ASSESSMENT OF THE FREQUENCY OF OCCURRENCE OF VARIOUS FORMS OF DISTAL MALOCCLUSION IN YOUNG PATIENTS

Abstract

Summary. Distal malocclusion is one of the most common dental anomalies. The aim of the study was to assess the frequency of occurrence of various forms of distal malocclusion in young patients. Materials and methods: the analysis of profile telerentgenograms of 123 patients with distal dentition aged 18 to 44 years who had not previously undergone orthodontic treatment was carried out. The following parameters were evaluated: the location and size of the upper and lower jaws, the type of facial growth, the slopes of the upper incisors, and the location of the occlusal plane. According to the results of the study, the incidence of skeletal distal malocclusion in patients seeking orthodontic care was 96.75%. The dental alveolar form was detected in 3.25% of the examined patients. In patients with distal malocclusion, a combination of jaw underdevelopment with their retroposition and retrusion of the upper incisors was most often detected. Among the jaw position anomalies in patients with skeletal forms of distal malocclusion, a combination of lower retrognathia with upper retrognathia was most often observed (50.4% of cases). Among the size anomalies in patients with skeletal forms of distal malocclusion, the combination of lower micrognathia with upper micrognathia was most often observed (55.5% of cases). Combined forms of skeletal anomalies, in which there is a change in the size and position of both the lower and upper jaw, were detected in 91.6% of cases. Anterior inclination of the occlusal plane was detected in 52.9% of cases and posterior inclination in 14.3% of patients with skeletal forms of distal malocclusion. Normal values were observed in 32.8% of cases. The revealed anatomical features of the dental apparatus and facial cranial structures can lead to blockage of the lower jaw and predispose to the development of TMJ and masticatory muscle dysfunction.

Choice of rehabilitation method for patients with shortened lingual frenulum depending on clinical and anatomical characteristics of ankyloglossia

Abstract

Ankyloglossia leads to a decrease in the strength and endurance of the tongue muscles, which disrupts the myodynamic balance and negatively affects the development of the dentoalveolar system. The aim of the study was to develop and evaluate the effectiveness of a myogymnastic exercise programme aimed at improving the range of motion and elasticity of the lingual frenulum, based on the clinical and anatomical characteristics of ankyloglossia. During the study, 60 patients (29 boys and 31 girls) aged 6.5 to 14.8 years were examined. The patients were divided into three groups: a control group; a first study group, consisting of subjects performing exercises according to the traditional method; a second study group, consisting of subjects performing differentiated exercises tailored to the type of lingual frenulum. The dynamics of changes were assessed using an electronic pneumatic system (IOPI Medical), silicone capsules and a manometer — before the start of treatment, and then 2 and 4 months after. The study findings confirmed that a differentiated selection of exercises, taking into account the type of lingual frenulum, ensures faster and more effective correction of the functional parameters of the tongue muscles compared with established methodologies.

The use of Mycosan in prosthetic stomatitis and its effect on microcirculation in periodontal tissues

Abstract

Older people suffer from combined oral pathology, including prosthetic stomatitis and periodontitis. The aim of the study was to determine the dynamics of the periodontal condition in older age groups with acrylic prostheses and diagnosed prosthetic stomatitis on the background of Mycozone use. Materials and methods. The study involved 27 people aged 60 to 85 years (68±7.7) with a Kennedy class I mandibular defect. The chewing efficiency of the patients was restored with partial acrylic prostheses. The conclusion about the periodontal condition was made on the basis of perfusion units of microcirculation. The dynamics of prosthetic stomatitis severity was studied taking into account the Candida concentration in saliva. Microcirculation and Candida concentration were assessed before and a week after professional oral hygiene, as well as on the 8th and 15th days of the implementation of antifungal therapy, which consists in applying Mycozone to the inner surface of the prosthesis base. Results. Microcirculation in the area of the mandibular incisors increased after professional oral hygiene from 6.37±1.3 degrees to 13.84±2.7 degrees. No reliable effect of antifungal therapy on microcirculation in the area of the mandibular incisors has been established. The dynamics of Candida concentration indicates that 10 (37%) patients have fungal resistance to topical Mycozone treatment.

