INFLUENCE OF ANTIBIOTIC RESISTANCE ON THE STRUCTURE ECONOMIC COSTS OF COVID-19 PHARMACOTHERAPY
Abstract
The COVID-19 pandemic has proved to be a serious test for public health, including from an economic point of view. Conflicting data on the treatment options for COVID-19 infection have been reflected in ever-changing pharmacotherapy recommendations. One of the problems was the uncontrolled use of medicines by the population for the treatment of a new coronavirus infection. Often, the unjustified use of antibacterial agents has led to an escalation of the problem of antibiotic resistance of microorganisms. In this work, a pharmacoeconomic assessment of the costs of purchasing medicines for the treatment of patients with a new coronavirus infection in the "Clinical Hospital" RZD-Medicine "Voronezh" for 2021 and determining the share of costs for the purchase of antimicrobial agents was carried out. In addition, we analyzed the spectrum of pathogens isolated from patients of the covid hospital and assessed their sensitivity to the main groups of antibacterial agents.

DOSAGE FORMS BASED ON THE MEDICINAL PLANT MOMORDICA CHARANTIA L
Abstract
Pharmacology, clinical efficacy, side effects, drug interactions and the place of bitter melon in therapy are described. Bitter melon (Momordica charantia) is an alternative therapy that has mainly been used to lower blood glucose levels in patients with diabetes mellitus. The components of bitter melon extract seem to have a structural similarity to animal insulin. Antiviral and antitumor activity has also been reported in vitro. Four clinical trials have shown that bitter melon juice, fruit and dry powder have a moderate hypoglycemic effect. However, these studies were small and were not randomized or double-blind. Side effects of bitter melon have been reported, including hypoglycemic coma and seizures in children, decreased fertility in mice, favism-like syndrome, increased levels of gamma-glutamyltransferase and alkaline phosphatase in animals, and headaches. Bitter melon can have an additive effect when taken with other hypoglycemic agents. To properly assess safety and efficacy, randomized placebo-controlled trials with adequate nutrition are needed before bitter melon can be regularly recommended. Bitter melon can have a hypoglycemic effect, but there is not enough data to recommend its use in the absence of careful observation

ASSESSMENT OF THE STRUCTURE AND SAFETY OF DRUG THERAPY FOR NEW CORONAVIRUS INFECTION IN HOSPITAL CONDITIONS
Abstract
The paper assessed the structure of pharmacotherapy agents used in the treatment of a new coronavirus infection COVID-19 in a hospital, as well as the safety of ongoing drug treatment. The groups of drugs most frequently prescribed in patients with a new coronavirus infection have been identified. The most characteristic changes in the data of laboratory and instrumental studies during the treatment of patients in the covid hospital, as well as in the post-covid period, were noted. The most frequently observed occurrence of anemia, thrombocytopenia, hyperglycemia, hyperlipidemia, prolongation of APTT. These changes persisted at the stage of treatment in the post-COVID period. During the period of treatment for a new coronavirus infection, most of the studied patients experienced abnormalities according to ECG data and persisted in the post-COVID period for at least 3 weeks after discharge from the hospital.

EXPERIMENTAL MODELS OF DIABETES MELLITUS (LITERARY REVIEW)
Abstract
Despite the long existence of diabetes mellitus and its high prevalence among the population, which has acquired the character of a pandemic, there are still many unexplored questions regarding both the pathophysiology of the development of the disease and methods of prevention and treatment [5,8,15,17,18]. Meanwhile, diabetes mellitus leads to numerous negative consequences, reducing the quality of life, in some cases leading to disability of the population, economic losses in health care [4,5,6,19,20]. All this necessitates an in-depth study of the pathogenesis of diabetes mellitus, means of its prevention and complex treatment, and leads to the creation of experimental models [6,15,18]. Currently, there are a large number of experimental models that allow the study of type 1 and type 2 diabetes mellitus, the means of their therapy: surgical, genetic, chemical, endocrine, immune models. However, all of them have their own advantages and disadvantages, which must be taken into account in an in vivo experiment [8,13,14,15].

