IDENTIFICATION OF ENDOGENOUS INTOXICATION PARAMETERS AS CRITERION OF LATE GESTOSIS SEVERITY DEGREE

Abstract

The objective of this investigation was to identify the degree of endogenous intoxication of a pregnant organism depending on the intensity of late pregnancy gestosis; 22 expectant mothers with physiological pregnancy were examined along with 54 pregnant women with gestosis of different severity degree. Concentration of low and mean molecular weight substances and oligopeptides in plasma, on erythrocytes and in urine was used as endotoxicosis indices; along with this integral coefficient of endogenous intoxication was calculated. Pregnant women with gestosis had the content of low and mean molecular weight substances and oligopeptides in plasma and on erythrocytes and integral coefficient considerably higher, than those having normal pregnancy progress. The most marked changes were noted in severe gestosis. The data obtained prove that endogenous gestosis parameters may be used in complex assessment of gestosis severity degree.

ERRORS IN DIAGNOSIS OF MUCOVISCIDOSIS IN CHILDREN

Abstract

Pathology of early aged children is characteristic by multiple syndromes, involvement of respiratory system and gastrointestinal tract; dystrophies indicate the necessity of laboratory examination for mucoviscidosis (sweat chlorides, DNA-diagnosis). Pathological and anatomic examination of children, who died from bronchial and pulmonary pathology or/and gastrointestinal syndrome serves the basis for the performance of histological analysis of pancreatic tissues with the aim to search for cystic and fibrous changes.

EPIDEMIOLOGY OF MILD BRONCHIAL ASTHMA IN CHILDREN AGED 1 TO 7 YEARS OLD

Abstract

The data obtained allow the assertion that children develop bronchial asthma at very early age. The first symptoms appear at the first year of life, obtaining maximum incidence rate at the age of 1 to 2 years old, while the diagnose is set 2, 3 years and sometimes 4 years later. Such children require dynamic monitoring and mandatory prescription of anti-inflammatory therapy due to the risk of moderate severity bronchial asthma development. If the systemic examination is done, educational programs are widely used, the state of children is monitored and the adequate treatment is administered, the risk of bronchial asthma development may be considerably reduced, the progression of the disease into moderate and severe degree - avoided and labor inputs and economic losses associated with the disease - decreased.

Abstract

The examination results have been reported on the incidence rate of purulent meningitis in infants on the territory of Voronezh city according to the data of Regional Pediatric Clinical Hospital No2 for the period of 2000-2003. Etiological factor was compared along with clinical and neurological manifestations and immune status was examined in infants up to 1 year old with purulent meningitis in its acute period. Immunodeficiency condition was identified both in cellular and humoral components, which should be taken into account while administering immunomodulatory therapy.

LIFE QUALITY ASSESSMENT IN CHILDREN WITH CHRONIC URINARY DISEASES

Abstract

The Nephrology Unit of Regional Pediatric Clinical Hospital served the basis for life-quality examination in children with chronic urinary diseases, using the adapted questionnaire SF-36. The examination has shown that chronically sick adolescents more often evaluate their health condition as bad compared to their healthy fellows. Teenagers suffering from urinary diseases have considerably higher life-quality parameters, compared to healthy youth, such as pain syndrome intensity (BP), absence of vital energy (VT), limitation in social life (SF), influence of emotional perception on physical activity (RE) and psychological ill-being (MH). The average values of social life limitation level (SF) turned the most different in healthy and sick adolescents, thus one of the major factors of this state was, most likely, the limitation in social life involvement of such teenagers.

INCIDENCE RATE AND STRUCTURE OF I TYPE DIABETES IN CHILDREN OF VORONEZH REGION

Abstract

Aimed at analysis of epidemiological situation in I type diabetes in children of Voronezh Region the group of children with ВВСД, aged 14 years old, (223 children) were examined in the period from 1998 to 2002. The results have shown that morbidity rate of I type diabetes increased 1.3 as much by 2002 and made up 14.4 cases per 100 thousand of children’s population. Sickness rate remained the same both in the city and in rural areas from 2000 to 2002, in 1998 and 1999 the morbidity rate in the city was prevailing. One third of patients had respiratory viral infection as an etiological factor. The period between the occurrence of initial diabetic symptoms to its diagnosis has reduced by 5 days for the 5 years, though the number of children admitted to the hospital in ketotic condition and even diabetic coma still remain high.

