PROTEOLYTIC ACTIVITY OF ANTISEPTICS USED IN MEDICAL ROOT CANAL TREATMENT

Abstract

ANNOTATION

Endodontic therapy consists of medical and mechanical root canal treatment. This is rather a challenge for a dentist who has to work practically blindfolded. Irrigation of root canals is one of the main steps in endodontic therapy. Medical treatment destroys the infection in the root end opening. This is a tooth-saving measure taken to prevent complications and secondary infections.  

Purpose. The purpose of this research is to compare and analyze proteolytic activity of antiseptics used for irrigation of root canals.   

Methods. The determination of proteolytic activity required research methods as follows: theoretic, empirical and mathematical.  

Outcome. Pieces of chicken breast of the same weight (initial wight) were fully submerged in antiseptics, earlier selected for the test. 24 hours later the chicken breasts were taken out and their weight was compared with the initial weight. Having done some necessary calculations we determined the proteolytic activity of the antiseptics.   

Conclusion. The research shows that Belodez has the highest proteolytic activity, while Chlorhexidine has the lowest

Scientific relevance. The cell wall of microorganisms contains proteins, fats and carbohydrates. The structure of the cell wall is composed mainly of the protein component, therefore antiseptics used for root canal treatment should have high proteolytic activity to destroy as many infectious agents in root canals as possible.   

Which antiseptic has the highest proteolytic activity? In order to find an answer to this question we compared several antiseptics used in medical treatment in endodontics. 

Purpose. To compare and analyze proteolytic activity of antiseptics used in medical treatment in endodontics. 

Methods. Proteolytic activity was determined by the formula: (М1-М2):М1*100%, where M1 is the weight of the chicken breast before and M2 is the weight after a contact with an antiseptic solution. Four antiseptics were selected for the experiment: 2% solution of chlorhexidine bigluconate (Chlorhexidine), 3% solution of hydrogen peroxide (Hydrogen Peroxide), 3% solution of sodium hypochlorite (Belodez) and Iodinol. Then we put 4 chicken breast pieces (4 g per piece) in four bowls (one per bowl), each filled with one of the abovementioned antiseptics. 24 hours later we weighted the meat pieces and put the results down.     

Outcome.

The chicken breast placed in Belonez lost 3 g of its weight; the chicken breast placed in Iodinol lost 2 g; so did the meat placed in Hydrogen Peroxide. The weight of the chicken breast placed in Chlorhexidine did not change at all. Therefore proteolytic activity of the tested antiseptics is as follows: Belonez – 12.5%, Chlorhexidine – 0%, Iodinol – 8.33%, Hydrogen Peroxide – 8.33%. In our opinion, these results have the following explanation: Chlorhexidine destroys microorganisms by changing the membrane permeability, connecting with phospholipids and displacing compounds of divalent cations, having no effect on the protein structures of the cells. Hydrogen Peroxide facilitates the loss of biological activity of organic compounds, which confirms the presence of proteolytic activity. Iodinol is a bactericide: it destroys proteins of the membrane of microorganisms. Belonez (sodium hypochlorite) neutralizes amino acids, causing their dehydration and hydrolysis, turning them into salt and water.  

Conclusion.

Medical root canal treatment is done with antiseptics, which can be absolutely different in their effect and application. Our research shows that Belonez has the highest proteolytic activity, while Chlorhexidine has the lowest. It is the difference in mechanisms of action of these antiseptics that affects the level of their proteolytic activity.
