Abstract

The article deals with the issue of the development of the professional self-concept. The attempt of the elaboration of the conceptual basis of the professional self-concept development of a modern medical university educator had been made. This basis allows activating personal potential of an educator. According to the results of the study, the types of the professional self-concept of medical university educators were distinguished: scientific-educational, curative-educational, productive-educational ones. It is proved that each type of the professional self-concept has its own combination of characteristics and personal features of subjects.

PSYCHOPATHOLOGY OF DIALOGIC SPLITTING OF THE SELF: COMPARATIVE ANALYSIS

Abstract

The symptom of the dialogic splitting of the self reflects disorder of the monologic feature of inner speech. The quality of involuntariness causes its similarity to mental automatism, pseudohallucinations, obsessive phenomena. However, with the dialogic splitting, there is no experience of alienation which is inherent in mental automatism and pseudohallucinations. Dialogic splitting has no experience of extraneous influence, of being made, which are characteristic of persecutory delusion and pseudohallucinations. Obsessions are characterized by absence of internal splitting of the self which is observed in dialogic splitting. The symptom of a dialogic splitting of the self is one of the specific manifestations of schizotypal disorder.

PECULIARITIES OF COGNITIVE DISORDERS IN PATIENTS WITH ISCHMIC STRORE

Abstract

During the work, the state of cognitive functions was studied in 54 patients who underwent ischemic stroke in various basins of the brain. Complete clinical studies were conducted with evaluation of neuropsychic status-memory, visual perception and attention. It was shown that the overwhelming majority of patients had violations (mostly mild and moderate) of auditory speech (98.1%) and visual (96.2%) memory, and attention violation was observed in 100%. The degree of expression and the nature of cognitive impairment directly depended on the location of the lesion. Thus, patients who underwent a stroke in the left middle cerebral artery were dominated by mild and moderate disorders of immediate auditory memory, and in severe disorders, delayed reproduction was more affected. At the same time, the disturbance of visual memory and attention was more pronounced in patients who had suffered a stroke in the vertebrobasilar basin and the anterior cerebral artery.

The prevalence of obesity is increasing in the world and has reached epidemic proportions. Obesity during childhood can have a harmful effect on the body and leads to serious health consequences. Disease impact is aggravated by low- efficiency modern therapy. The results of epidemiological studies make obesity one of the most common chronic diseases among children in the world. The review focused on the analysis the literature of childhood obesity’s problem. The results of modern studies showed the influence of various factors to the developing overweight and obesity among children such as eating behavior, salivary amylase and vitamin D.

CLINICAL AND ENCEPHALOGRAPHIC STUDY IN DISORDERS OF THE REPRODUCTIVE CYCLE IN ADOLESCENCE

Abstract

Disorders of the reproductive cycle in adolescence are juvenile metrorrhagia, algodysmenorreya (attacks of intense pain during menstruation), amenorrhea and opsomenorrhea (violations of monthly frequency). The aim of our study was to clarify the features of the functional state of the brain (the EEG) in adolescent girls with disorders of the reproductive cycle. The study involved 200 girls with menstrual irregularities at the age of 15,47+1,44 years. Clinical and electroencephalographic study of adolescent girls with menstrual irregularities suggests they have one-third of cases the depressive reactions. State of low mood is more common in adolescents with amenorrhea and juvenile metrorrhagia. Depressive reactions in girls with disorders of the reproductive cycle correspond to three types of EEG patterns, which can be roughly described as moderately pointed, sharp and clearly out.

THE STUDY OF EATING BEHAVIOR IN CHILDREN OF PRE-PUBERTAL AGE WITH THE PATHOLOGY OF THE GASTROINTESTINAL TRACT

Abstract

31 children aged 3-11 years (mean age 7.42 ± 0.542 years, 15 boys, 16 girls) with various gastrointestinal diseases were examined: hypertension of the esophagus veins (1 person), encopresis (1 person), jaundice (1 person), celiac disease (6 people), ulcerative colitis (4 people), dyspeptic disorders (9 people), agenesis of the biliary tract (1 person), Crohn's disease (8 people). The average value of the family cohesion scale, according to the FACES-3 methodology, corresponded to the divided level of family relations, and the average value of adaptation to a chaotic level (a divided-chaotic profile of family functioning). This suggests a balanced nature of the relationship between individual family members, but a disadaptive level of the functioning of the family as a whole. A study of the parents of patients using the Family Alarm technique showed that none of the subscales ("Wines", "Anxiety", "Tension") reached the diagnostic level (the sum of the scores was 5 or more). Therefore, one can not speak about the expressed level of anxiety among family members of the children surveyed. The study using the "Scale of Food Behavior" (DEBQ) method showed that the excess of reference values was noted only in the subscale "Emotional eating behavior", hence this type of eating behavior prevailed in the parents of the children surveyed. The results of the correlation analysis showed that the emotional eating behavior of parents was directly related to overeating in children. Some features of eating behavior in children were associated with this or that type of family functioning. Thus, the tendency to overeating was associated with a high level of family cohesion. The relationship of such characteristics as high sensitivity to saturation with slow rate of food intake, food selectivity, emotional malnutrition, high level of family tension and severity of child anxiety is shown.

