TACTICS OF FERTILITY IN CHRONIC FETOPLACENTAL INSUFFICIENCY

Abstract

The article contains an analysis of the course of pregnancy, childbirth, the state of newborns in 63 patients who showed significant disorders of uterine and fetal blood flow in the Doppler study. The conclusion is made on the informative value of prenatal methods of assessing the fetus condition, the optimal way of delivery in this group of pregnant women is shown.

CLINICAL-PSYCHOPATHOLOGICAL CHARACTERISTICS OF YOUNG PEOPLE OF CALLING AGE WITH AUTODESTRUCTIVE BEHAVIOR

Abstract

44 young men of conscript age with a history of autodestructive behavior (self-harm, not having the goal of depriving themselves of life) were examined by clinical-psychopathological, psychological, statistical and follow-up methods. It is established that most of them - 63.6%, reveal a deformation of the mental warehouse in the form of personality disorders. Mentally healthy make up 31.8%. The motive of self-harming actions in personality disorders was the need to remove autochthonous or psychogenically generated emotional tension, and for mentally healthy persons - the desire to influence the nearest environment or subordination to the "rules of the game". These actions were performed against the background of emotional-volitional immaturity during the period of the physiological pubertal crisis. However, in the subsequent, they critically assessed the perfect act, were well adapted in the microsocial environment, which promoted "emotional-strong- willed ripening" and allowed them to recognize them as mentally healthy and fit for military service with insignificant restrictions.

EFFICIENCY OF LOW INTENSITY LASER RADIATION IN THE TREATMENT OF OSTEOARTHRITIS

Abstract

The aim of the research is to evaluate the efficiency of low intensity laser ra-diation (LILR) in the treatment of osteoarthritis. 50 patients with osteoarthritis were enrolled in the study and allocated into two groups. The patients of group 1 underwent drug therapy and laser therapy, the patients of group 2 - routine phar-macologic therapy. The technique of laser treatment consisted in local subcutane-ous irradiation of affected joints. The irradiation was performed using the impulse laser emitting in the infrared wavelength 890 nm. The folсяlow up time was 1 year, during which 3 sessions of laser therapy were carried out. The response to laser treatment was evaluated according to the changes in the clinical findings and labo-ratory estimates. Questionnaire interrogations based on patient self assessment of their condition and their level of daily living activities and limitations were also conducted. The patients with osteoarthritis responded favorably to laser treatment. The statistical analysis showed a highly significant difference between the routine drug therapy and complex treatment including LILR. These findings give patho-genic grounds for the use of LILR in the treatment of osteoarthritis.

Abstract

The authors used the method of drug electrophoresis of 0.5% novocaine solution and 0.5% lidocaine solution in the treatment of closed chest injuries. It was established that physiotherapy methods of anesthesia in patients with thoracic trauma are acceptable at early stages of treatment and allow to achieve optimal analgesia of this category of patients.

SURGICAL TREATMENT OF PRIMARY HYPERPARATIROSIS

Abstract

We analyzed the surgical treatment of 45 patients with primary hyperparathyroidism operated from 2003 to 2005 in the endocrine surgery department of the Voronezh Regional Clinical Hospital No.1. Before the operation, all patients underwent complex examination: radiography of the bones of the skeleton, densitometry, determination of indicators of total calcium and phosphorus, amount of parathyroid hormone, neck ultrasound, CT of the neck, scintigraphy using technetium. Based on the research, it was established that despite the expansion of the arsenal of diagnostic methods at present, primary hyperparathyroidism is still not being detected in a timely manner; primary hyperparathyroidism in 69% of cases is combined with various diseases of the thyroid gland, this causes difficulties in diagnosing; timely and adequate surgical treatment of primary hyperparathyroidism in the conditions of a specialized clinic improves the quality of life of patients with this pathology.

OPTIMIZATION OF TREATMENT OF PATIENTS WITH NON-PROBABLE NUMBER-CELL LUNG CANCER (NSCLC)

Abstract

Despite the introduction of extended and combined operations, new chemotherapy and expensive schemes, targeted therapy, the results of treatment of patients with NSCLC for the past 10 years remain the same. Our comparative study of the accelerated hyperfractionation (CNART) and cisplatin technique as a radio modifier with a classical treatment regimen and the use of cisplatin as a radio modifier allowed us to obtain a higher frequency of the effect in the CNTT technique. Also, the longer time to progression of the disease in the group of patients treated by the accelerated hyperfractionation technique may be an indicator of the activity of the combination CHART scheme and cisplatin at a dose of 30 mg / m2, and the moderately severe toxicity of the test method allows it to be used in the treatment of patients with inoperable NSCLC.

