The popularity of vapes, which has grown in recent years, and their uncontrolled use pose a threat to the health of adolescents. EVALI (e-cigarette, or vaping, product use-associated lung injury) is a relatively new disease. The symptoms are non-specific, which complicates differential diagnosis with other bronchopulmonary diseases. Objective. To present the features of clinical manifestations and difficulties in the differential diagnosis of EVALI in an adolescent girl. Materials and methods. A clinical observation of a patient was conducted, and medical records were analyzed. Results. An adolescent girl was admitted to the emergency department with clinical signs of acute respiratory failure of the 2nd degree. The history indicated that the girl had been vaping for the last 2 years. Examination revealed tachycardia, tachypnea, hypoxia, inflammatory changes in the complete blood count, elevated CRP, and hypertransaminasemia. Repeated spirometry demonstrated obstructive disorders with a positive response to a bronchodilator. Computed tomography of the chest revealed areas of "ground-glass opacity". Allergy testing showed sensitization to household and epidermal allergens. Conclusion. In this clinical case, two diagnoses were considered: EVALI and severe exacerbation of bronchial asthma. All criteria for a confirmed case of EVALI were fulfilled. Bronchial asthma was not diagnosed due to a lack of sufficient evidence. Physicians working with adolescents need to consider the possibility of vaping.