PREDICTION OF THE RISK OF THE DEVELOPMENT OF REPEATED UNSTABLE ANGINA: THE ROLE OF MARKERS OF ENDOTHELIAL DYSFUNCTION AND OXIDATIVE STRESS
Abstract
Ischemic heart disease is a common pathology leading to high mortality rates worldwide. One of the manifestations of this condition is unstable angina, which is often repeated. As a result, it is necessary to develop new approaches for assessing the functional state of patients with coronary heart disease. One of the promising methods for timely diagnosis is to identify a direct relationship between such blood parameters as endothelin-1, ADMA and ADPGn and the likelihood of developing unstable angina pectoris.

COMPARISON OF DYNAMIC INDICATORS OF OXIDATIVE STRESS IN PATIENTS WITH MYOCARDIAL INFARCTION WITH DIABETES MELLITUS
Abstract
Cardiovascular disease is a major health problem worldwide due to its high prevalence and mortality. In their structure, the largest percentage is coronary heart disease (CHD), the main cause of which is atherosclerosis. One of the manifestations of this condition is myocardial infarction. Another no less formidable pathology is diabetes mellitus. This disease is polyetiological and is characterized by either insulin resistance or a reduced amount of insulin, or both. For this reason, it is necessary to develop new approaches to assessing the functional state of comorbid patients with myocardial infarction and diabetes mellitus. Thus, the aim of the study was to identify a direct relationship between such blood parameters as ADPGn, KDPGn, ADPGo, and the likelihood of developing oxidative stress in patients with diabetes mellitus and myocardial infarction.

The influence of complex therapy on clinical and functional parameters and laboratory markers of patients with coexisting pathology of musculoskeletal system
Abstract
Objective: to evaluate the efficacy of the use of diacerein in the complex therapy of secondary osteoarthritis in patients with rheumatoid arthritis. Materials and methods. 248 patients with secondary osteoarthritis (OA) in rheumatoid arthritis were enrolled in the trial (87,5% females and 12,5% males), with the mean age 47,04±9,72 years, activity on DAS 28 I and II . Patients were divided in 4 groups: group I (n=62) patients of secondary OA in RA on the basis of methotrexate (MT) took diacerein in complex with laser therapy (LT), patients of group II (n=60) - МТ +D, in group III (n=62) on the basis of МТ the course of LT and in group IV (n=64) - MT. Healthy were included in control group (n=40). The efficacy of treatment we estimated in 3 and 6 months with the use of indexes WOMAC, Lequesne, DAS 28, markers COMP and IL-1. Results and discussion. After 6 months of complex therapy, the advantage of using diacerein in relation to the dynamics of the functional indices WOMAC and Lequesne was noted. Against the background of taking MT (group IV), a decrease in inflammation markers was also noted. However, the dynamics of values is significantly less. A comparative analysis showed a statistically significant advantage (p<0.01) of the use of diacerein in combination with MT in relation to the dynamics of IL-1 and COMP in the blood serum of patients.

MODERN ASPECTS OF INFECTIOUS DIARRHEAS OF VIRAL ETIOLOGY IN ADULTS: A CLINICIST'S VIEW
Abstract
Most of the publications on the problem of viral diarrhea are devoted to morbidity in children. However, acute intestinal infections of viral etiology in adults are becoming increasingly important. In the Voronezh region, there is an increase in the proportion of viruses as the etiological cause of acute infectious gastroenteritis. This is due to the introduction of centralized research using molecular biological methods for the detection of pathogens. PCR made it possible to simultaneously determine the presence of bacteria and viruses in the test material - the causative agents of acute intestinal infections (multiplex format). In the etiological structure of viral diarrhea in children and adults, rotaviruses dominate. With the expansion of the range of laboratory tests and clinical observations performed, the number of cases of norovirus infection is growing. The stability of pathogens in the environment, the presence of asymptomatic carriers, the frequent family and group nature of the disease, the ease of infection in various ways, the low coverage of specific prevention, create prerequisites for an increase in the incidence. This article studied the clinical and epidemiological features of the most relevant infectious diarrhea of rotavirus and norovirus etiology. This information is of undoubted interest for practitioners, it aims to help in the early detection of patients, the implementation of adequate anti-epidemic measures and rational pathogenetic treatment.