MODERN APPROACH TO DIAGNOSTICS AND TREATMENT OF MALIGNANT TUMORS OF THE ADRENALS

Abstract

Twenty-six patients with malignant tumors of the adrenal glands were examined, 20 of them were operated. Long-term results of surgical treatment were studied in the period from 2 to 9 years. Patients underwent complex clinical, laboratory and instrumental examination. Adrenocortical carcinomas are more often hormonal-inactive and occur without characteristic clinical manifestations. The use of fine needle aspiration biopsy is justified as a diagnostic method. If the adrenal cancer is suspected of a malignant tumor, an "open" adrenalectomy with a tumor and a dissection of the regional lymph nodes using thoracophrenolumbutic access should be preferred, with adrenal gland suspected of open adrenalectomy using "Mercedes" access. Timely surgical treatment of patients with adrenal tumors is effective, determines the prognosis of the disease and the quality of life.

Abstract

For the first time in our region we use the method of continuous ambulatory peritoneal dialysis (CAPD) for the treatment of adult patients with end-stage renal disease. This method was used on the base of dialysis department in the Elets Hospital № 1. We have conducted comparative analysis of efficiency between CAPD and hemodialysis. The more high factors of compensations of anemia, social-psychological rehabilitation and low annual mortality allow us to use CAPD in the manner of the first method of treatment of terminal renal failure.

INTERRELATION OF EFFECTIVENESS OF ANTIAGREGANT THERAPY OF ACETYLSALICYLIC ACID AND TOLERANCE TO PHYSICAL LOAD IN PATIENTS WITH STABLE VENTRICARDIUM OF STRESS

Abstract

The aim of the study was to evaluate the efficacy of various forms of acetylsalicylic acid (ACA) on aggregation activity of platelets in patients with ischemic heart disease: stable angina pectoris with a sensitivity factor to disaggregant therapy (Ra%) and to prove the presence of correlations between biochemical and clinical resistance to ASA 58 patients with stable angina pectoris of II-III FC were observed. ADP-induced platelet aggregation and patient tolerance to exercise were evaluated against the background of administration of Thrombone ACC 50 and ASA 125 mg / day. Criteria for the efficacy of antiplatelet therapy (AAT) using a sensitivity factor for disaggregant therapy (Ra%) and an index of the degree of maximum aggregation (Amax) are proposed. Using these criteria, insufficient sensitivity was found in 34.5% and resistance in 27.6% of patients with Trombo ACC 50, as well as lack of sensitivity in 17.2% and resistance in 20.7% of patients on the background of administration of ASA 125 mg /day. The relationship between the efficacy of AAT and the tolerance of patients to physical activity has been revealed. Thus, the obtained results indicate the validity of the use of the introduced criteria for the effectiveness of AAT (Ra% and Amax) and the presence of correlations between biochemical and clinical resistance to ASA drugs.

PECULIARITIES OF CYTOKINE STATUS OF PATIENTS WITH NEST ALOPECY

Abstract

The features of the cytokine profile in 120 patients with alopecia areata (GA), aged from 8 to 54 years are considered. The obtained results suggest that in the progressing stage of GA, the cellular part of the immune system plays the leading role, whereas in the stationary stage the changes on the part of the humoral link come to the fore. It is likely that autoimmune processes triggered by the formation of autoreactive clones of cytotoxic lymphocytes leading to hair loss are further supported by immune reactions associated with the production of antibodies. This makes it possible to consider the advisability of including in the complex therapy of GA drugs that reduce the activity of pro-inflammatory cytokines.

Abstract

The aim of the study was to compare the efficacy and safety of therapy with beta - adrenoblocker atenolol in combination with the lipid - lowering drug of the statin group "Vazilip" and with combined therapy of atenolol with a herbal preparation based on the Shiitake fungus. The positive effect of atenolol and shiitake therapy on the psychophysiological indices of persons associated with the movement of trains is shown, and the ability of the therapy to provide adequate hypotensive control along with a decrease in the atherogenic potential of the blood of patients with AH is demonstrated. Combination therapy with atenolol and vasilip has an antihypertensive effect, has pronounced anti-atherogenic properties, but does not affect the psychophysiological parameters of AH patients.

Abstract

Resume: The aim of the research is to evaluate the efficiency of low intensity laser radiation (LILR) in the treatment of rheumatoid arthritis. 70 patients with rheumatoid arthritis were enrolled in the study and allocated into two groups. The patients of group 1 underwent drug therapy and laser therapy, the patients of group 2 - routine pharmacologic therapy. The technique of laser treatment consisted in local subcutaneous irradiation of affected joints. The irradiation was performed using the impulse laser emitting in the infrared wavelength 890 nm. The follow up time was 1 year, during which 3 sessions of laser therapy were carried out. LILR provided modulation in production of some types of T-lymphocytes. The results of the study demonstrate that there is a relationship between the improvements of the clinical findings and immunomodulating effects of LILR. Laser therapy of rheumatoid arthritis appears to exhibit immunomodulating and anti-inflammatory properties.