FEATURES OF PRESENT-DAY STOMACH ULCER COURSE IN CHILDREN

Abstract

The case histories were analyzed of children with the diagnosis either stomach ulcer or chronic erosive gastroduodenitis, treated in Regional Clinical Pediatric Hospital during 2002. It was identified that in modern conditions the rate of duodenum bulb deformation development does not depend on the disease duration, which speaks for the importance of early diagnosis, timely treatment and clinical examination of children and adolescents at the very onset of the disease; differential diagnosis of stomach ulcer and chronic erosive gastroduodenitis is possible only when clinical and anamnestic data are compared to endoscopic examination results. Essential clinical symptoms, indicating that there is high probability of gastrointestinal hemorrhage in acute condition of stomach ulcer, are the presence of positive Mendel’s symptom and the absence of heartburn; the probability rate of gastro-duodenal reflex is increasing with the disease course duration; the efficiency of treatment with tetracycline, semisynthetic penicillin and macrolideis is identical.

HEALTH STATE ASSESSMENT OF BABIES WITH NEONATAL HYPERTHYROITROPINEMIA AT THE FIRST MONTH OF THEIR LIVES

Abstract

The influence of neonatal hyperthyroitropinemia has been studied on the postnatal period and infant health state assessment. The level of thyrotropic hormone in capillary blood was identified in all newborns. 114 newborns with neonatal hyperthyroitropinemia were examined (thyrotropic hormone - 10,71±3,52 micro units/liter) and 88 infants with thyrotropic hormone level up to 5 micro units/liter (thyrotropic hormone -3,04±0,9 micro units/liter) - control group, р<0,05. The examination results have shown that most newborns with neonatal hyperthyroitropinemia had different health disorders during the first month of their lives. The pathology was identified in 85.9% of babies of the main group, in the control group this index was considerably lower, (p<0.01) and made up less than 64.8%. Babies with neonatal hyperthyroitropinemia were more often born in condition of asphyxia and had disorders of postnatal adaptation. Babies with neonatal hyperthyroitropinemia develop considerable health disorders, among which perinatal disturbance of CNS is registered most often, conjugated jaundice with prolonged course, anemia and infectious diseases, specific for the perinatal period.

NEW DATA ON MORPHOGENESIS OF UTERINE FIBROMYOMA NODES

Abstract

Examination of three-dimensional focal arrangement of uterine myoma formation showed that two-dimensional conception of myoma “micro-nodule” and “macro-nodule” was quite conventional. Structural unit of uterine myoma is apparently the “growth focus” - the group of microscopic “sun-nodules” simultaneously originate from the “growing point” located in the center of the group. New growth of the bundle of muscular cells follows complicated toroidal curves. Uterine “growth focuses” originate in “waves” - very often by several pieces at once. “Growth focuses” derived simultaneously are changing synchronously. Formation of capsules and vessels indicate the completion of the active fibromyoma growth. As the result of the fusion of several “growth focuses” some large uterine fibromyoma nodules may arise very fast. Nodes, arising as a result of single “micro-nodule” increase, apparently grow slowly. Further work is required to study the features of transition from latent to clinically marked uterine fibromyoma.

Abstract

The experiment, done on white male rats, exposed to gamma radiation and ethanol, with the use of general histological, neuro-histological and histo-chemical methods, showed up the complex of standard non-specific morpho-functional neurocyte changes; their character and intensity degree depended on effecting factors and the time passing from the exposure termination. It was found that the factors under examination effected neurocytes causing multiple changes in the activity of aerobic tracts of bioenergetic metabolism and alcohol catabolism along with the changes of functional state of hematoencephalic barrier.

