Experimental evaluation of the application of polyethereketone in the manufacture of removable covering dentures supported on dental implants

Abstract

Improving the technology of dental prosthetics, especially those based on dental implants, is closely related to the development and research of structural materials. The article provides data on a comparative study of the physical and mechanical properties of polyetheretherketone as a material for the manufacture of removable dentures supported by dental implants. Data on indicators of bending strength, diametric tensile strength and coefficient of friction for steel were studied. An experiment to study the retention capacity of a telescopic fixation system for removable dentures is described and the data obtained are analyzed.

Investigation of the effect of fixing zinc-phosphate cements on the tissues surrounding a fixed denture structure

Abstract

Despite the rapid development of orthopedic dentistry and the release of a large number of materials used to fix fixed structures, it is impossible to single out among them an ideal one for every case. There are several classes with significantly different characteristics. Even representatives of the same group have significant differences in properties, which is associated with continuous work on the recipe to adapt to modern requirements.

Every orthopedic dentist tries to choose for himself a material that satisfies all parameters, used in dental practice as a fixing fixed structures. He pays special attention not only to the convenience of working with cements or their physico-chemical, physico-mechanical properties, but also to its bioinertness and biocompatibility.

To assess the effect of fixing materials on surrounding tissues, a comparative assessment was carried out with the measurement of the toxicity index of materials and the papillary-marginal-alveolar index (PMA).

Abstract

The oral cavity is essentially an ecological system. This system is part of both the organism itself and the external environment. The normal microflora of the oral cavity guarantees resistance to colonization. This can be designated as the main goal for any factor that directly or indirectly affects adhesion. It also affects the normal resistance of the oral cavity to external manifestations. The human body is constantly functioning and in close contact with the surrounding world and its organisms. Thus, the human body itself is densely populated with various types of microorganisms. These microorganisms are divided by origin, composition, type, size, type of vital activity. In order to study the biotype of the human body as thoroughly and in detail as possible, an approach based on the ecological principle has been formed in this study.

To identify the patterns of the existence of microorganisms, we apply an ecological approach that allows us to reliably describe the structure of bacterial communities that make up the biocenosis. Removable plate prosthesis will necessarily affect the human body in some way, and this measure of influence can be both positive and negative. A positive effect is observed in the exact and complete replacement of defects in the dentition. The negative effect can be characterized by both inflammatory reactions and mechanical damage. Also, the materials used for the manufacture of removable plate prostheses may be incorrectly selected. Depending on the already existing diseases of a person, his physiological characteristics, habits, it is necessary to treat the choice of consumables as appropriately as possible.

It is impossible to overestimate the positive effect of the microflora of the human body in the processes of metabolism, digestion, etc. Thus, the issue of preservation of human microflora and prevention of dysbiosis is relevant in modern science.

INTERRELATION RECURRENCE RATE OF CHEILITIS FROM THE LEVELS OF VITAMIN D

Abstract

Vitamin D deficiency is a widespread problem among carriers, people living in acute lack of sunlight in northern latitude regions, this is the result of low serum concentrations of the vitamin. Chronic lip fissure and meteorological cheilitis are the most common forms of cheilitis among residents of the Far North. The role of vitamin D deficiency has not been studied in patients with these diagnoses. The purpose of the study is to measure the level of vitamin D in the blood serum in patients with chronic lip fissure and meteorological cheilitis, as well as to dynamically study the dependence cases of disease recurrence on the concentration vitamin D. The study was conducted in 79 residents of the Murmansk region for 12 months. Diagnostic results have established that low serum vitamin D concentrations support the recurrence of meteorological cheilitis and chronic cleft lip. The use of vitamin D to maintain a normal level of vitamin in the blood serum improves the regenerative ability of the lip skin, maintains the barrier function of the epithelium, and reduces cheilitis recurrence rate. The study found that in the conditions of the Far North one of the mandatory components of the prevention and pathogenetic therapy of meteorological cheilitis and chronic lip fissure should be vitamin therapy with cholecalciferol preparations.

Abstract

The issues of the need for immobilization and postoperative orthopedic rehabilitation of patients after fractures of the lower jaw were studied to eliminate the risk of complications such as violation of the occlusal relationship of the teeth, contracture of the lower jaw, increased muscle tone, and as a result, the development of pathology from the temporomandibular joint. A comparative analysis of electromyographic parameters and palpation of the masticatory and temporal muscles in patients with fractures of the lower jaw and healthy individuals proves the need for the use of orthopedic correction methods for the rehabilitation of the victims.

