Abstract
Introduction. Asthma and osteoarthritis (OA) are common pathologies that are often combined with obesity. Obesity can negatively affect the course of asthma and OA. However, there is little data on the impact of OA on quality of life (QoL) and the clinical course of asthma. Goal. To evaluate the impact of OA on quality of life and the characteristics of the asthma course in combination with obesity. Materials and methods. The sample consisted of 137 patients with moderate asthma. Two groups were formed: group 1 included patients with asthma without signs of OA; group 2 – patients with asthma and OA. Clinical assessment of diseases, determination of the Lequesne index, spirometry, assessment of quality of life and asthma control were carried out. Results. Patients with asthma and OA had higher inflammatory indices and body mass index (BMI), and QoL, spirometric indicators and the level of asthma control were significantly lower than in patients with asthma without OA. Conclusions. The results of the study indicate a negative impact of OA on the quality of life and the level of asthma control in obese patients.

Введение. В настоящее время БА является одним из наиболее распространенных заболеваний бронхо-легочной системы, которое также характеризуется высокой частотой регистрации сопутствующих патологий [1]. Одним из значимых коморбидных состояний при БА, негативно влияющим на течение, прогноз и качество жизни, является ожирение, которое также способствует развитию ОА [2]. Однако данные о влиянии ОА на БА ограничены.
Целью работы являлась оценка влияния ОА на качество жизни (КЖ) и особенности течения БА в сочетании с ожирением.
Материалы и методы исследования. Исследование включало 137 пациентов с БА средней степени тяжести. Участники были разделены на две группы: 1-ая группа состояла из 46 участников с БА без признаков ОА (13 мужчин и 33 женщин, средний возраст 51,57 ± 10,45 года); во 2-ую группу были включены 91 пациент с БА и ОА (23 мужчины и 68 женщин, средний возраст 54,10 ± 9,17 года).
Были проведены клиническая оценка заболеваний, определение индекса Лекена, спирометрия, оценка КЖ (опросник качества жизни при астме, AQLQ-S) и контроля БА (опросник контроля симптомов астмы ACQ-5).
Результаты исследования. Различий получено не было по полу (p = 0,71), возрасту (p = 0,16), длительности течения БА (p = 0,34). Группы различались по уровню контроля БА (p = 0,0000), индексу Лекена (p = 0,0000), ИМТ (p = 0,0000), уровню ИЛ-4 (p = 0,012079) и ИЛ-6 (p = 0,0000), основным спирометрическим показателям (ОФВ1 (p=0,0001), фЖЕЛ (p=0,0108), ОФВ1/фЖЕЛ (p=0,0055)) и результатам опросника ACQ-5 (p=0,0000). Среди пациентов с БА и ОА было достоверно больше больных с неконтролируемой БА (χ2 = 55,232; p = 0,0000), а также лиц с ожирением (χ2 = 132,612; p = 0,0000). Группы различались по всем шкалам AQLQ-S (p = 0,0000, кроме шкалы «окружающая среда – p = 0,0018).
Заключение. Результаты исследования указывают на негативное влияние ОА на качество жизни и уровень контроля БА у пациентов с ожирением.
