DAPSON APPLICATION IN COMPLEX TREATMENT OF PATIENTS WITH UROLITHIASIS AND CONCOMITANT CHRONIC PYELONEPHRITIS

Abstract

32 patients with urolithiasis complicated with chronic pyelonephritis have been examined while being in the active treatment phase. Prior to the treatment all the patients were found to have immune disorders of different severity degree, particularly, in the cellular components. 16 patients undergoing traditional antibacterial therapy in postoperative period still showed the marked immune suppression. 16 patients who additionally received immunomodulator Dapson in postoperative period were noted to have abnormal components of immune systems stimulated, which enabled to achieve better treatment results in this group of patients.

SOME STANDARDIZATION ASPECTS OF HEALTH CARE DELIVERY TO PATIENTS WITH UROLITHIASIS

Abstract

Endemic diseases cause considerable growth of regional health care expenses. In Voronezh Region such a disease is urolithiasis. Taking into account some recommendations elaborated at scientific and practical conferences considering the issues of health care standardization (Moscow, 1999-2001), orders No 12/2 of 19.01.98 and No 23/3 of 24.01.02 of the RF Ministry of Health and the RF Federal Mandatory Medical Insurance Fund it is possible to conclude that optimal management of health care delivered to patients with urolithiasis in Voronezh Region and its economic provision may be obtained only basing on standardization of diagnostic and treatment approaches. Non-observance of examination principles leads to inefficient treatment and its cost increase. The time necessitates obtaining maximum objective approach to method selection of urolithiasis treatment basing on examination standards and development of well-defined criteria for selection of patients. To solve this problem there should be a Report of urolithiasis patients’ management with the records of required volume and sequence of diagnostic and treatment measures depending on clinical condition. Standards of health care management have been developed for the welfare of patients and to be counted in the future. If this condition is met they will correspond to the modern level of demonstrative medicine, will contribute to efficiency increase everywhere, to improvement of urolithiasis patients’ treatment in Voronezh Region, to efficient usage of budgetary funds in districts and to introduction of advanced medical technologies at the initial stages of urolithiasis diagnosis and treatment.

TREATMENT OF ARTERIAL HYPERTENSION IN CHILDREN WITH GLOMERULONEPHRITIS

Abstract

The data have been reported on the efficiency of angiotensin-converting enzyme inhibitors and calcium channel blockers during treatment of acute glomerulonephritis hypertension in children. Calcium channel blockers and their combination with angiotensin-converting enzyme inhibitors were shown to have maximum efficiency. It was found that angiotensin-converting enzyme inhibitors, administered for proteinuria treatment to children with normal arterial pressure and having chronic glomerulonephritis, do not decrease the pressure.

EXAMINATION OF SEMAX ACTION ON INDICES OF SENSORY AND PERCEPTIVE AREAS IN CHILDREN SUFFERING FROM INFANTILE CEREBRAL PALSY

Abstract

The examination results have been reported on the change of indices of sensory and perceptive areas in children suffering from infantile cerebral palsy of different clinical forms (n=85) depending on the therapy administered. There were three groups selected: I (n=40) - standard therapy + semax, II (n=25) - single drug therapy with semax only , III (n=20) - standard therapy. The comparative results have been shown of five therapy courses (К1-К5), which proved that there was general positive therapeutic effect and that semax administered along with the standard therapeutic complex had some advantages compared to monotherapy with semax only and standard therapy only.

Abstract

In the experiment with laboratory animals (dogs and rats) the role of hyperbaric oxygen was studied in the restoration of functional and metabolic disorders, taking place at local and general hypoxia of an organism: restoration of myocardial contractive capacity, electrolytic metabolism in cardiac muscle and brain tissues, oxygen homeostasis and centrogenous regulation mechanisms. Sano-genetic features of hyperbaric oxygen, providing for universal adaptation reactions in pathological conditions, allow the assumption that they serve as a correction factor for metabolic, regulatory and executive systems, capable to maintain optimal homeostasis under terminal conditions.