Abstract

The high level of impulsivity, manifested primarily in impulsive actions, reduced ability to plan and self-control, as well as the infantilism, manifested as a low level of self-orientation, avoiding cooperation with other people and the tendency to search for unusual transpersonal spiritual experience are risk factors for early formation of dependence. At the same time, maturity and willingness to cooperate with society constrain early access to alcohol. Early formation of alcohol dependence contributes to greater severity of disease manifestations. The high level of impulsiveness, manifested in the difficulties of planning one's life and overcoming obstacles, contributes to the problems with the law.

Abstract

Vascular diseases of the brain (GM) - one of the leading causes of morbidity, mortality and disability in the Russian Federation (RF). In the Russian Federation, stroke develops annually in 450,000 people, of whom about 35% die in the acute period of the disease. In recent decades, there has been an increase in the frequency of cerebral strokes in young people, which is associated with changes in lifestyle, nutrition and chronic overstrain. Often after a stroke, in addition to the neurological deficit, patients have different disorders of higher brain functions. The article discusses the prevalence of cognitive impairment in patients with ischemic stroke in the late recovery period. Conclusions are drawn about the necessity and expediency of using the method of transcranial magnetic stimulation in patients with ischemic stroke.

THE STATE OF HEALTH OF STUDENTS OF MEDICAL SCHOOL ACCORDING TO QUESTIONNAIRE

Abstract

In psychology classes with the help of an anonymous questionnaire, first-year students of the medical University evaluated their physical and mental health on a five-point scale. Analysis of the questionnaire data showed that more than a third of students consider their mental and physical health "bad" or «very bad». Students called the causes of the low self-esteem: chronic physical illness, educational stress, lack of sleep. Feeling of constant fatigue, irritability, emotional instability, anxiety, depressed mood, lack of sleep have become a reason for low self-esteem mental health.

In this work, using the method "Diagnosis of the level of empathy" V. V. Boyko, the degree of manifestation of empathy among students of the Voronezh Medical University was studied. Abstract

Chronic obstructive pulmonary disease is the fourth leading cause of death worldwide. Especially with severe consequences, the disease occurs in patients with a combination of chronic obstructive pulmonary disease with increased weight. The aim of the work was to study psychosomatic correlations of improving the effectiveness of therapeutic and preventive measures in patients with chronic obstructive pulmonary disease with obesity. The study included 97 patients with chronic obstructive pulmonary disease, occurring against the background of obesity. Clinical, psychopathological, experimental and psychological methods were used, which included analysis of General anamnetic data, peculiarities of the course of the disease, description of psychopathological symptoms and syndromes, their dynamics in the course of therapy for a month. RESEARCH OF SOME PSYCHOSOMATIC ASPECTS OF SKIN DISEASES IN ADOLESCENTS

Abstract

20 adolescents were examined (mean age 14.95 ± 0.420 years) - patients of the skin clinic with the following diagnoses: psoriasis (6 people), atopic dermatitis (4 people), scleroderma (2 people), alopecia (3 persons) acne (5 people). It was shown that the average values of the scores of clinical scales (the scale of self-esteem of Beck depression for adolescents, the scale of obsessive-compulsive disorders of Yale-Brown) corresponded to the level at which there was no current psychopathological symptomatology. A family factor study using the FACES-3 technique revealed that in most patients the family was disconnected (except for patients with scleroderma and acne), which fits into the notion of conditionally adaptive family functioning. Family adaptation corresponded to a chaotic level, related to disadaptive forms of family functioning (except for patients with alopecia). The average value of the "Self-Body Satisfaction Scale" (SHUST) (80.5%) corresponded to the high index, which indicates a sufficient degree of satisfaction with one's own body. Lower scores were noted in the evaluation of segments such as "skin" (corresponding to the pathology profile of the patients examined), "face" and "teeth." However, there were no statistically significant differences between the different segments according to the Wilcoxon rank criteria. (P> 0.05). The average value of the scale "Assessment of the image of one's own body" (OOST) (median -12.0, wallmark-6) indicates an insignificant tendency to negative assessment of one's own body (this is not critical and diagnostic significance). The study of the correlation relationship (rank correlation of Spearman) revealed that the patients' age was inversely correlated with the mean value of the OST scale (ρ = -0.436, p = 0.055), that is, the positive self-body score in adolescents increased with age. The sum of points on the Beck scale moderately correlated back to the "Cohesion" scale (ρ = -0.460, р = 0.041) and SHUST (ρ = -0.610, р = 0.004). Thus, depressive symptoms in adolescents with cutaneous pathology are more pronounced with a high level of cohesion between family members and low satisfaction with one's own body.

