The main role in this pathology is attributed to slow calcium channels, which are able to increase the intracellular concentration of calcium ions and thereby cause oscillations of membrane potential

However, there is evidence for the involvement of Na-Ca exchange, in the occurrence of arrhythmias. The question of the predominant involvement of these two different mechanisms in the occurrence of fibrillation remains to be elucidated. In this connection, the task of the present study was to establish the leading role of one of the two pathogenetic processes.

In experiments, on isolated hearts of white rats, the flow of calcium ions inside cardiomyocytes was activated by Na-Ca exchange. This produced ventricular fibrillation of the heart. After addition of a selective inhibitor of slow calcium channels (isoptin) to the perfusion medium, a significant (70%) decrease in the amplitude of cardiac contractions was observed.

However, the presence of the inhibitor did not prevent the occurrence of ventricular fibrillation. It appeared in 100% of experiments.

Based on the obtained experimental data, it was concluded that the leading role of Na-Ca exchange mechanism in the pathogenesis of cardiac ventricular fibrillation.