Abstract

Iodination of colloid amino acids, in the first hand, and alkaline and acid phosphatase, in the second hand, have the major diagnostic value for the examined criteria at chronic exposure to electromagnetic field. At that the age-related dynamics has been observed of successive replacement of criterion priority. The density of induced currents and pulse frequency are of considerable but still less importance. Besides, identification of diagnostically considerable criteria enables to define indirectly the most sensitive chain in hormonopoiesis at the exposure to chronic pulse electromagnetic field.

Abstract

Neurohistomorphological features of cerebellar cortex cells exposed to low frequency pulse electromagnetic field (frequency - 50 Hertz, ultra-short duration pulses - 15-40 nanosec. applied during 5,7 and 10 months in different pulse modes) have been reported. The investigations conducted have proved that long-time exposure to pulse electromagnetic field leads to the complex of neurocyte changes of different type, having the phase character and certain biotropy of its parameters.

CLINICAL EFFICIENCY OF IMMUNOTHERAPY APPLIED AT EARLY POSTPSYCHIC PERIOD OF ALCOHOLISM

Abstract

The objective: assessment of clinical efficiency of complex therapy applied to patients addicted to alcohol with the use of immunomodulator ridostin. 73 male patients, aged 26-53 yeas old, diagnosed “Alcohol addiction, average (second) degree. Abstinent condition (withdrawal syndrome) with delirium” underwent medical examination. Patients were divided into the following groups depending on the therapy applied: the 1st (n=38) - traditional psychopharmacotherapy; 2nd (n=35) - psychopharmacotherapy+Ridostin. The control group included 28 practically healthy donors, comparable in age and sex. Complex psychopharmacotherapeutic scheme comprised ridostin in the dose of 8 mg. Clinico-psychopathological method was applied during the work; the examination was conducted with the use of standard clinical score having four-scale systems, developed by I.V. Bokiy and I.P Lapin (1976), modified by V.N. Lakusta (1982); its results underwent frequency analysis. Comparative analysis of the examination results has shown that application of immunomodulator leads to the reduction of clinical symptoms on average by 7.5%, at that somatovegetative features turned more sensitive to the complex treatment, thus proving great advantages of the combined treatment method.

ROLE OF MENTAL DISORDERS CORRECTION IN THE COURSE OF ANGINA PECTORIS IN AGED PATIENTS

Abstract

Work objective: searching and identification of psychological, psycho-social and behavioral risk factors of coronary heart disease (CHD). 147 patients above 55 years old with stable angina pectoris (functional class II-III) were involved into the examination. The criterion of alexithymia identification and grouping of patients was the index above 75 scores according to Toronto alexithymia scale. The patients were divided into three groups. The first group included the patients with alexithymia, who along with the basic treatment of stable angina pectoris were administered a two-week course of melatonin, in a daily dose of 3 mg taken before night sleep; the second group - alexithymia patients, who did not receive melatonin; the third group - patients without alexithymia, prescribed melatonin in addition to the basic therapy. After two weeks of combined treatment with melatonin statistically significant improvement have been marked of all the assessed criteria of psychological state and most criteria of objective status

Conclusions. Intensive therapy applied to patients with severe craniocerebral injury in conditions of computer-based monitoring of major parameters of systemic hemodynamics enables to normalize efficiently general oxygen transport and its consumption by tissues, to eliminate apparent hypermetabolism, specific for the acute period of craniocerebral injury.

Abstract

Verapamil was found to affect slightly the head minute blood flow, minute cardiac output and head blood flow index. Still the total action of Verapamil, applied in combination with laser therapy, on hemodynamics reveals considerable antiarrhythmic effect, particularly, in patients obtaining optimum of those parameters. The factors, weakening Verapamil antiarrhythmic effect, include its negative action on venous blood outflow from the head.
