Modern treatment strategy, problems and prospects of acute lymphoblastic leukosis in children

Abstract

The results have been reported on the treatment of 1079 children, aged 1 to 15 years old, with acute lymphoblastic leucosis in the centers of pediatric oncohematology in the period from 1991 to 1998. The patients were administered the program treatment according to the modified protocol ALL-BFM-90m, 107 children received non-program treatment. Within the framework of the protocol the patients were divided into groups with standard, mean and high risk (respectively 29.0%; 63.2%; 7.1%), which determined the volume of polychemotherapy and radiation therapy. The treatment results were assessed according to the indices of overall (OS) and event-free (EFS) survival rate using the method of Kaplan-Mayer. The remission was observed in 84-92% of cases. Treatment tolerance was wholly satisfactory. Five-year event-free survival rate (EFS) made up 0.63; 0.72; 0.5 correspondingly depending on the risk group. The advantages of five-year program treatment of acute lymphoblastic leukosis were seen in the results of overall survival rate (OS-0.69), considerably exceeding similar results in the group of patients, administered non-program treatment (OS - 0.32). Infectious complications against the background of neutropenia (29%) and viral hepatitis (45%) were a menace to the lives of patients. Recurrences still remain the main cause of failures happening in the course of treatment according to the protocol ALL-BFM-90 m. It was identified that the causes of acute lymphoblastic leukosis development, features of its localization, and course and recurrence outcomes in children are associated with pathological process severity and non-adequate treatment administered at the initial disease stage. The indices of event-free survival rate, according to the results of our investigations, conform to the general tendency of five-year survival rate growth in children with acute lymphoblastic leucosis. The program therapy for acute lymphoblastic leucosis administered to children according to the protocol ALL-BFM-90m is more efficient than non-program treatment. *Centers of pediatric oncohematology were established in Russia in 1991 in the following cities: Vladivostok, Volgograd, Voronezh, Yekaterinburg, Rostov-on-Don, Novosibirsk, Novokuznetsk, Omsk, Habarovsk with the support of charitable Foundation “Care”, Germany.

Early rehabilitation of patients with coronary heart disease ensures the enhancement of functional systems of an organism and complete restoration of working capacity. The authors have developed the management system of early rehabilitation of CHD patients after coronary artery grafting on the bases of prognostic model, which enables to select properly the group of patients with predominant pathological syndromes, to assess the disease severity and possible complications; differential schemes of drug and functional rehabilitation enable to undertake specific actions on the mechanisms of pathological processes.

Functional analysis of occlusive relations in patients with cleft lip, alveolar process and palate

Abstract

Functional occlusive relations have been analyzed in 23 patients aged 13 to 17.5 years old with congenital unilateral cleft lip, cleft alveolar process and palate. Analysis of dynamic occlusion in patient with congenital unilateral cleft lip, cleft alveolar process and palate has proved that there are marked functional occlusive disorders, particularly, at the cleft side. At the same time high activity of compensation and adaptation process has been observed at the level of masseters and temporomandibular joint, allowing clinical manifestations of occlusive disorders to be leveled. Authors stress the necessity to continue investigations in this area, analyzing dynamic changes of functional occlusive relations in the course of orthodontic (complex) treatment as well as in the retention period.

Objective color assessment of hard dental tissues and filling material using the method of color coordinates

Examinations have been conducted on 50 teeth of the frontal group directly in oral cavities, chromaticity coordinates were identified of cervical area, body and cutting edge of different teeth. The analysis of measurument results allows the consumption that all the spectra have different shapes; on chromaticity diagram each spectrum corresponds to its own coordinate point. Objective color difference was identified between cervical area, body area and cutting edge per units of color threshhold.