Abstract

Multiple sclerosis (MS) is the most common autoimmune demyelinating disease of the central nervous system (CNS) - the brain and spinal cord, characterized by the development of time-dispersed foci of demyelination and diffuse changes in brain tissue, which is clinically manifested by multifocal neurologic symptoms. MS is one of the most severe forms of organic damage to the nervous system that affects mostly young and working-age people, which leads to a decrease in their quality of life and the development of disability. Unfortunately, MS is not so easy to diagnose at the earliest stage. But some changes in the human psyche can become signs of a developing ailment. Often they are not considered a manifestation of the disease and relatives and close relatives of MS patients do not take them seriously, or are treated as disorders of a completely different nature.

Abstract

Recently in the Russian Federation (the Russian Federation), as well as all over the world, there has been an increase in the number of disabled people, and unfortunately also among children, which is an indicator of the deterioration of the health status of the population. Therefore, the prevention of childhood disability, the early rehabilitation of children with cerebral pathology goes beyond the purely medical problem and acquires the status of social. The article describes a clinical case of rehabilitation of a child with infantile cerebral palsy (cerebral palsy), a lower parapaperhesis of moderate severity under conditions of a rehabilitation department of a multi-profile pediatric hospital. POSTPARTUM AFFECTIVE DISORDERS IN WOMEN WITH HEALTHY NEWBORNS AND NON-HEALTHY CHILD IN THE DEPARTMENT OF NEONATAL PATHOLOGY

Abstract

Studied 113 women in the postpartum period (patients of the perinatal center of SPbSPMU). They were divided into 2 groups: 1st group - women (78 people), whose children were discharged in 3-6 days after birth (examination took place on an outpatient basis); 2d group - women (35 people), whose children were patients of the department of perinatal pathology during the period of investigation. All women were examined twice: first time within 10 days after childbirth, then one month after. Methods of the examination: clinical-anamnestic and clinical-psychopathological, included Edinburgh Postnatal Depression Scale (EPDS); The Hospital Anxiety and Depression scale (HADS); the Snaith-Hamilton pleasure scale modified for clinician administration (SHAPS-C) and the version of the Mini-Mult adapted for Russia. Initial examination demonstrated that there was no significant difference in the results in both groups. According to EPDS severity grade (χ² = 0,884, р = 0,347), as well as The Hospital Anxiety and Depression scale (χ² = 3,326, р = 0,334) and (χ² = 0,465, р = 0,793) respectively. Results obtained within further examination demonstrated significant differences between the severity level in the1st and 2nd groups. According to EPDS (χ² = 20,214, р = 0,0001), HADS- anxiety scale (χ² = 29,166, р = 0,0001) and depression (χ² = 18,459 , p = 0.0001). Thus, in the 1st group there was a tendency to decrease the severity level for all indicators. The Mini-Mult indicated the preference of the average value of the scale of "Psychasthenia" in the 1 st group, as in the 2nd group "Paranoia" scale. So, women who were for a long time in the department of neonatal pathology have a tendency to increase of severity or delay in the postpartum affective disorders manifestation, which may indicate a significant role of psychogenic factors in the disease development

Abstract

The article describes some aspects of the computer dependence. The supposed causes and factors for the formation of the computer addiction were highlighted during working with literature on this topic. A lot of authors pay special attention to the portrait of the computer addiction. There are many opinions on the subject of computer dependence. Different views on this subject are given in this review. Based on the above, there are no perfect diagnostic methods. There are many questionnaires at the present, but they only help to diagnose computer addiction, and they are an auxiliary tool. Questionnaires are used after the diagnosis is actually put. Treatment of this type of the dependence is complex. It includes both the pharmacological component and non-pharmacological therapy.