Optimized adhesive protocol: laboratory and clinical results

Abstract

A comprehensive clinical and laboratory comparative study was conducted to evaluate the efficacy of the "optimized" adhesive protocol developed by the authors and the "basic" adhesive protocol, performed in accordance with the manufacturer's instructions using a 5th-generation adhesive system (Etch & Rinse Two-Steps Adhesive System). It was found that the use of the "optimized" adhesive protocol does not reduce the strength of the adhesive bond between the light-cured composite material and the dentin of the "laboratory" teeth. In clinical settings, the "optimized" adhesive protocol demonstrated superior efficacy in terms of the following parameters: postoperative sensitivity (up to 14 days), marginal adhesion based on tactile testing, marginal adhesion based on vital staining, and the absence of pigmentation at the interface between the composite material and tooth tissue (at 18-24 months post-treatment).

Use of standardized test questionnaires in the diagnosis of adherence in dental patients

Abstract

Relevance: In Russian dental practice, there is a shortage of psychometrically validated instruments for assessing treatment adherence, developed specifically for the dental setting. Aim: To develop and test the full (96 items) and short (24 items) versions of questionnaires for assessing adherence to treatment in dental patients. Materials and Methods: The theoretical basis was the concept of vital metacompetence [2]. Psychometric testing included content, criterion, and external validity, involving expert assessments by dentists (n=59), and internal consistency (Cronbach’s α). Results: The full version (QDA-96) consists of 12 scales reflecting awareness, trust, and behavioral patterns. The short version (QDA-24) includes three aggregated scales (“Awareness”, “Trust”, “Behavior”) with a total score range of 24–96. A significant correlation (r=0.44; p≤0.01) was found between the total adherence score on the QDA-24 and independent physician assessment. Conclusion: The developed questionnaires are valid and allow differentiated assessment of the level and structure of adherence, which meets the modern requirements of a patientcentered approach [1].

Ankyloglossia in the children’s population of Voronezh: prevalence and clinical-anatomical characteristics

Abstract

Ankyloglossia is one of the most common developmental anomalies of the maxillofacial region in children. Its high population prevalence, association with occlusal disorders and speech impairments, alterations in swallowing patterns, as well as a pronounced gender predisposition and hereditary determination justify the need for early screening, a multidisciplinary approach to diagnosis and correction, and parental education regarding the clinical signs of ankyloglossia and the potential consequences of untreated cases. The aim of the study was to estimate the prevalence of ankyloglossia among the pediatric population of Voronezh, to characterize the gender‑specific features of the pathology and associated clinical disorders. Within the scope of the study, a comprehensive examination was conducted on 350 children aged 7–15 years. Ankyloglossia diagnosis was performed via clinical examination with a detailed assessment of the anatomical parameters of the lingual frenulum in accordance with established classifications. Additionally, medical history data were analyzed, along with the presence of concomitant disorders: occlusal anomalies, dental arch development issues, and abnormalities of individual teeth. The study results confirm a high prevalence of ankyloglossia (20.29 %) among school‑aged children and its impact on occlusal disturbances. The necessity of employing multiclassification approaches in the diagnosis of ankyloglossia has been substantiated. A multidisciplinary approach to the diagnosis and correction of this anomaly is required.

Correlation of radiographic changes in bone tissue in the area of dental implants with inflammatory and microbiological factors of resorption against the background of cardiovascular diseases