Abstract

Arterial hypertension, ischemic heart disease, diabetes mellitus - the most spread chronic noninflammatory diseases - are the main causes of mortality among the population, arising great interest of investigators to epidemiology, risk-factors and approaches to treatment. One of the early predictors of cardio-vascular complications of arterial hypertension is microalbuminuria. In this work the attempt was made to trace the correlation ties between this index and metabolic, rheological and instrumental parameters in patients with arterial hypertension, ischemic heart disease and II type diabetes. The dynamics of microalbuminuria level was studied also in the course of long-term hypotensive therapy along with the relation between this index and severity of cardio-vascular complications.

Application of video endoscopic techniques during outpatient reception hours in dental surgery unit facilitated greatly the differential diagnosis of paranasal sinus disease, reduced the number of patients appointed inpatient treatment, enabled to decrease traumatism and to raise the rate of patient rehabilitation. Clinically proved combination of conservative treatment methods with endoscopic diagnosis and sparing surgery resulted in stable recovery in the shortest time and in reduction of disability period in patients.

FEATURES OF DISSEMINATED SCLEROSIS DEVELOPMENT IN EARLY AGE

Abstract

The tendency has been observed lately to the extension of the age limits within which disseminated sclerosis occurs. The disease is more often diagnosed both at the age up to 10 and after 59 years old. The authors have analyzed the disease course in 13 patients getting sick at early age (before 20 years old). It was found that women were prevailing among the diseased, the onset was monosymptomatic, started with retrobulbar neuritis, sensitive, somewhat more rarely trunk damage. During the first years apparently remittent course was marked with short-term, not severe exacerbation and nearly complete remission. Low and moderate progression rate was noted, which was followed later on by secondary progressive forms and slow accumulation of neurological deficit.

EFFICIENCY ASSESSMENT OF METHOD APPLICATION OF COMBINED POSTOPERATIVE PERITONEAL CAVITY SANATION IN COMPLEX TREATMENT OF GENERAL PERITONITIS

Abstract

Inclusion of combined postoperative peritoneal cavity sanation into the complex treatment program for general peritonitis increases peritoneal sanation efficiency in postoperative period and thus enables to arrest timely the syndrome of endogenous intoxication, to reduce the number of postoperative complications and to eliminate enteroparesis in the shortest possible time. Inclusion of combined postoperative peritoneal sanation method in the treatment of the main group led to the reduction of mortality rate from 25% down to 17.8%. The data obtained allows that combined postoperative peritoneal sanation method is recommended for inclusion into the complex treatment program for acute peritonitis general forms.

Abstract

The application of SE-36 and MiGAN methods for life quality evaluation has proved the possibility of reliable quantity assessment of the condition of patients after excision of hernias of intervertebral disks. Not touching the causes of postdiskectomy syndrome origin, life quality was considerably lower in patients, according to the monitoring data, who had surgery without application of microsurgical techniques, particularly, in female patients. Our experience of microsurgical excision of hernias of lumbar vertebra intervertebral disks enables to conclude that this treatment method is highly efficient and to recommend it for wider application in surgical treatment of the pathology discussed above.

Generalizing the results of experimental research conducted, it is worth noting that all the working media of the studied synthetic surface-active substances recommended by the All-Union Research Fire Extinguishing Institute of the USSR Ministry of Internal Affairs may be used in practice by the officers of fire fighting services, still if the mentioned concentrations of the working media are not observed or exceeded in conditions of long-term contact with them, the studied foam solutions may cause the marked changes of major vital functions of an organism. They may pollute the atmospheric air in very vast area, drinking water, soil and plants growing on it.

CORONARY HEART DISEASE IN PATIENTS WITH PRE-DIALYSIS RENAL FAILURE AND ARTERIAL HYPERTENSION

Abstract

The authors have defined CHD incidence rate in patients with pre-dialysis renal failure, assessed the similarity of pathogenetic chains and possibilities of mutual potentiation. The arterial wall elasticity shifts were identified, depending on severity of endothelium dysfunction, which had a systematic character. Serious disorders of carbohydrate and lipid metabolism served as supplementary factors, taking into account that many patients had either diabetes mellitus or nephrotic syndrome. Such medications as converting enzyme inhibitors, nitrates and calcium channel blockers or beta blockers are required to be added to the standard therapy administered to patients with pre- dialysis chronic renal failure and CHD.

