DENTAL STATUS OF ТEN YEAR OLD CHILDREN OF THE CITY OF SMOLENSK
Abstract
The aim of our study was to compare the prevalence and intensity of dental caries in children, the prevalence of dental anomalies in children aged 10 years, which were carried out preventive examinations in 2015, 2016 and 2017. From 01.01.2018 year came into force the new order of Ministry of health of the Russian Federation "About carrying out of preventive medical examinations of minors" No. 514н, according to which the dentist is part of the medical Commission. In children aged ten years, the prevalence of caries of temporary teeth increased from 73,3% in 2016 to 67,9% in 2017, in 2015 - 66,6% (р>0,05). The intensity of caries was 1.4 in 2017 less than in 2016 (1,6) and in 2015 (1,7) (p>0.05). Preventive examinations of children allow timely detection of existing dental pathology and assess the effectiveness of dental care.

EXPERIENCE IN PREVENTING INFLAMMATORY COMPLICATIONS IN PATIENTS AFTER COMPLEX TOOTH EXTRACTION
Abstract
Operations to remove teeth on the lower jaw, due to the peculiarities of its anatomical structure, are often accompanied by the development of postoperative complications. The clinical picture is most pronounced in cases of multiple treated, significantly destroyed teeth, when using resorcinol formalin and other situations. The relevance of issues of prevention of inflammatory complications in the postoperative period occupies an important place in modern surgical dentistry. Today, an important area is the development of comprehensive approaches to the prevention and treatment of complications after tooth extraction. Study questions for of the antioxidant and Pro-oxidant system of the body is of essential importance, assessing total compensatory potential of the human body, imbalance of equilibrium which leads to the development of oxidative stress, thus starting a pathogenetic factor in many inflammatory processes. The study of domestic and foreign literature predisposes to a more detailed study of shifts in the antioxidant and Pro-oxidant systems in complex dental extraction operations and the effect of various drugs on oxidative stress.

ULTRASOUND TREATMENT OF TEETH WITH OZONATED WATER AS METHOD OF SECONDARY PREVENTION OF PERIODONTITIS
Abstract
Inflammatory-destructive periodontal diseases are one of the most important challenges in dentistry, while the task of treating and preventing them remains unsolved. In this situation, the most technologically advanced and efficient techniques - ultrasonic scaling for eliminating the main etiological factor of periodontitis - plaque and calculus, - as well as the Vector Fluid polishing device for rehabilitation of periodontal pockets and minimally invasive root resection with hydroxylapatite, are becoming more widespread. In this study, a natural antiseptic - medical ozone - was used as a treatment option for gum recovery instead of conventional antibacterial agents. The study included 93 patients with chronic generalized periodontitis of moderate severity. The results of clinical, laboratory and radiological investigations supported the fact that ultrasound polishing of tooth roots using hydroxylapatite included in the complex therapy with a Vector Fluid polish together with simultaneous antiseptic treatment of periodontal pockets using ozonated water appears to be an effective method of treatment and prevention of periodontitis compared with the conventional approach involving administration of antimicrobial agents.

DIFFERENTIAL DIAGNOSIS OF PRIMARY THE LOSS OF THE MINERAL COMPONENT OF TOOTH ENAMEL USING A CONE-BEAM COMPUTER TOMOGRAPHY
Abstract
Clinical differential diagnosis of primary hypomineralization and demineralization is a complex task, which fundamentally determines the tactics of further treatment of both childrens and adults. It is possible to establish exact quantitative criteria for the differential diagnosis of these conditions using CT densitometry. Native enamel without pathological changes has microhardness values in the range from 2800 to 4000 HU (Hausfields). Research have shown that with primary congenital hypomineralization of permanent teeth, these values are reduced to 1600-2000 HU, with subsurface demineralization - 1500-2600 HU with a decrease in hardness of more than 30% in the layers lying under the preserved enamel area without changing the thickness of the enamel in the area equator tooth. When surface demineralization values lie in the region from 1600 to 2900 HU, but there is a thinning of the enamel layer of less than 1 mm in the equatorial region of the tooth. The derived patterns make it possible to more reliably and confidently talk about the genesis of the identified pathological changes, and therefore - to choose the most optimal way to treat these conditions.

THE PROBLEM OF BONE REGENERATION IN REPEATED ENDODONTIC TREATMENT: BIOACTIVE SILERS, MECHANISM OF ACTION
Abstract
Modern requirements for biomaterials used as dental implants are due to the need for maximum compliance with the structure, composition and physico-biological properties of native bone. General and applied clinical requirements for selection of materials for implantation are considered. It is emphasized that in each clinical situation, bone defects have their own characteristics related to the etiology, length, general matic status of the patient and other factors. The work on the creation and optimization of existing biomaterials with given properties is now extremely relevant. There are presented clinical examples of use of bioactive obturation systems containing biosteclo, having activity on formation of hydroxyapatite crystals. Maximum quality of sealing and biocompatibility of these materials are confirmed.

BIOMECHANICAL CHARACTERISTICS OF ENDO-PERIODONTAL LESIONS OF SINGLE ROOT TEETH
Abstract
The state of extensive deformation of ‘tooth-parodont’ system has been explored considering the stage of the alveolar crest bone resorption and anatomical consequences of tooth depulpation. The research involving mathematical simulation in combination with the finite element analysis and Schleicher-Nadai theory has stated that removal of single-rooted teeth pulp caused by periodontal diseases does not increase the maximum allowable level of occlusal load in case of good bone mineralization. If it is necessary to remove the tooth pulp, one should strive either for periodontal tissue should not be substituted with fibrious tissue, or replaced with one third intra alveolar length. Biomechanical features imply a varied choice of methods and tooth filling materials used for filling root canals as well as revision of parodontogram rates in terms of periodont changes after pulp removal.

