ORGANIZATIONAL ASPECTS OF SURGICAL HOSPITAL ACTIVITY IN CONDITIONS OF MEDICAL SANITARIAN PART OF LARGE INDUSTRIAL FIRM

Absatarov R.A., Absatarov D.R.

Abstract

The analysis of versatile surgical hospital activity in 1997-1999 years was made. It was found out that the predominance of a kind of treatment - conservative or operating, volume of each patients examination varies greatly depending on the economical platform of hospitalization. So, at the charge-free service the resources of medical entity are spent not always rationally. In a surgical hospital the volumes of conservative treatment and operating are approximately identical and 50% more examinations for 1 patient independently of how serious the desease is. In case of paid service only conservative treatment is met extremely seldom, operational powers and the surgical proficiency of the doctors is used more rationally. And quantity of complications and relapses in patients does not depend on what economical basis they been given help. Using organizational technology of an optimum ratio of volumes of the paid and charge-free help it is possible, to increase the effectivity of surgical hospital work and to improve the quality of medical help.

THE ACTIVITY OF LOCAL ANESTHESIA AND MESOMERIE EFFECT AMONG METHOXYCOMBINATIONS OF 2 - DIAETHYLAMINOETOXI - 3 - PHENYLPROPIOPHENON

Isaev P.P., Nikolaevsky V.A., Alexyne M.P., Reznikov K.M.

Abstract

Experimental research data of a number of synthesized Local Anaestheties are shown. Until now only the similarity of structure and strengthening of antiarrhytmmie properties after changing aminoalcoxyl chain structure in blokers of Na+ channels were not known. We have proved that the introducing of one benzol ring into the 1-2-3-position of methoxy-group or changes of the aliphatic chain lead to the pharmacological activity changes. Studying connection between the structure and the effect of the medicine one must take into consideration the mesomeric effect as one of the factors reshaping biological potency of carbonyl and its subsequent effect on the cell receptors’ active centres. Later it is advisable to conduct direct -chemical calculations of electronic structure of medicines under analysis and Local Anaesthetics showing the same effect. It will let us quantatively estimate the structure of receptor’satisfactory active centre in biomembrane and introduce the new criteria for prognosing anaesthetics’ biological potency.

ANALYSIS OF INFLUENCE OF THERMAL FACTOR IN ABDOMINAL CAVITY SANITATION UPON THE COURSE OF ACUTE EXPERIMENTAL PERITONITIS

Gluhov A.A., Banin I.N.

Abstract

The influence of therminal factor at abdominal cavity sanitation upon the dynamics of postsanitation intoxication in able-bodies animals and in conditions of peritonitis is investigated. 242 white rats were taken for the experiment. We have studied a number of indexes of intoxication syndrome peroxide oxidation of lipids antioxidant system and others. It was ascertained that the application of hyperthermal sanitations increases antibacterial activity of working solution. The realization of a hypothermal sanitation, as against hyperthermal, prevents initial postsanitation growth of intoxication, However, after initial strengthening of intoxication at a pyrexial sanitation, the subsequent decrease of a degree of the indicated phenomena manifestation is marked.

OBSERVATION OF BLACK ACANTHOSES IN PATIENTS WITH STOMACH STOMACH

Yurchenko A.A.

Abstract

The outcome of examination of 153 patients with the stomach adenocarcinoma are submitted. Black acanthosis is observed in 9 cases. Sex distribution has compounded men 6, women 3. The majority of patients were older than 60 years. 8 patients had the 2-nd and the 3-d stage of tumoral process one-generalized form of stomach carcinoma. 8 patients were given radical treatment, one was made tentative laparotomy. The controlling inspections 3 and 6 months later revealed the regress of paraneoplastic process in the majority of patients. In 5 patients the black acanthosis has preceded to the clinical development of tumor for the term from 4 months till 3 years in remaining paraneoplastic syndrome developed simultaneously with the clinical development of tumor. The conclusion is that the early diagnostic of tumoral process in patients with black acanthosis could promote more well-timed therapy and improvement of long-term results of treatment.

Kravetz B.B., Kostenko S.M., Potapov Y.N., Glotov A.I., Shakirova N.S., Velichko M.G.

Abstract

The purpose of research was the analysis of Ronkoleukin efficiency in combination with Reaperonum, Cyclophosphanum in doses lowering T-supressors and also increasing doses of Reapheronum in patients with widespread and generalized forms of renally cell-like cancer. It was found out that the combination of Roncoleukinum with the mentioned above drugs gives positive medical effect in the patients stage IV kidney cancer. To obtain more valid data the further prelongation of research is required. The marked medical effect of gradual Reapheronum dose increasing in pathients with the generalized renally cell-like cancer alloves us to go on analysing the efficiency of this method.

RHEUMATIC ILLNESSES AND PARANEOPLASTIC SYNDROME IN DIAGNOSTIC OF STOMACH CARCINOMA

Yurchenko A.A.

Abstract

The outcome of examination of 189 patients with stomach carcinoma is analyzed in the article. In 10 people tumor was combined with rheumatic syndrome. The conducted research testifies to its paraneoplastic nature. In the majority of cases rheumatic syndrome has preceded a clinical development of tumor. The distribution of the patients is following: 4 patients with the 1-st and the 2-nd stages and 6 with the 3-d and 4-th stages of tumoral process. The conclusion is that the early diagnostic of stomach carcinoma, based on paraneoplastic nature of rheumatic syndrome could change tactics of treatment and distant forecast of life for the majority of patients of described group.

CLINICAL AND ANATOMICAL PECULIARITIES OF SURGICAL ILLNESSES

Absatarov R.A., Absatarov D.R.

Abstract

More than 500 case histories of patients taking treatment on surgic hospital have been analysed. It was revealed that twin organs showed statistically different frequency of right- or leftside localization of the pathological process. The difference concerns both nosological classes of diseases (IDS 10) and anatomical localization of pathological nidus (head, upper extremities, trunk, lower extremities). These facts with some amount of subjective estimate deserve more detailed studying and may become the basis for prognosing morbidity and prophylactic measures recommendation.

WHAT DOES THE SPECTRAL ANALYSIS OF VARIABILITY OF CARDIAC RHYTHM REVEAL

Scherbatych G.V.

Abstract

The research is devoted to the analysis of cardiac rhythm’s variability in the standard and the condition, of emotional stress. Different hypothesis of origin of slow waves (Wfand VhF) registrated at spectral analysis are discussed. The importance of taking into consideration the methodical approaches used at spectral analysis is underlined. The attempt of correlating the generators of LF and VLF waves with different parts of the vegetative nervous system is done. The analysis findings obtained from the students, quiescent and in examination stress condition have revealed close correlation of absolute power between waves and parameters of variability of cardiac rhythm, referring to the activity of Parasympathical nervous system. The supposition that at an emotional stress, there is an attenuation of controlling effect on heart on the part of overlying departments of the central nervous system is put forward.
