CLINICAL AND LABORATORY FEATURES RESPIRATORY-SYNCYTIAL INFECTION OF CHILDREN IN THE FIRST YEAR OF LIFE

Abstract

Currently, respiratory-syncytial viral infection (RSVI) is the most common cause of severe lower respiratory tract infection in young age children, being a heavy burden on pediatrics. The aim of our study was to study the clinical and laboratory features of RSVI, which leading to hospitalization of children in the first year of life. In observation were 35 children with acute respiratory tract infection clinic and positive result of PCR on RSVI smears from nasopharynx. The majority of patients (94.3%) had background pathology, unfavorable obstetric and gynecological history of mothers (77.1%). The largest number of children with RVSI were hospitalized in the cold season. The leading clinical manifestation of infection is the defeat of the lower respiratory tract with the development of obstructive bronchitis 65.7% and bronchiolitis 25.7% with slightly pronounced intoxication. 17.1% of children with RSVI had laryngitis syndrome without stenosis, 11.1% had pneumonia.

RHINOVIRUS INFECTION IN CHILDREN

Abstract

Rhinovirus infection (RI) is up to 25%-50% of SARS. Despite the "banality" of clinical manifestations rhinoviruses often lead to complications (otitis, sinusitis, bronchitis, pneumonia, etc.), determining the indications for hospitalization, especially in young children. In order to study the peculiarities of the course of RI, 56 children with laboratory-confirmed infection aged 10 days to 14 years were examined on the basis of the profile Department of the children's infectious diseases hospital in 2018. The age structure of RI among hospitalized patients was dominated by young children, whose infection proceeded as mono-viral, and had typical clinical manifestations: prolonged rhinitis against the background of normal, rarely subfebrile temperature. In children older than one year, RI was observed as a combined infection with other respiratory viruses and respiratory mycoplasmosis, the clinical picture of which was characterized by a more pronounced feverish intoxication syndrome, involvement in the lower respiratory tract with less pronounced catarrhal manifestations, which was probably due to other respiratory pathogens.

MINOR ABNORMALITIES OF HEART DEVELOPMENT IN CHILDREN WITH DIGESTIVE DISORDERS COMBINED WITH CONNECTIVE TISSUE DYSPLASIA

Abstract

The article reports on clinical and morphological manifestations of minor abnormalities of heart development in children with the digestive system diseases combined with connective tissue dysplasia. The study included 126 patients with various gastropathology combined with connective tissue dysplasia, who were being examined and treated in the gastroenterological department. The results of the study showed that in children with gastropathology combined with connective tissue dysplasia, its phenotypic signs and somatic stigma from the digestive tract are often combined with connective tissue dysplasia of the heart, the pathologies of the chordal apparatus and mitral valve prolapse being the most common of them.

COMPLICATIONS AND ERRORS OF SURGICAL TREATMENT OF DAMAGE TO THE THORACIC AND LUMBAR VERTEBRAE

Abstract

Over the past 70 years, the number of victims with spinal cord injury has increased by 200 times. Russia annually recorded about 8-9 thousand cases of spinal cord injury, in 80% of cases it is the prerogative of persons 15-45 years. The problem of treatment of victims with spinal injury is an acute problem of domestic neurosurgery. The article presents a review of the literature on the errors of neurosurgical treatment of injuries of thoracic and lumbar vertebrae. Of the main tactical errors, the authors identified inadequate instrumental fixation in extremely unstable fractures (type B and C). The paper deals with the causes of post-traumatic deformation of the spine. The following tactical errors are also identified: inadequate decompression, failure or failure in deformation correction, purposeful destruction of the rear support column. Technical error: violations in the technique of metal implantation, fracture and malposition rods; fracture, dislocation and mariposite pedicle screw. Close attention is paid to the mistakes made by the patient and the shortcomings of metal structures. Particular interest is paid to infectious (pneumonia, uroinfection, bedsores, suppuration of postoperative wounds) and neurological (liquorrhea, durotomy, radiculopathy) complications. The causes of these complications were studied in detail and measures to reduce them were proposed. In conclusion, the authors proposed ways to reduce the complications of surgical treatment of spinal cord injury.

NEPHROPATHY DEVELOPMENT IN CHILDREN, DEPENDING ON THE BODY WEIGHT AT BIRTH

Abstract

The article describes epidemiological screening programs held in rural areas of the Lipetsk region to early reveal urinary system disorders in children. Incidence of nephropathy development in children having body weight at birth renal loading was studied. The article informs of 5032 children aged 0 - 15 years (2593 females and 2439 males among them) having been examined following the same program. 193 children appear to suffer from nephropathy. Research findings demonstrated birth weight affects the incidence of nephropathy in children. Nephropathies most often develop in children born with signs of hypotrophy. A high body weight at birth is also a risk factor for kidney disease.

Abstract

Over the last century, the intensification of the educational process increases dramatically. Motor activity of students is sharply reduced and leads to a decrease in the adaptive capacity of them. Frequent stressful situations lead to functional insufficiency of the cardiovascular and nervous systems and the development of psychosomatic diseases. Studies conducted by students of the Voronezh medical state University on the apparatus "Cardiocode" revealed a decrease in cardiac parameters. Rehabilitation programs were offered to students with the revealed pathology which promoted improvement of indicators of cardiovascular system and strengthening of health of students.