Abstract

The development of peri-implantitis and bone resorption is associated with a complex of factors, including microbial biofilm and systemic inflammation. Patients with cardiovascular disease (CVD) are at particular risk, as they have preexisting endothelial dysfunction, which increases the susceptibility of the vascular wall to the virulence factors of periodontal pathogens and proinflammatory cytokines. A total of 104 patients aged 27 to 72 years with integrated dental implants supported by orthopedic structures were studied for 5 years. The patients were divided into two groups based on the severity of bone resorption around the dental implants. Patients underwent questionnaires, CBCT, microbiological testing, and assessment of IL-6, TNF-α, and EN-1 levels. The concentration of the cytokine IL-6 was established corresponding to the degree of bone resorption in the implant area: the greatest increase in the indicator was observed in Group 2. The level of TNF-α in Group 1 did not exceed the reference value. When assessing the correlation of EN-1 with P. gingivalis, T. denticola, P. intermedia, a moderate relationship was found only with P. gingivalis among the entire sample. When assessing the correlation of IL-6 with periodontopathogenic microorganisms, a moderate relationship was found only with P. intermedia, TNF-α showed a weak correlation in all cases. The level of IL-6 and TNF-α showed a noticeable correlation with the degree of bone resorption in the entire sample. A relationship was established between the clinical and radiographic degree of bone resorption in the implant area, the concentration of cytokines IL-6, TNF-α and the occurrence of P. intermedia and T. denticola. Analysis of the cytokine profile may have prognostic value in planning implantation due to the sensitivity of IL-6 and TNF-α to the persistence of P. intermedia and P. gingivalis, which in turn affects the state of the vascular endothelium, and through cytokine proinflammatory activity on the resistance of jaw bone tissue in patients with a history of cardiovascular diseases.

A personalized navigation template for conduction anesthesia of the maxilla in implantology practice (clinical case)

Abstract

Reliable anesthesia is the foundation for successful maxillofacial surgery. Standard conduction techniques in the maxilla often carry a risk of complications due to individual differences in the nerve and vascular distribution. Digital technologies (cone beam computed tomography, 3D printing) allow the creation of personalized navigation templates for precise needle insertion, but their clinical value has not yet been adequately studied. The aim of this study was to demonstrate, using a clinical example, the effectiveness of a navigation template for conduction anesthesia in the maxilla during dental implantation. Material and methods. A 43-year-old female patient with chronic periodontitis of teeth 11, 12, 21, and 22 required their removal and immediate implantation. Based on CBCT data (additional infraorbital foramina and a cordate incisive foramen were detected) and intraoral scanning, a custom-made template for infraorbital and incisive anesthesia was designed and 3D printed. The anesthetic was administered intraorally through the guide bushings of the template, and the volume was controlled with a graduated syringe. Results and discussion. The total anesthetic consumption for two infraorbital and one incisor block was only 1.0 ml, which is significantly less than the usual volumes used with manual techniques. Complete anesthesia was achieved, allowing for the atraumatically extracted four teeth and the placement of two implants (Anyridge 4.0×13.0 mm and 4.0×11.5 mm) with primary stability of 20 Ncm. The postoperative period was uneventful, and sutures were removed on the 12th day. Conclusion. The use of a custom navigation template ensured precise placement of the target nerves, reduced the anesthetic volume, and allowed for a safe and effective procedure. This method merits further research and implementation in surgical dentistry.

About the problem of damage to the mucous membrane of the mouth when smoking electronic cigarettes

Abstract

Modern scientific research shows that electronic cigarettes (vapes) cause serious health problems, despite the widespread belief that they are relatively safe compared to traditional cigarettes. Scientific analysis indicates the risk to the respiratory system, cardiovascular system, cognitive functions, and the development of caries, periodontal diseases, and oral mucosa. The purpose of this study is to determine the types of pathological processes in the oral mucosa caused by electronic cigarette smoking. The authors examined patients who visited the dental clinic of the N.N. Burdenko Voronezh State Medical University with clinical manifestations of the consequences of e-cigarette smoking on the oral mucosa. The authors studied the pattern of oral mucosa lesions, which manifested as catarrhal inflammation of various parts of the oral mucosa, desquamative glossitis, and limited foci.

