SUBMICRON COMPOSITES: INNOVATIVE APPROACH TO DIRECT COMPOSITE RESTORATION OF TEETH, CLINICAL POSSIBILITIES

Abstract

Clinical experience with the new universal submicron composite Brilliant Ever Glow is presented, the results of using this composite in various situations requiring tooth restoration are presented. It is emphasized that due to the presence of composite masses with different transparency and the Duo Shade system, the composite has excellent color rendering properties that allow you to successfully solve almost any clinical aesthetics of hard tooth tissues, including restoration of teeth previously undergoing endodontic treatment. Results are shown that emphasize the high efficiency of the composite during combined restorations together with metal-free ceramics.

APPLICATION OF PLATELET-ENRICHED FIBRIN IN PATIENTS WITH INFLAMMATORY DISEASES OF MAXILLOFACIAL REGION IN THE SURGICAL HOSPITAL

Abstract

The effectiveness of the use of improved platelet-enriched fibrin (a-PRF) in the treatment of odontogenic diseases was evaluated under conditions of 26 patients aged 22 to 65 years. It has been found that stimulation of angiogenesis, processes of cell proliferation and differentiation allows to achieve good results in treatment of both odontogenic diseases of maxillofacial region and osteonecrosis of jaws of different etiology in early postoperative period. The accessibility of this technique allows it to be used at all levels of specialized and qualified medical care. It is emphasized that the modification of the a-PRF clot with other drugs opens up new possibilities for the integrated treatment of purulent wounds and osteomyelitis of the maxillofacial region.

Abstract

In the daily practice of a dental surgeon, a doctor is often faced with the need to carry out surgical interventions for complex tooth extraction in the lower jaw. These surgeries are performed for various reasons, for example, for chronic periodontitis, periodontal diseases, teething diseases, orthodontic indications. All stages of surgical intervention for tooth extraction are accompanied by the risks of complications of various origins. In order to prevent all complications, there are many ways and techniques of both the extraction operation itself and drugs that reduce the risks of such. However, the relevance of the study of this issue does not lose its importance, since every day doctors also face the problems of complications of inflammatory genesis in the post-extraction period.

USE OF PERIODCHIP IN COMPLEX TREATMENT OF PERIODONTAL DISEASES

Abstract

It is noted that periodontal diseases are widespread in all countries of the world, the population of working age is detected. Untimely diagnosis, due to weak clinical manifestations, and, as a result, seeking help in advanced stages of the disease, can lead to serious consequences and difficulties in choosing tools and methods of treatment. In terms of clinical management of such patients, it is of interest to use dosage forms with prolonged action of the active ingredient. One embodiment of the prolonged action means is the Dexcel Pharma 3 PerioChip matrix. PerioChip containing 2.5 mg of chlorhexidine enclosed in a biologically soluble shell with dimensions of 5x4x0.35 mm. The use of the drug confirmed the elimination of exacerbations of the inflammatory process, a decrease in the depth of the pathological tooth-bearing pocket and a decrease in the degree and complete leveling of tooth mobility.

ELECTROCHEMICAL CHARACTERISTICS OF THE SYSTEM "DENTAL IMPLANT - PROSTHESIS"

Abstract

The development of implantology requires studying the urgent problem of electrochemical compatibility of construction materials and implants in order to prevent pathological processes caused by galvanism in the oral cavity. The electrochemical compatibility of a titanium implant and materials of dental prostheses in vitro was studied by measuring the electrode potentials of alloys in a stationary state, interacting through layer of a model solution of different thickness. It was experimentally established that in the system "metal alloy prosthesis - model solution (artificial oral fluid) - titanium implant" galvanic currents arise, the magnitude and direction of which depend on the type of metal alloy. For the manufacture of dentures on titanium implants it is recommended to use chromium and chromium-nickel alloys and limit the use of stainless steel and gold-platinum alloys.

ASSESSMENT OF RESIDUAL MICROFLORA IN PATIENTS WITH INFLAMMATORY DISEASES OF MAXILLOFACIAL REGION

Abstract

In the practical work of the maxillofacial surgeon, it is necessary to take into account the presence of a pathogenic microflora with a small number of colony-forming units, since when the reactivity of the body decreases, they will cause local inflammatory reactions, which leads to a deterioration in microcirculation and trophy of neighboring tissues. The aim of the study was to assess residual microflora in patients with inflammatory diseases of the maxillofacial region. In 43 patients surgically treated with the purulent focus, oral flora surrounding the purulent focus of tissues and the wound contents directly were examined. In most cases, a combined microbiological flora has been isolated. The difference in the amount of CFU/ml determines the course of the local inflammatory reaction, as well as the state of microcirculation. The latter factors play a large role in the successful course of a smooth postoperative period.

