The comparative analysis was applied to the information on adult population health as regards to the ecological situation in the Voronezh districts and in Voronezh region. Chronic pharyngitis, chronic glandular diseases, bronchial asthma, thyrotoxicosis and tumors are registered considerably more often in the districts characteristic for highly polluted air. In the regions using intensely pesticides or being contaminated with radioactive substances women more often have tumors of genital organs, endometriosis, menstrual cycle disorders, pregnancy pathology and congenital anomaly of fetus development.

Significant relations have been identified between excessive body weight and alexithymia, which is explained by hyperphage response to stress in these patients. The group of patients with alexithymia is characterized by increased anxiety and depression level and low life-quality index, which requires the addition of psycho-pharmacological correction and rational psychotherapy methods to the treatment scheme.

The results of hospital treatment of 897 patients with chemical esophageal burns of different severity degree have been analyzed. The comparative assessment was applied to methods of esophageal cicatrical stenosis prevention. It was proved that early local application of ozonized vegetable oils in combination with mechanical dilatation of an organ with elastic gastric tube had high clinical and economic efficiency at sever esophageal burns.

COMPARATIVE CHARACTERISTICS OF MODERN METHODS OF HYPERBILIRUBINEMIA TREATMENT IN NEWBORNS

Abstract

156 mature newborns with hyperbilirubinemia in early neonatal period and bilirubin level in blood serum from 207.8 to 370 micromole/liter were examined. All the patients were divided into three groups: I - 55 babies, administered traditional therapeutic complex, II - 53 babies, having additional GBO courses included into the treatment complex, III - 48 babies, treated with the application of such medications as “Toxfighter” and “Loxein”, manufactured by the Russian Company “Artlife”. The inclusion of GBO into the treatment complex for conjugated hyperbilirubinemia of newborns resulted in the greatest reduction of billirubin level. Application of “Toxfighter” and “Loxein” in the treatment of newborns with hyperbilirubinemia seems purposeful causing more intensive reduction of billirubin level compared to traditional treatment complex. Management of GBO courses, inclusion of “Toxfighter” and “Loxein” into the treatment complex for hyperbilirubinemia in newborns enabled to reduce the number of babies sent from Maternity Hospital to Specialized Units of Regional Pediatric Hospital for further treatment.

EXPERIENCE OF HEMOLASER THERAPY APPLICATION IN TREATMENT OF BRONCHIAL ASTHMA WITH CONCOMITANT RHINOSINUSITIS

Abstract

88 patients having mean severity of bronchial asthma with concomitant rhinosinusitis were kept under medical observation; they were divided into three groups. The first one included 36 patients, whose tympanic membranes and projections of paranasal sinus were treated with laser. The second group, consisting of 28 patients, was administered blood laser irradiation, and at last, the third group under examination, comprising 24 people, received medical treatment. The greatest clinical efficiency of treatment was observed in the first group of patients, which was seen in the evident positive dynamics of clinical, instrumental and spirometric parameters as regards to bronchial asthma as well as in full regression of symptoms of concomitant rhinosinusitis, which was proved by radiographic data. The treatment turned less efficient in the group of patients administered medical therapy.

Abstract

Oesterogen deficiency - menopause, the state occurring after hysterectomy and ovariectomy, is a serious risk factor for the development of hypertension, obesity, insulin resistance, dyslipemia, and, finally, atherosclerosis and CHD, which is apparently conditioned by the loss of specific protective mechanisms of oesterogen action on cardio-vascular system. Hormone replacement therapy, in its turn, decreases the intensity degree of climacteric complex of symptoms, improves considerably the life quality of female patients at the very first stages of its application, enables to avoid critical hypertension course, and, in most cases, to normalize the values of systolic and diastolic arterial pressure, and does not worsen the indices of lipid and carbohydrate metabolism.