Features of adaptation of elderly and senile patients to removable dentures in orthopedic rehabilitation based on quality of life assessment

Abstract

The quality of life is an indicator that characterizes not only the physical and psychological state of the patient, but also his moral state. In recent years, there has been widespread dental pathology in the form of partial and complete adentia. Complete and partial absence of teeth (secondary adentia) affects the assessment of the patient's quality of life, as it contributes to the disruption of the process of chewing food. This has an effect on the digestive process. Also, dental defects cause prerequisites for the development of diseases of the temporomandibular joint and, as a result, pain syndrome. Violations of articulation and diction have quite complex consequences for the social status of patients in this category. These complications have an impact on communication skills. The aim of the study was to establish the features of adaptation of elderly and senile patients during orthopedic rehabilitation with removable plate prostheses with a two–layer base based on an assessment of the quality of life. Research materials and methods. The study included 60 patients with disease coding according to ICD 10 "K08.1 – tooth loss due to an accident, removal or localized periodontitis." The age of the patients ranged from 60 to 85 years; 23 men and 37 women. Elderly and senile age according to the WHO classification. The results of the conducted research. The OHIP - 14 questionnaire before the start of orthopedic treatment showed that the quality of life in the observed patients of the 3 groups was of "unsatisfactory" and "poor" levels. 6 months after treatment, 85% of patients in group 1 had an "unsatisfactory" quality of life, and 15% had a poor quality of life. In group 2, 35% of the subjects had a "satisfactory" quality of life, 55% of the patients had an "unsatisfactory" quality, and 10% had a "poor" quality. In group 3, 55% of patients had a "satisfactory" quality of life, and 45% of patients had an "unsatisfactory" quality of life. The study of the quality of life of elderly and senile patients with dental defects was conducted using the OHIP-14 questionnaire, which allowed for an objective assessment and confirmed the correctness of the choice of treatment and proved its effectiveness.

AB0 blood group and COVID-19: a biochemical perspective

Abstract

It is important to study the factors influencing the severity and course of COVID-19.In this study, we examined the relationship between ABO blood group and metabolic parameters in patients with COVID-19. Statistically significant differences in the metabolic composition of the blood were found between patients with COVID-19 and healthy volunteers depending on their blood group. In particular, patients with COVID-19 showed decreased levels of total protein and albumin, especially in individuals with A(II) and AB(IV) blood groups. Significant changes in glucose levels and lactate dehydrogenase activity were found in all COVID-19 patients, especially in patients with AB(IV) blood group. The results of intergroup comparison indicate statistically significant differences in the levels of calcium, chloride, albumin, creatinine, and C-reactive protein, as well as in ALT and AST activities in patients with COVID19. The obtained data indicate the influence of ABO blood type on metabolic processes and the inflammatory response in COVID-19, which must be taken into account when developing personalized approaches to the diagnosis, monitoring, and treatment of the new coronavirus infection.

The features of biochemical blood indicators in various severities of psoriasis

Abstract

This article presents data on the biochemical blood analysis of psoriasis patients, depending on the severity of the disease. It has been shown that with increasing psoriasis severity, the proportion of patients with abnormal biochemical parameters related to carbohydrate, lipid, and protein metabolism increases, indicating a link between psoriasis and the risk of developing or worsening metabolic syndrome. Metabolic syndrome can also contribute to the development of psoriatic disease due to the similarity of several common pathogenic factors, such as proinflammatory cytokines, oxidative stress, and intestinal microbiota imbalance. Therefore, for more effective psoriasis management, it is recommended to monitor regularly not only blood biochemical parameters but also the body's antioxidant status and intestinal microbiota, and to consider these parameters when prescribing therapy.

Abstract

Methyl/ethyl 6-amino-5-cyano-2-oxo-2-phenyl-2H-spiro[indoline-3,4-pyrano[2,3-c]pyrazole]-3carboxylates were synthesized and their inhibitory activity against cyclooxygenase 2 (COX-2). was studied. The analgesic activity of the synthesized compounds was evaluated using the "Acetic cramps" method. Two compounds were found, the analgesic activity of which statistically significantly exceeds the effect of the comparison drug, under the influence of which the number of convulsions in experimental animals decreases by more than 80%. The interaction of synthesized compounds in the form of S and R enantiomers with respect to COX-2 has been studied. A comparative analysis of the binding of enantiomers was performed and compounds with maximum inhibitory activity were found. Based on the results of molecular docking, a binding site was identified that provides a targeted search for compounds with analgesic activity in the COX-2 series under study. It has been established that this class of compounds is promising for further search for biologically active compounds with analgesic activity.