ASSESSMENT OF BIOLOGICAL COMPATIBILITY OF METHYL ACRYLIC POLYMERS IN CORRECTION OF PARTIAL SECONDARY ADENTIA AND CONCOMITANT PATHOLOGY OF DIGESTIVE ORGANS

Abstract

The article presents the data of an original study on the improvement of orthopedic treatment of partial secondary adentia in patients with concomitant chronic pathology of the gastrointestinal tract by the method of removable plate prosthetics. For this purpose, a comparative analysis of three alternative to each other preparations of methyl acrylic polymer for the manufacture of bases of prostheses was made: 1) «GC», 2) «3M-Espe», 3) «Ftorax». A scientific assumption has been formulated that the choice of a specific polymer preparation (within the comparison base) will lead to a fundamental decrease in the level of local inflammatory changes in the mucous membrane of the prosthetic bed. In three independent groups of patients, using the compared preparations of methyl acrylic polymer, a cytological assessment of local inflammatory changes was made. For this, the parameters of neutrophils were studied (show a picture of subacute and chronic inflammation) and the level of fibroblastic infiltration (shows a picture of chronic inflammation) within the mucous membrane of the prosthetic bed. After the implementation of the study, it was found that the most positive results are shown by the polymer «GC», the least positive - by the polymer «Ftorax», the polymer «3M-Espe» occupies an «intermediate position». The results obtained have a high level of statistical significance, which makes it possible to recommend them for consideration for use in the practice of orthopedic dentistry.

THE PROBLEM OF FRONTAL TEETH ROOT CANALS CLASSIFICATION: A RATIONAL DECISION BASED ON MULTI-FACTOR ANALYSIS OF ADVANCED DIAGNOSTIC VISUALIZATION DATA

Abstract

The article presents the data of the original research on the development of the optimal ergonomic classification of the root canals of the frontal teeth. The classification system was created on the basis of multivariate analysis of a significant statistical data set (400 research objects). Empirical data on root canal morphology were obtained using the currently most accurate diagnostic imaging technique - cone-beam computed tomography (CBCT) instead of traditional radiography. The solution to the problem of classification and objectification of research results is based on the use of variation statistics and cluster analysis. When classifying the morphological characteristics of the root canals of the teeth of the frontal group, their basic classifying categories were confirmed - the type of tooth, maxillary or mandibular affiliation, the number of root canals, their diameter and separation. In addition, in the course of the study, 11 most common «additional morphological singularitys» of the structure of the canal system were typified. Due to the procedure of clustering the morphological characteristics of the channels, their statistically stable combinations were determined, which made it possible to identify 16 classification groups. The verification of the developed classification model has shown its high efficiency - due to its use, it became possible to significantly facilitate the choice of endodontic programs in a specific clinical situation and improve the subsequent results of the planned treatment.

IMPROVING THE DIAGNOSIS OF PERIODONTAL DISEASES USING A MODERN RESEARCH METHOD

Abstract

In modern times, inflammatory periodontal diseases remain in the leading place among the pathology of organs and tissues of the oral cavity. The percentage of the total population suffering from chronic inflammatory diseases of periodontal tissues ranges from 70 to 98%. Intact periodontium occurs in 2-10% of the population of Russia. In 52% of the population, inflammatory phenomena in the periodontal tissues of an initial character are noted, in 24% there are manifestations of moderate and 14% of severe [1]. For the diagnosis and the appointment of personalized treatment, the most complete examination of the patient is necessary. To develop optimal treatment tactics, patients undergo laboratory and other types of examinations of the contents of the gingival groove and / or periodontal pocket. The combination of diagnostic methods allows examining the patient to identify the etiological agent, formulate the correct diagnosis and prescribe personalized treatment. In the study, we evaluated the titer of aerobic and anaerobic microorganisms in samples, in larger quantities in a clinically significant titer. There are a number of different instruments for taking the contents of the gingival groove and / or periodontal pocket, but they all have many drawbacks that complicate the inoculation of the material, degrade the quality of the samples obtained and distort the research results. Taking into account the advantages and disadvantages of existing instruments for taking the contents of the gingival groove and / or periodontal pocket, we have developed an instrument for collecting the contents of the periodontal pocket for the purpose of microbiological examination.

Abstract

Clinical experience of restoration of abfraktsionny defect with the help the adgezivnyykh of systems and the composite materials Clearfil ™ SE Bond (Kuraray) is presented. Clinical and morphological features of defect restoration in the cervical region of the tooth were discussed. It is recommended to perform water-cooled preparation to eliminate overheating of the vascular-nerve bundle of the tooth. When using adhesive techniques, it is preferable to use spontaneous systems to avoid the risk of acid necrosis and to provide the same depth of penetration of conditioning agents and adhesive, which will reduce the risk of postoperative sensitivity.

Dentistry as a branch of medicine works with problems of the oral cavity, which as an anatomical area is available for examination and instrumental examination using basic and additional methods. A doctor who has the concept of “on-alertness” will be able to avoid trauma to the soft tissues of the oral cavity in his work. A doctor who has the concept of “oncological diagnosis” will not miss pathology, he will be able to conduct differential diagnosis, a course of therapeutic treatment in a strictly defined time frame and, if necessary, refer the patient to oncologist. The primary care doctor will be able to psychologically correctly inform the patient about serious problems in the oral cavity.

COMPARATIVE ANALYSIS OF THE CHEMICAL COMPOSITION OF BREAST MILK AND ITS SUBSTITUTES

Abstract

The article provides statistical data on the chemical composition of human milk of women, discusses the features of fatty acid composition of human milk in comparison with animal milk and artificial milk formulas. Methods of analysis and methods of quantitative determination of the main components of milk, which make it possible to distinguish human milk from animals, are discussed. It has been shown that there are correlations between the content of saturated and unsaturated fatty acids. Given that the total fat content of milk is a phenotypically dependent value and does not depend on the diet, but the ratio of milk fatty acids depends on the diet. Instrumental methods for determining the fatty acid composition, amino acids of milk protein, lactose are presented.