Abstract

460 patients with chronic obstructive pulmonary diseases of different severity degree underwent examination. basing on the mathematical analysis of examination results clinical hemodynamic groups were selected, reflecting the stages of pathological process development and condition severity in patients; algorithms were developed as well to evaluate statistical tendencies of parametric changes in the course of drug therapy. The emphasis was made on the study of relationships between hemodynamic disorders, lipid peroxidation, bioenergetic processes, heart rate variability and hormonal disorders during aggregation of chronic obstructive pulmonary disease. It was found that severity increase of chronic obstructive pulmonary disease led to the intensity growth of free radical processes against the background of progressive antioxidant system insufficiency; activation of intracellular glycolytic processes in erythrocytes, indicating the growing bioenergetic insufficiency, disturbance of vegetative regulation of heart rate with sympathetic influence prevailing. The algorithm was proved for antioxidant treatment combined with calcium channel blockers at different stages of chronic cor pulmonale development.

PATHOMORPHOSIS OF TUBERCULAR PLEURISY

Abstract

The changes have been analyzed happening in the 20-year period (1979 - 1998) in social structure of adult patients with tubercular exudative pleurisy and in clinical and morphological signs of the disease. It was identified that the number of non-working people capable to work increased threefold, (from 7.1% to 23.3%), sick pensioners, disabled people and migrants grew in number; the rate of alcohol addiction and concomitant illness increased. Subacute, symptom-free and asymptomatic onset of the disease was marked more often. The detection rate of tubercular mycobacterium in sputum increased in independent forms of tubercular exudative pleurisy from 3.0% to 11.0% of cases, in tubercular exudative pleurisy combined with active tuberculosis - from 30.3% to 45.1% of cases. The rate of drug resistance of tubercular mycobacterium detected in sputum grew from 17.0% to 31.9% of cases. Stereo morphometric examinations of pleural biopsy materials showed that in the last years the specific number of granulomas with caseous necrosis on the plaura section area increased.

FEATURES OF AFFECTIVE DISORDERS IN WOMEN WITH CLIMACTERIC SYNDROME DEPENDING ON THE STATE OF REPRODUCTIVE SYSTEM

Abstract

The results have been reported on the study of the structure and phenomenology of affective disorders in women being in climacteric period depending on the state of their system of reproductive organs and occurrence or absence of surgery. 100 women aged 45 to 50 years old were examined during peri and post menopause using the questionnaires with scores: Spilberger-Hanin, the questionnaire of activity state and mood (САН), Beck's tests to identify depression, anxiety and depression scales (HADS), clinical questionnaires to define somatic disorders. Two groups were selected: I group (n=30) - without gynecological pathology, II group (n=70) - having gynecological pathology. Significantly marked manifestations of depression and anxiety were found in the group burdened with gynecological pathology. The group of operated women turned to have significantly higher indices of reactive and personal anxiety and depression. It was concluded that emotional and affective disorders prevailed in the form of anxious and depressive syndrome in the groups of women having gynecological diseases and being operated.

APPLICATION ADVANTAGES OF NEW DOMESTIC COMBINED BIOLOGICAL PREPARATION BIFILACTRIN, CONTAINING SPECIMENS OF OBLIGATE INTESTINAL MICROFLORA

Abstract

93 children under 3 years old with acute diarrhea were kept under medical observation. The patients had complex checkup, which included examination of immune status and parameters of gastrointestinal tract functioning (coprocytogram, number of lacto- and bifidobacteria, content of fecal carbohydrates and fecal pH). New domestic medicine Bifilactrin was administered to treat acute diarrheal diseases. Comparative assessment of different therapeutic methods was made using new probiotic Bofodumbacterin and antibacterial drugs. The administration of double-culture probiotic was proved to have clinical and laboratory efficiency in the treatment of acute intestinal infections of light and moderate severity degree.