RESEARCH OF STRENGTH CHARACTERISTICS OF COMPOSITE RESTORATION MATERIAL OF RUSSIAN PRODUCTION
Abstract
A comparative study of the strength characteristics of the composite material of Russian production Restavrin and modern imported universal composites was carried out. The bending strength and elastic modulus were determined in accordance with the requirements of GOST R 31574-2012. We studied composite materials Restavrin (Technodent, Russia), Charisma Classic (Kulzer, Germany), Filtek Z250 (3M, USA/Germany), Estelite Asteria (Tokuyama, Japan), GrandioSO (VOCO, Germany). In terms of Flexural strength and elastic modulus, the Russian-made composite material Restavrin (Technodent) is not inferior (p>0.05) to imported composite materials and universal composites. Universal nanohybrid light-curing composite material of Russian production Restavrin can be recommended for the restoration of lateral teeth in the daily practice of a dentist.

ADHESION STRENGTH OF COMPOSITE MATERIALS TO DENTINE OF CERVICAL DENTAL AREA
Abstract
In laboratory conditions, the adhesion strength of light-curing composite materials to dentin of the cervical region of the teeth was determined. Sample preparation and adhesion strength testing was carried out in accordance with ISO 29022:2013 Dentistry - Adhesion - Notched edge shear bond strength test. Significant differences were found in the adhesion strength of the composite material: to mantle dentin in the area of the crown - 34.4±1.27 MPa; in the area of the neck from the vestibular surface at the level of the mantle dentin - 6.7±1.02 MPa; on horizontal sections performed at the level of the neck of the teeth - 2.3±0.94 MPa. With direct composite restoration of teeth with cervical non-carious lesions, it is necessary to apply measures improving the adhesion of the composite material, ensuring a long marginal fit and compensating for the effects of factors that are important in the occurrence of this pathology.

MODERN INSTRUMENTAL EQUIPMENT PROVIDING PLASTICIZATION AND MODELING OF DIRECT COMPOSITE RESTORATIONS
Abstract
The issues of efficiency of use in daily clinical practice of a set of six Hu-Friedy instruments for carrying out composite dental restorations are considered. A series of in vitro laboratory studies was carried out using dark and light-field microscopy and raster electron microscopy of manual simulated samples of composites by tools of various companies. It was found that on the composite layers formed by the standard tool there are deep furrows, presumably left by the rotary tool during the finishing of restoration, minor depressions are visible on a sample of the composite modeled by the Hu-Friedy tool. With increased resolution on the same samples, using a standard tool, defects in the form of furrows were found that violate the integrity of the surface structure and cohesive fractures of the composite in the pore area. Using the Hu-Friedy tool, pores up to 100 nm in size and minor indentations with even edges are determined.

COLLAGEN MATRIX IN TREATMENT OF LOCALIZED RECESSIONS OF GUM OF NON-INFLAMMATORY ETIOLOGY
Abstract
The aim of this study was to evaluate the effectiveness of the collagen matrix in combination with a coronally moved flap in the treatment of localized gum recessions of non-inflammatory genesis. The study involved 73 patients with localized gum recessions, which eliminated this pathology using a collagen matrix or subepithelial graft. The largest percentage of defect coverage in the recession area was obtained in the group (after 1 year it amounted to 93%) where we used the improved method using a collagen membrane. In the comparison group, where a subepithelial connective tissue graft was used, the percentage of coverage was 88,8% after 1 year, which also corresponds to good indicators.

COMPARATIVE CHARACTERISTICS OF VARIOUS ADHESIVE PROTOCOLS (LABORATORY STUDY)
Abstract
In laboratory conditions, the strength of the composite material adhesion to the tooth dentin was studied using an adhesive system depending on the method of adhesive preparation. The production of samples and the study of the adhesion strength were conducted in accordance with the ISO 29022:2013 Dentistry - Adhesion - Notched edge shear bond strength test. We used a 5-generation adhesive Gluma 2Bond (Kulzer). 5 variants of adhesive preparation were studied: (1) according to the instructions, (2)" alcohol adhesive Protocol", (3) medical treatment with ethyl alcohol, (4)" chlorhexidine adhesive Protocol", (5) medical treatment with chlorhexidine. A significantly higher strength of adhesion of the composite material to the tooth den-tin was established when using the 5th generation adhesive system according to the instructions (13.5±0.57 MPa) compared to the adhesive preparation in accordance with the "alcohol adhesive protocol" (5.2±0.25, p<0.00001) and "chlorhexidine adhesive protocol" (7.2±0.28 MPa, p<0.00001). The results of the study cast doubt on the information disseminated in social networks regarding the high efficiency and clinical feasibility of using the "spirit adhesive protocol" and the "chlorhexidine adhesive protocol" in practical dentistry. Comprehensive laboratory and clinical research in this area, based on the principles of evidence-based medicine, is necessary.

MARKETING ANALYSIS OF RENEWAL OF THE REGIONAL ASSORTMENT OF MEDICINES
Abstract
The analysis of renewal of the assortment of reference medicinal products that passed state registration in 2019 was carried out. The stages of creating a reference drug include not only long-term preclinical and clinical studies, but also a lengthy registration process. Russian legislation, like the legislation of most countries, provides for a simplified procedure for introducing generic medicines into circulation. The study of the range of long-term reference medicines will solve a number of problems both at the level of inventory management of pharmaceutical organizations and in the provision of medicines to the population. The purpose of this work is to analyze the renewal of the assortment of reference drugs for 2019, as well as to study the physical and economic availability of first registered reference medicinal products at the regional level. It was found that the supply of new drugs on the local pharmaceutical market is formed only by 11%, which limits the ability of retail to realize the potential demand of intermediate and final consumers.
