ASSESSMENT OF THE FREQUENCY OF OCCURRENCE OF VARIOUS FORMS OF DISTAL MALOCCLUSION IN YOUNG PATIENTS
Abstract
Summary. Distal malocclusion is one of the most common dental anomalies. The aim of the study was to assess the frequency of occurrence of various forms of distal malocclusion in young patients. Materials and methods: the analysis of profile telerentgenograms of 123 patients with distal dentition aged 18 to 44 years who had not previously undergone orthodontic treatment was carried out. The following parameters were evaluated: the location and size of the upper and lower jaws, the type of facial growth, the slopes of the upper incisors, and the location of the occlusal plane. According to the results of the study, the incidence of skeletal distal malocclusion in patients seeking orthodontic care was 96.75%. The dental alveolar form was detected in 3.25% of the examined patients. In patients with distal malocclusion, a combination of jaw underdevelopment with their retroposition and retrusion of the upper incisors was most often detected. Among the jaw position anomalies in patients with skeletal forms of distal malocclusion, a combination of lower retrognathia with upper retrognathia was most often observed (50.4% of cases). Among the size anomalies in patients with skeletal forms of distal malocclusion, the combination of lower micrognathia with upper micrognathia was most often observed (55.5% of cases). Combined forms of skeletal anomalies, in which there is a change in the size and position of both the lower and upper jaw, were detected in 91.6% of cases. Anterior inclination of the occlusal plane was detected in 52.9% of cases and posterior inclination in 14.3% of patients with skeletal forms of distal malocclusion. Normal values were observed in 32.8% of cases. The revealed anatomical features of the dental apparatus and facial cranial structures can lead to blockage of the lower jaw and predispose to the development of TMJ and masticatory muscle dysfunction.

Abstract
The development of peri-implantitis and bone resorption is associated with a complex of factors, including microbial biofilm and systemic inflammation. Patients with cardiovascular disease (CVD) are at particular risk, as they have preexisting endothelial dysfunction, which increases the susceptibility of the vascular wall to the virulence factors of periodontal pathogens and proinflammatory cytokines. A total of 104 patients aged 27 to 72 years with integrated dental implants supported by orthopedic structures were studied for 5 years. The patients were divided into two groups based on the severity of bone resorption around the dental implants. Patients underwent questionnaires, CBCT, microbiological testing, and assessment of IL-6, TNF-α, and EN-1 levels. The concentration of the cytokine IL-6 was established corresponding to the degree of bone resorption in the implant area: the greatest increase in the indicator was observed in Group 2. The level of TNF-α in Group 1 did not exceed the reference value. When assessing the correlation of EN-1 with P. gingivalis, T. denticola, P. intermedia, a moderate relationship was found only with P. gingivalis among the entire sample. When assessing the correlation of IL-6 with periodontopathogenic microorganisms, a moderate relationship was found only with P. intermedia, TNF-α showed a weak correlation in all cases. The level of IL-6 and TNF-α showed a noticeable correlation with the degree of bone resorption in the entire sample. A relationship was established between the clinical and radiographic degree of bone resorption in the implant area, the concentration of cytokines IL-6, TNF-α and the occurrence of P. intermedia and T. denticola. Analysis of the cytokine profile may have prognostic value in planning implantation due to the sensitivity of IL-6 and TNF-α to the persistence of P. intermedia and P. gingivalis, which in turn affects the state of the vascular endothelium, and through cytokine proinflammatory activity on the resistance of jaw bone tissue in patients with a history of cardiovascular diseases.