PULMONARY EMBOLISM IN PRACTICE OF MULTI FIELD HOSPITAL

Abstract

The practice of multi field hospital has shown that pulmonary embolism is obviously predominant in the Emergency Cardiology Unit. Traditional diagnostic methods, particularly, ECG with supplementary leads (chest right leads), still remain of great diagnostic value in pulmonary embolism life-time diagnosis. Dynamic electrocardiographic monitoring is still considered of extreme diagnostic importance. The accuracy improvement of pulmonary embolism diagnosis is obtained with the use of non-invasive monitoring of central homodynamic parameters. Application of low-molecular heparins allows the reduction of pulmonary embolism rate in patients with high risk-factors.

ROLE OF CORRECTION OF PSYCHOLOGICAL DISORDERS IN THE COURSE OF ANGINA PECTORIS IN AGED PATIENTS

Abstract

Prevention of coronary heart disease (CHD) and influence on the risk factors causing this disease have been found the most challenging in cardiology, where preventive approach has been developed dramatically in the last decade. Still traditional risk factors do not allow the explanation of all the cases of CHD onset and development in population. In this connection some Russian and foreign authors have concluded that psychological, psycho-social and behavioral CHD risk factors should be specified. 147 patients over 55 years old with stable angina pectoris (ФК II-III) were involved into investigation. Index above 74 scores according to Toronto alexithymia scale served the criterion for alexithymia identification and for grouping the patients. The patients were divided into three groups. The first group included the patients with alexithymia, who along with the basic treatment of stable angina pectoris were administered a two-week course of melatonin, in a daily dose of 3 mg taken before night sleep; the second group - alexithymia patients, who did not receive melatonin; the third group - patients without alexithymia, prescribed melatonin in addition to the basic therapy. After two weeks of combined treatment with melatonin statistically significant improvement have been marked of all the assessed criteria of psychological state and most criteria of objective status.

MEDICAL REHABILITATION OF PATIENTS WITH TRANSMURAL MYOCARDIAL INFARCTION HAVING THE THREAT OF CARDIAC MUSCLE RUPTURE

Abstract

The examination results were analyzed of 154 patients with transmural myocardial infarction, who, when admitted to the hospital, had the menace of cardiac muscle fracture, the ECG sign of which was S+ scar in M-complex. They were kept under medical examination during one-year period after discharge from the hospital. Among these patients 86 people (the first group) were treated with angiotensin-converting enzyme inhibitors and beta-blockers against the background of the main therapy, and 68 people (the second group) were administered the main therapy. The first group was found to have more smooth post infarction course, which was proved by the fast improvement of clinical parameters, reduction of complication number and mortality rate decrease.

DAILY HEART RATE VARIABILITY IN HEALTHY PEOPLE AND IN CHD PATIENTS WITH CORONARY ARTERY DISEASE

Abstract

The objective of the investigation was to identify what features of daily heart rate variability (HRV) reflected more precisely the functional state of an individual patient. Daily HRV was assessed in healthy people, in patients with coronary pathology and patients with coronary artery disease, having surgery of coronary artery bypass grafting. Daily rhythmograms, obtained during Holter monitoring with the use of DMS-system, were examined. Traditional parameters of daily HRV were analyzed, such as SDNN, SDANN and NN50. Apart from this during HRV analysis the new approach was applied, basing on the evaluation of variations of short rhythmogram areas.

Abstract

Life quality dynamics has been studied in patients with rheumatic heart diseases, initially having different life quality level. 47 patients with rheumatic heart diseases underwent examination. Circulation failure I-II, ФК I - Ш. Life quality was evaluated with the use of three questionnaires (Nottingham’s health profile, Minnesota’s questionnaire and questionnaire of heart disease symptom intensity): initially, then after 1, 3 and 6 months after complex medical therapy and surgical correction of a defect. It was identified that patients initially having deeply marked symptoms and heart disease manifestations showed higher life quality level after successful surgery than patients with less severe heart disease manifestations.