Lipase immobilization on polyacrylate hydrogel with correction for dynamic swelling of the carrier

Abstract

In clinical laboratory diagnostics and biotechnology, there is a growing demand for stable, regenerable enzyme preparations to automate analytical procedures. Immobilization of lipase on hydrogels is a promising approach$ however, the traditional calculation of activity per mass of dry support introduces systematic error due to the dynamic swelling of the matrix in aqueous media. This study investigates the swelling kinetics of polyacrylate hydrogel (cross-linking degree 1 – 2 %) in solutions of Candida rugose lipase at 30 – 55 °C. It was established that the swelling ratio reaches its maximum at 40 °C and an enzyme concentration of 0,1 mg/mL. An algorithm was developed for calculating specific activity with normalization to the mass of the equilibrium-swollen gel, which eliminates overestimation or underestimation of results by 15 – 30 %. Immobilization conditions (40 – 45 °C, 48 h, 0,1 mg/mL) are substantiated and recommended for stable reagent systems intended for clinical diagnostic applications.

Quantitative determination of atorvastatin and glycyrrhizic acid in a combined drug by HPLC

Abstract

A method for the quantitative determination of atorvastatin (ATV) and glycyrrhizic acid (GK) as part of a combined drug in tablet form by high-performance liquid chromatography (HPLC) with diodematrix detection has been developed and validated. Chromatographic separation was performed on a Poroshell C18 column (Agilent 1260) using a mobile phase acetate buffer (0.05 M, pH 5.0) – acetonitrile and diode matrix detection. The quantitative content of the components is calculated using the external standard method. According to the results of five repeated measurements (n=5), the average ATP content found was 8.734 mg (87.34% of the claimed 10 mg) with RSD of 1.16%, the HA content was 46.27 mg (92.55% of the claimed 50 mg) with RSD of 1.21%; the relative error of the average result did not exceed ~ 2%. The methodology is validated in terms of specificity, linearity, precision (repeatability), correctness and reproducibility; the results obtained comply with the requirements of the State Pharmacopoeia of the Republic of Kazakhstan and the Pharmacopoeia of the EAEU.

Antioxidant and antiradical activity of alkyl 6´-amino-5´-cyano-2-oxo-2´H-spiro[indolin-3,4-pyrano[2,3-c]pyrazole]-3´-carboxylates

Abstract

9 compounds of a number of alkyl 6´-amino-5´-cyano-2-oxo-2´Н-spiro[indolin-3,4-pyraho[2,3c]pyrazole]-3´-carboxylates were resynthesized, and their effect on oxidative stress induced by a solution of hydrogen peroxide in Escherichia coli cell culture, strain "Ecolume", was studied. The antiradical activity was evaluated by the method of indirect photoelectrocolorimetric determination of the stable radical 2,2diphenyl-1-picrylhydrazyl (DPPG). It was found that the studied compounds do not possess actioxidant properties and exhibit weak antiradical activity - the range of free radical loss values was 0.2 - 30%, while 2 compounds showed a proradical effect.

Application of spectrophotometry for quality control of candesartan tablet forms

Abstract

The article presents the development of a method for the quantitative determination of candesartan cilexetil in tablets by UV spectrophotometry. The relevance of the study is due to the high prevalence of arterial hypertension among the population, which increases the risk of developing severe cardiovascular diseases such as myocardial infarction and cerebral stroke and the need for reliable quality control of antihypertensive drugs. The study showed that the structure of candesartan cilexetil allows the use of UV spectrophotometry, and benzimidazole, due to the similarity of its absorption spectrum, is a suitable substance for use as an external standard. The developed technique demonstrates high sensitivity, specificity and reproducibility, with a relative error of less than 2%, which makes it promising for simplifying and reducing the cost of quality control of medicines.

Investigation of the composition and antimicrobial activity of polysaccharides of common cocklebur (Xanthium strumarium L.)