VIRAL HEPATITIS C: GENETIC, IMMUNOLOGICAL AND CLINICAL PARALLELS

Abstract

Hepatitis C virus (HCV) is one of the most common viruses in the world. About 170 million people are carriers of this chronic infection. Currently, drugs and effective, direct, antiviral treatments have been developed, eliminating more than 90% of HCV cases. However, many people infected with the virus do not have access to these expensive drugs and are unable to undergo therapy. An estimated 399,000 people die each year due to cirrhosis or hepatocellular carcinoma associated with current or past chronic HCV infection. Detailed study of the virus will help to better understand its life cycle, which will expand treatment options and/or limit the development of pathology associated with disorders in the immune system of a patient infected with HCV. Viral hepatitis C has high social and economic significance along with human immunodeficiency virus and other infectious diseases. Data of scientific studies showing that preparations of direct action often do not lead to complete cure from HCV and practically do not affect issues of treatment of consequences - hepatocillular carcinoma and cirrhosis of liver.

COMPOSITION DEVELOPMENT AND PHARMACEUTICO-TECHNOLOGICAL PROPERTIES STUDY OF A TABLETED DOSAGE FORM MADE ON THE BASIS OF THE PRESSED APPLE SKINS AND POTATO STARCH

Abstract

The paper is devoted to composition development of a tableted dosage form on the basis of pressed apple skins milled to powder-like state (humidity 8.1±0.5%, particle size 0.5-1.0mm) and potato starch (State Standard 7699-78 ). The choice of potato starch is determined by a number of its specific properties such as a less degree of crystallinity in comparison with potato starch, enzymoresistance, low amino acids and protein content. These factors should be taken into consideration in enterosorbents development for the people suffering from diabetes mellitus, obesity, colitis, large intestine and rectum cancer, as well as for children with a number of genetic disorders. Tablets composed of 0.50g of pressed apple skins and 0.50g of potato starch were obtained by stamp pressing method. Pharmaceutico-technological properties of these tablets satisfy the requirements for such a dosage form according to technological normative documents. At a ratio of the height to the diameter (38.46%) and disintegration test (8 minutes) the dosage form meets the requirements of Industry Standard 64-072-89 and General Pharmacopoeia Article 1.4.2.0009.15. Sorption activity of the obtained tablets and pressed apple skins powder towards nickel cations (49.89±0.50 mg/g; 51.43±0.50 mg/g) and zinc cations (62.76±0.50 mg/g; 61.33±0.50 mg/g) was determined by chelatometry method respectively. Non-essential differences in sorption activity (less than 1%) of the obtained tablets from the powder form indicate that potato starch does not influence sorption properties of the preparation, but improves its technological characteristics. All mentioned above provides the opportunity to consider the developed dosage form as a promising, convenient, natural and inexpensive enterosorbent.

Abstract

The desire to increase the economic efficiency of pharmaceutical business in conditions of unstable market situation and high competition requires from subjects the circulation of medicines the introduction of modern approaches to the organization of their activities. The article discusses the advantages of practical application of the process approach to the management of the pharmacy organization, which contribute to the maintenance and growth of its competitiveness. On the basis of the concept of a balanced scorecard, present the main elements of the schematic simulation of the business process "Sale of goods" in the pharmacy organization. Offer a methodology for rapid monitoring of the indicators of sale of goods in a retail pharmaceutical organization. The object of the study was the reporting data of the program "1С: Enterprise" of the analyzed pharmacy organizations carrying out pharmaceutical activities in the territory of the Voronezh and Lipets regions, for 2016-2017. The methodological basis of work was the principles of functional-logical modeling, system and process analyses, computer programming. In the course of the study, the components that make up the system of cause-and-effect relations of the business process of the pharmacy organization have been identified. As modern technological support of internal business processes the method of operational monitoring of performance indicators of pharmaceutical enterprise is proposed. A software product has been formed on the PowerBI Business Intelligence platform, which allows you to process and visualize imported information, forming a business report in real time. The practical application of business analytics methods is a significant element in the modeling of business processes of the pharmacy organization and contributes to the optimization of labor functions of specialists in the field of pharmaceutical activity management, objective assessment of the degree of management efficiency, competitiveness of the organization of retail trade in medicines, as well as allows to structure the system of management solutions for optimization of financial and economic activity of the pharmacy enterprise.

DISCURSIVE ASPECTS OF HEALTH PROMOTION: A REVIEW OF MODERN COMMUNICATIVE STRATEGIES

Abstract

The article deals with the comparative analysis of foreign and national researchers’ perspectives on preventive discourse in its broad sense and the language of preventive discourse as a mediator between health professionals and common people, i.e. the target audience of preventive medicine. The relevance of the study is practical in prevention, which success depends on successful communication and relevant communicative strategies. The material of the research comprises foreign and national authors’ scientific papers. The objective is to identify the concept of texts of preventive medicine, their purpose, genres and techniques, to define the main communicative strategies and their functions based on the analysed material. Despite the absence of a single interpretation of communication in the sphere of preventive medicine, the authors conclude that it is possible to equate the notions of “health promotion” and the language of preventive medicine, where the language acts as a communicative tool of preventive discourse. The authors suggest to use the concept “health promotion”.