BIOIMPEDANCE ASSESSMENT OF HEAT-CARTER PHYSIQUE IN MALES OF DIFFERENT ETHNIC GROUPS OF THE FIRST MATURE AGE PERIOD
Abstract
Data on somatometry and bioimpedansometry in persons of Kyrgyz and Uzbek nationality of the city of Osh are given. Comparative analysis of interethnic group somatotypes revealed a predominance of mesomorphic body types. In persons of Uzbek nationality, ectomorphs are more common than in persons of Kyrgyz nationality. The endomorphy is recorded only in Kyrgyz. The distribution of somatotypes showed variability in absolute and relative values. In Kyrgyz, bowls are found from endomorphic groups - endomesomorphy, from mesomorphies - balanced mesomorphic, ectomesomorphic and mesoectomorphic, from ectomorphies - balanced ectomorphic body type. The relative significance of subgroups of mesomorphies in Kyrgyz is higher compared to similar values in persons of Uzbek nationality. Of the ectomorphies, there is a balanced ectomorphic type of physique and endoectomorphy. Body composition parameters, namely the content of fatty, active cell mass and other components, are important in setting regulatory standards in healthy men of different ethnic groups of the first mature age period.

CURRENT VIEWS ON THE TREATMENT OF HORMONALLY ACTIVE NEUROENDOCRINE TUMORS OF THE PANCREAS, EXCLUDING INSULINOMA
Abstract
The prevalence of neuroendocrine tumors (NETs) has been increasing in recent years. The most common localization of NET is the gastric-intestinal tract, in particular the pancreas (PV). Among hormonally active NETs, the most common are insulinoma, gastrinoma, glucagonoma and VIPoma. The treatment of the latter three is rarely addressed in the scientific literature due to their low prevalence. The purpose of this review is to summarize the latest information on the treatment of hormone-active RV tumors such as gastrinoma, VIPom and glucagonoma (excluding insulin). For treatment with gastrin, symptomatic therapy is used, including antisecretory drugs (mainly proton pump inhibitors), surgical treatment is currently used more often than in the past. Symptomatic therapy also plays a major role in the treatment of VIP, since the tumor is often malignant, and by the time of diagnosis, most patients have metastases. Symptomatic treatment includes replenishment of fluid and electrolytes, including with the help of infusion therapy, administration of somatostatin analogues, antidiarrheal agents. Somatostatin analogues, resection of the primary pancreatic tumor are used for glucagon treatment. In metastatic-advanced NETs, surgical treatment, tumor embolization, ablation, targeted therapy, and in some cases liver transplantation are used.

Abstract
The article contains up-to-date data on factors affecting the development of the intestinal barrier in premature infants and promising markers for the diagnosis of small intestine permeability. The significant role of early breastfeeding on the state of the mucous membrane of the small intestine and colonization of the intestinal microbiota is shown.

The presence of severe forms of IBD is observed in patients with comorbid pathology, a large proportion of which are metabolic disorders (obesity, type 2 diabetes mellitus).

THE VALUE OF DETERMINING THE LEVEL OF PROSTATE SPECIFIC ANTIGEN IN THE DIFFERENTIAL DIAGNOSIS OF PROSTATE CANCER
Abstract
The article deals with the diagnosis and differential diagnosis of prostate cancer and benign prostatic hyperplasia. The increased level of prostatic specific antigen is one of the indicators on the basis of which a decision is made to perform a prostate biopsy and conduct a histological examination of the obtained material. The results of histological findings in patients with benign prostatic hyperplasia and malignant growth in the prostate gland were compared with the values of prostatic specific antigen. The importance of the relationship between the level of prostate specific antigen in serum and detection of prostate cancer through a multifocal spectacle needle biopsy of the prostate under ultrasound examination control.
