Its deficiency is widespread among patients with type 2 diabetes mellitus (T2DM) and is associated with disease progression, as well as dysfunction of the endothelium, immune system, and cellular defense mechanisms. One of the key factors involved in these processes is the heat shock protein HSP-70, which possesses cytoprotective properties, along with netrin-1, which regulates inflammation and apoptosis. However, the correlation between changes in these biomarker levels and the severity of vitamin D deficiency, as well as the underlying mechanisms of these processes, remains insufficiently studied. The aim. To assess the impact of varying levels of vitamin D deficiency on inflammatory processes, apoptosis, and carbohydrate metabolism in patients with T2DM, with a focus on changes in netrin-1 and HSP-70 levels. Materials and Methods. The study included 160 participants, comprising 130 patients diagnosed with T2DM and 30 healthy individuals. All participants were further divided into groups based on their vitamin D levels. Biomarker analysis was performed using enzyme-linked immunosorbent assay (ELISA), and statistical data processing was conducted using nonparametric statistical methods. Results. Patients with severe vitamin D deficiency exhibited reduced levels of netrin-1 and HSP-70, indicating impaired protective mechanisms. After vitamin D correction, a positive trend was observed, accompanied by a decrease in inflammatory and apoptotic processes. Conclusions. Vitamin D deficiency is a significant factor contributing to the progression of pathological changes in T2DM.