THE SIGNIFICANCE OF BACTERIOSCOPY IN THE COMPLEX OF DIAGNOSTICS OF COMPLICATED FORMS OF DENTAL CARIES OF PATIENTS WHO PREVIOUSLY HAVE COVID-19

Abstract

The issues of the complexity of diagnosing diseases of the pulpoperiodontal complex of teeth of patients who had previously recovered from COVID-19, as well as the need to select the most adapted algorithm for the treatment of pathology, depending on the clinical manifestations in the oral cavity, were studied. An increase in the activity of opportunistic microorganisms, which normally do not show activity in the oral cavity, gives rise to the development of various pathologies of hard tissues of the teeth, the pulp and periodontium. The expediency and necessity of including in this category of patients, in addition to the standard dental examination, also additional examination methods (for example, blood test, bacterioscopy, bacteriological tests, testing of the microvasculature, and others) before the start of any dental treatment. Emphasis is placed on the elimination of a certain type of microorganism, identified with the help of additional examination methods, thereby preventing the occurrence of long-term complications after endodontic treatment of the tooth.

Results of a study of zinc-phosphate cements for fixation of fixed orthopedic structures under the influence of material hardening time

Abstract

Patients often prefer non-removable prosthesis designs to replace dental row defects, given their undeniable benefits. However, there is a possibility of developing complications during prosthetics, it is impossible to completely exclude medical errors and errors of the dental technician.

Dentists know that in 20% of cases, prosthetics with non-removable types of prostheses end unsuccessfully due to errors in treatment, as well as improper use of fixing cements at the stage of storing the prosthesis. About 20% of cases are complications in the near future after prosthetics during the operation of the prosthesis. The proportion of complications due to carious tooth damage under prostheses can reach more than 49%.

Non-removable prostheses, firmly fixed by zinc-phosphate cements, gain a number of advantages by choosing this particular material: a slight percentage of shrinkage; low percent solubility; increased strength; creation of a holding layer between tissues of the oral cavity and a selected type of prosthesis.

Our study is devoted to the issues of assessing the most significant qualities of fixing cements, which is of great practical importance.

Laboratory assessment of the accuracy of the fit of artificial crowns to tooth stumps

Abstract

Relevance. Among the variety of reasons for unsatisfactory results of treatment of patients with artificial crowns and their premature removal or cementation, the researchers highlight the development of secondary caries. One of the significant reasons for its occurrence is the quality of the marginal fit of artificial crowns.

The goal is to perform a laboratory assessment of the thickness of the cement film in the area of ​ ​ the edge of the artificial crown and establish the degree of its influence on the probability of the development of the carious process.

Procedure. Splints of 50 freshly removed teeth covered with artificial crowns were made, on which the thickness of the cement film was measured, and the presence of caries at the edge of the artificial crown was determined.

Results. The development of caries under the artificial crown is typical of teeth dissected with a ledge. The threshold value of the thickness of the cement film, above which caries more often developed in the studied sample of teeth, was 115-120 microns. The probability of unacceptable thickness of the cement film and the development of caries in teeth prepared with a shoulder is 6.6 times higher than in preparation without a shoulder.

Clinical substantiation of the platelet-rich plasma effectiveness in complex surgical treatment of gingival recession

Abstract

Gingival recession is a non-inflammatory pathological process characterized by apical migration of the marginal gingiva below the CEJ. The use of platelet-rich plasma is one of the current technologies in the complex treatment of gingival recession. The aim of the study was to increase the effectiveness of surgical treatment of gingival recession using platelet plasma. As a result of the treatment of gingival recession I, II and III Miller’s classes, the coverage area of the recipient surface was 84%, 77% and 60%, accordingly; the thickness of the keratinized gingiva increased by an average of 2 times. The use of platelet plasma in the complex treatment of gingival recession optimizes the healing period in the regeneration phase, improves the microcirculation of periodontal tissues and reduces the duration of treatment by an average of three days; shows more efficiency due to less physiological shrinkage of the graft over time.

