinical features of mental disorders in neurosyphilis, as well as to evaluate modern diagnostic methods of this disease in the context of its epidemiological significance, particularly in the Russian Federation. Materials and methods. The review was prepared using data from domestic and international studies, including epidemiological reports, works in neurology, psychiatry, and laboratory diagnostics. Special attention was paid to recent publications reflecting the specifics of neurosyphilis in Russia, as well as studies describing the association of the disease with HIV infection and the variability of clinical manifestations (from asymptomatic forms to severe dementia and psychoses). Results. Neurosyphilis manifests a wide range of mental disorders, including progressive dementia, psychoses with delusions and hallucinations, manic-depressive states, and neurotic disorders. Diagnosis is complicated by the lack of a unified standard. Conclusions. Neurosyphilis remains a relevant interdisciplinary problem requiring the integration of neurological, psychiatric, and laboratory diagnostic methods. Early detection and antibiotic therapy are critically important to prevent irreversible cognitive and mental impairments.
