PROGNOSTIC VALUE OF LUNG LESION VOLUME ACCORDING TO CT DATA IN COVID-19

Abstract
Coronavirus infection for the period of its existence and today is a disease to which the eyes of the public health and the world community are constantly turned, because despite the apparent dynamics of reducing the incidence due to compliance with all anti-epidemic measures, there is still a threat of outbreaks and modification of the RNA-containing virus of the Coronaviridae family. Thus, when assessing the burden of COVID-19, the results were summed up – the economic component suffered, there was a social protest against self-isolation measures, changes were identified in the procedures for preventive measures, and a lot of work was done in the re-equipment of departments for patients with a New coronavirus infection, the purchase of additional equipment and research in this area.
Goal
Analysis of the course of covid infection with standard methods of treatment, depending on the degree of lung damage according to CT data.
Materials and methods
A retrospective analysis of 93 case histories was carried out on the basis of the BUZ IN "VGKBSMP No. 10". Laboratory, clinical and instrumental research methods were carried out. In the study group, the number of women was 53%, men - 47%. Statistical data processing was performed using Microsoft Office Excel – analysis of the mean value and standard deviation, evaluation of the value of variables of average values using the parametric Student t-test, as well as Spearman correlation.
The results of the study. In all patients – subfebrile temperature, with CT-2-4 increase in heart rate, CT-2-4 increase in BPD. ESR is an increase in CT-0-4. Indicators of general metabolism: glucose – increase with CT-0-3; indicators of liver failure: total bilirubin – increase with CT-4, AlAt – hanging with CT-3-4, AsAt increase with CT-0-4; indicators of renal failure: urea – increase in 1 day with CT-2, creatinine – increased upon admission of CT-0. CRP is an increase in CT-0-4. Ferritin – increased CT-0-4. Indicators of homeostasis: fibrinogen and INR – a slight increase in CT-0-4.
Conclusions
Based on the analysis of the course of covid infection with standard methods of treatment, depending on the degree of lung damage according to CT, we can conclude that women are most often ill, the average age is 61-70 years. Women are more seriously ill, since CT-4 is more common at the time of admission, CT-1 is more common in men. Throughout the day, patients with CT-0-4 had elevated ESR, CRP, ferritin, fibrinogen, INR and an increase in individual indicators at different stages of CT. Individual indicators may be prognostically significant, such as an increase in ESR and platelets, eosinophilia of blood, deviations in liver and kidney function.

With COVID-19 infection, patients have a different clinical picture depending on the stage of CT, concomitant diseases, and lifestyle of patients and leads to different outcomes – from complete recovery to a fatal result [2]. One of the main issues facing healthcare is to establish the causes that can complicate the course of coronavirus infection through the use of clinical, laboratory and instrumental research methods, as well as dependence on gender and age [5, 8].
The main role in determining the course of coronavirus infection depending on the stage of lung damage, predicting the clinical picture, evaluating the effectiveness of treatment, and diagnosing COVID-19 manifested by typical opaque glass opacities in the images belongs to computed tomography [6, 7]. This instrumental method made it possible to compare the results of staging coronavirus infection from the degree of lung damage according to CT data [6].
Goal

Materials and methods

The results of the study
Upon admission, the highest figures of systolic blood pressure (SAD) were observed at CT-3, the lowest – CT-0. By 10 days, SAD in all groups returned to normal. Diastolic blood pressure (DBP) the highest values were present at CT-1, the lowest – at CT-4. By 10 days, the indicators of DAD returned to normal. As for the heart rate (HR) and respiratory rate (BPD) – the highest figures at admission belonged to the CT-4 group, normalization of indicators in all groups came to 1 day. All patients had subfebrile temperature, which reached normal values by 5 days.
Assessment of the general blood test – upon admission, there was a decrease in eosinophils at CT-0, CT-1, CT-2 and CT-4 and platelets at CT-1. On day 1, a decrease in eosinophils was observed in the CT-0, CT-1, CT-3 and CT-4 groups, a decrease in platelets at K-1 and a decrease in hemoglobin at CT-0. On day 5, there was a decrease in eosinophils at CT-0 and a slight increase in platelets at CT-3. On day 10, platelets increased in groups CT-0, CT-2 and CT-3.
Conclusion