Abstract

Due to the limited distribution and difficult harvesting conditions of many medicinal plants, it becomes urgent to search for new sources of biologically active substances (BAS) among weedy and invasive plant species, such as common cocklebur (Xanthium strumarium L.). Purpose. To carry out a qualitative and quantitative analysis of polysaccharides of cocklebur herb and evaluate the antimicrobial activity of individual fractions of polysaccharides. Materials and methods. The aerial part of the common cocklebur was used as the object of the study. The quantitative content of polysaccharide fractions was assessed by gravimetry. The method of radial diffusion into agar and the method of serial dilutions were used to evaluate antimicrobial activity. Results and discussion. The content of water-soluble polysaccharides was 9.13±0.5%, pectin substances 14.31±0.7%, hemicellulose A and hemicellulose B - 1.82±0.1% and 10.57±0.5%, respectively. The presence of free glucose, galactose, rhamnose, xylose and arabinose was detected in the raw materials. After complete acid hydrolysis, it was found that water-soluble polysaccharides and pectin substances contain mainly arabinose, glucose and rhamnose as monomers, rhamnose and arabinose were found in hemicellulose A, and rhamnose, xylose, galactose and arabinose in hemicellulose B. The analysis of antimicrobial activity demonstrated the greatest effectiveness of pectin substances against Staphylococcus aureus and Escherichia coli. Conclusion. It has been established that the herb common cocklebur is a promising source of polysaccharides, including antimicrobial agents.

Quality assessment and determination of optimal harvesting periods for Norway spruce roots

Abstract

The aim of this work is to study the content of the main groups of biologically active substances (BAS) in common spruce (Picea abies) roots to determine quality indicators and optimal harvesting times. Materials and methods. Samples of raw material for the study were collected in the Ilyinsky district of Perm Krai from May to November 2024. Roots up to 5 cm in diameter were harvested. The essential oil content was determined by hydrodistillation of crushed raw material samples using a Ginsberg apparatus. The flavonoid content was determined spectrophotometrically, calculated as chlorogenic acid equivalent. The antiradical activity of a decoction of common spruce roots was assessed using the reaction with the stable free radical 2,2-diphenyl-1-picrylhydrazyl (DPPH). Results. The study found that the total content of phenolic compounds, calculated as chlorogenic acid equivalent, in common spruce roots ranges from 8.12% to 10.58%, while the essential oil content varies from 0.22% to 0.35%. It was established that no significant seasonal fluctuations are observed in the content of the main groups of BAS in common spruce roots. Conclusion. Based on the obtained data, numerical indicators for quality assessment of common spruce roots and the optimal harvesting time are proposed.

Pharmacognostic characterization and quantitative evaluation of biologically active substances Iris scariosa L. from Central Kazakhstan

Abstract

This study presents a pharmacognostic characterization of the rhizomes of Iris scariosa L. from Central Kazakhstan, including determination of total extractives, mineral composition, and quantitative content of major groups of bioactive compounds. Materials and Methods: The study material consisted of rhizomes of I. scariosa collected during the flowering phase in the Karaagash Forestry (Zhanaarkinsky District, Ulytau Region, Kazakhstan). Bioactive compounds were analyzed using pharmacopoeial and standard methods: spectrophotometry, titration, and gravimetric analysis. Mineral composition was assessed by atomic absorption spectroscopy. All experiments were performed in three independent replicates, and results are expressed as mean ± standard deviation. Results: The rhizomes were dominated by total extractives (17.81 %) and polysaccharides (3.43 %), with moderate levels of coumarins, tannins, and alkaloids, and minimal levels of flavonoids, saponins, and free organic acids. Among macroelements, potassium (51.608 ± 0.051 mg/g), calcium (41.170 ± 0.121 mg/g), and magnesium (8.305 ± 0.028 mg/g) predominated. Microelements Fe, Zn, Mn, Cu, and Ni were present in trace, physiologically relevant amounts, while toxic elements Pb and Cd were not detected. Conclusions: I. scariosa is characterized by the dominance of total extractives and polysaccharides, moderate levels of coumarins, tannins, and alkaloids, and minimal amounts of saponins and organic acids, confirming its pharmacological potential and supporting its prospects for standardization as medicinal plant material.