The effectiveness of dental orthopedic manipulations performed by students with different lateralization of brain functions

Abstract

This article highlights the peculiarities of the influence of the dominant hemisphere on the effectiveness of orthopedic dental manipulations. Left-hemisphere and right-hemisphere students dissected the first molar of the lower jaw for a metal-ceramic crown with different hands. The effectiveness of the students' work was evaluated according to the criteria of time spent on preparation (A), thickness of grinding from the occlusal (B) and contact surfaces of the tooth (C), convergence angle (D), and the width of the formed ledge (E). According to the results of the study, it became known that right-hemispheric students working with the right hand showed abnormal results: the A indicator was 16.7 minutes; B - 1.6 mm; C - 1.7 mm; D - 22˚; E - 1.9 mm. The results of these students, revealed when working with the leading hand, correspond to normal parameters: indicator A - 14.5 min; B - 1.7 mm; C - 1.3 mm; D - 14˚; E - 1.2 mm. Left hemispheric students working with the right hand also showed results within the normal range: indicator A - 14.4 min; B - 1.8 mm; C - 1.3 mm; D - 12˚; E - 1.1 mm. The conducted research indicates the need to pay increased attention to the training of left-handed students and to provide this group of students with special educational equipment.

Influence of factors of systemic inflammation and endothelial dysfunction in somatic pathologies on the success of dental implantation (literature review)

Abstract

In the presented literature review, the issue of the influence of general somatic factors associated with diseases of the cardiovascular and endocrine systems on the success of dental implantation was studied. The issues of the relationship of systemic inflammatory processes and periodontal status with the dental health of patients with background pathologies associated with an imbalance of pro- and anti-inflammatory cytokines, insulin and glucose in the blood in diabetes mellitus and obesity, factors of endothelial dysfunction in atherosclerosis, hypertension and coronary heart disease are highlighted. Based on the analyzed information, it was confirmed that poor-quality history taking, the lack of a thorough diagnostic examination (biochemical blood test, coagulogram, reaction to the presence of specific infections and viral diseases), ignoring the need to correct existing local and systemic inflammatory phenomena before dental implantation can lead to a violation the process of osseointegration, the occurrence of peri-implantitis, implant rejection reactions. In this regard, it can be concluded that when planning complex dental treatment, attention should be paid not only to local dental symptoms, but also to the general history and laboratory data, which will improve the quality of dental implantation.

SPECTROPHOTO AND DIGITAL COLORIMETRIC DETERMINATION INDICATOR OF BETA-ADRENOREACTIVITY OF ERYTHROCYTE MEMBRANES OF PATIENTS WITH BRONCHIAL ASTHMA

Abstract

The change in the adrenoreactive properties of membrane and cellular structures under the influence of catecholamines makes it possible to consider the beta-adrenergic reactivity of the erythrocyte membrane (β-ARM) as a systemic indicator of the organism's adrenoreactivity. To assess adrenoreactivity, a technique is used based on a quantitative assessment of the degree of inhibition of hypoosmotic hemolysis of erythrocytes in vitro in the presence of the beta-blocker propranolol. The article presents data on the comparison of spectrophoto- and colorimetric determination of erythrocyte membrane beta-adrenoreactivity based on the registration of erythrocyte hemolysis with digital signal processing. A smartphone was used to register the images of the analyzed samples. Photography was carried out in a special box under standard lighting conditions. The chromaticity of the obtained images was evaluated using the Photoshop program within the RGB color model intended for computer color rendering. The obtained RGB values were used for quantitative determinations. The study was performed on 20 blood samples of patients suffering from bronchial asthma. The discrepancy between the results of determining β-ARM using the colorimetric and spetrophotometric methods was no more than 5.6%, which indicates the possibility of replacing the spectrophotometer with image registration in digital format using software.

INVESTIGATION OF PROBIOTICS-SORPTION PROPERTIES OF THE COMPOSITION BASED ON POMACE

Abstract

The use of enterosorbents to relieve intoxication and to increase the content of prebiotics in the body is a topical problem of every second metropolis resident. The pharmaceutical market offers a huge number of enterosorbents, some of which can cause negative consequences (for example, mechanical damage, constipation etc.). The others that are more compatible with the body, for example, pectins, are expensive due to the energy-intensive process of their isolation. We offer a preparation based on pomace powder (60%) with the addition of starch, kaolin and pectin from sea grass eelgrass (Zostera marina) (6%). The powder is obtained by a previously developed and patented method and has an established sufficiently high sorption activity. At the same time, apple pomace also contains pectin, which is a substrate for the growth of beneficial microorganisms living in the colon.

The article presents the data of a comparative analysis of the sorption activity of the developed powder and a number of enterosorbents offered by the pharmaceutical market in relation to standard test substances: methylene blue and pharmaceutical gelatin. It is shown that the composite enterosorbent obtains effective sorption properties and equals to common sorbents. When dissolved in water, the powder acquires a gel-like consistency that gives it additional therapeutic and preventive properties. The use of apple pomace as a basis enables to reduce the cost of producing expensive pectin and make it more affordable for the population.