CRIMINAL AGGRESSION OF PERSONS WITH MENTAL LEAVE, RECOGNIZED BY THE COURT UNBEATABLE

Abstract

33 patients of a male with mental retardation who committed criminal aggression and were found insane by the court (7 teenagers from 16 to 18 years and 26 adults from 19 to 52 years) were examined clinico-psychopathologically, clinically-dynamically and statistically. It was found that for the majority of the study population, hereditary burden (oligophrenia or alcoholism of immediate relatives) is characteristic, as well as deviant behavior at school age. In adults with mental retardation, criminal aggression was determined by the presence of a psychopathic syndrome and, often, alcohol intoxication, and in adolescents - by an attempt at rape. High risk of criminal aggression is possessed by persons with the presence of psychopathic syndrome and alcohol intoxication, as well as previously recognized as insane and undergoing compulsory treatment. In the latter case, high tropism was found to commit criminal aggression against relatives. Significant risk factors for criminal aggression (in particular, murder) of persons with mental retardation are family alcoholism and education in the conditions of a children's home-boarding school. Recommendations are offered on complex methods of primary and secondary psycho-prophylaxis.

Проведенное исследование показывает необходимость выявления аффективных расстройств и их коррекции включением в комплексную терапию больных хроническим панкреатитом с тревожно-депрессивными расстройствами препаратов, влияющих на аффективную сферу для повышения эффективности лечения и улучшения качества жизни пациентов.

Abstract

The interrelation of infringements of parameters of the immune status at patients with genital herpes infection in a combination with clamidiosis, ureaplasmosis and trichomoniasis and groups of blood is certain. At patients with combined infection transferred sexual by, the highest level immune reactance is established at persons with the third group of blood, the least at pa-tients with the second group of blood. It is revealed, that «contestability» immune parameters on correction derinat and is registered by magneto-infrared-laser influence on decrease in following sequence: patients with group O (I) A (II) B (III).

Abstract

The results of the study of the frequency of occurrence of hyper and hypo-functional immune system in pharmacoresistant patients with schizophrenia, comorbid with pulmonary tuberculosis (n = 74), against the background of monthly use of standard psychopharmacotherapy (n = 25), combination of SPFT and cycloferon (n = 23), as well as fluanxol monotherapy (n = 26). In the latter group, D30 showed a greater number of patients with normal and elevated values of individual immunity indices.

STUDY OF THE ANTIHYPOXIC ACTIVITY OF OXINICOTIN ACID AND ITS DERIVATIVES

Abstract

The antihypoxic effect of new derivatives of oxynicotinic acid on the model of acute hemic hypoxia and in acute anoxia was studied. The experiments were performed on male rats and nonlinear white mice with a mass anaesthetized with ethanal sodium. It was found that the maximum activity close to the activity of sodium oxybutyrate is manifested by the compound under the laboratory code XC-2. In acute anoxia, oxynicotinic acid and its derivatives, with the exception of the connection under the code XC-9, do not have an antihypoxic effect and do not exert a direct activating effect on the bioenergetics of the heart muscle.

METHODOLOGICAL APPROACHES TO DIAGNOSTICS OF THE CASH CAUSED BY GASTROESOPHAGEAL REFLUX DISEASE

Abstract

Based on the data of our own research, it was found that cough is a frequent manifestation of gastroesophageal reflux disease: 87 (34.8%) of 250 patients with gastroesophageal reflux disease. Tussography combined with a 24-hour esophageal pH-meter made it possible to identify a cough connection with episodes of reflux. Patients with a cough caused by gastroesophageal reflux disease are a heterogeneous group: in 49 of 87 patients the cough was or significantly decreased due to antireflux therapy. The parameters of spectral tusophonobarography were revealed, which can serve as predictors of the insufficient effect of antireflux therapy in patients with cough caused by gastroesophageal reflux disease. In 34 of 38 patients, the insufficient effect of antireflux therapy was due to the presence of eosinophilic inflammation in the bronchi. The administration of beclomethasone dipropionate contributed to a significant reduction in cough in patients with gastroesophageal reflux disease with eosinophilic bronchitis.

Abstract

The article presents the results of the study of the structural properties of circulating erythrocytes (sorption capacity and osmotic resistance of membranes) in the syndrome of endogenous intoxication. The dependence of the studied characteristics on the degree of severity, the clinical and pathogenetic stage and the nature of the course of this pathological condition was revealed. High specificity of the studied parameters and the pathogenetic validity of this approach allows us to recommend an analysis of the structural properties of red blood cells as markers of endotoxicosis.

Abstract

The immunomodulating activity of oxynicotinic acid (XC-1) and its new acyl derivatives (XC-2, XC-3, XC-4, XC-9) was studied under conditions of acute toxic hepatopathy caused by the introduction of CCl. It was established that ONK and its derivatives dose of 1 TD) exert an immunomodulatory effect and their effectiveness increases in the sequence XC-1, XC-2 -> XC-3 ~ »XC-4 -» XC-9. The drug of comparison - mexidol in a dose of 20 mg / kg has a pronounced immunocorrecting action. The compound XC-4 (100 mg / kg) is equal to the activity of mexidol, the compound XC-9 (35 mg / kg) is superior in activity.
