STUDIES OF ANTIPYRETIC PROPERTIES OF TETRAHYDROPYRIDO [2,1-b] [1,3,5] THIADIAZINE DERIVATIVES

Abstract

Antipyretic activity of tetrahydropyrido [2,1-b] [1,3,5] thiadiazine derivatives was studied in vivo in comparison with paracetamol and analgin. Compounds with a pronounced antipyretic activity significantly higher than that of paracetamol were found. A clearly pronounced high antipyretic effect is associated with the ability to inhibit TsOG-2 after penetration of the blood-brain barrier.

THE EVOLUTION OF SURFACTANT INTRODUCTION: PRACTICES OF THE XXI CENTURY

Abstract

About 4 million newborns die every year in the world, where children with respiratory distress syndrome account for at least 1 million of these deaths. Despite the progressive development of medicine and perinatology, respiratory distress syndrome of newborns (RDSN) it still remains a rather serious problem, and approaches to the diagnosis and treatment of this disease have been changing over the past decades. It is important to note that the discovery of the etiology of RDSN, which is based on surfactant deficiency, and then the introduction of this synthesized substance into widespread practice, had a positive effect on outcomes, but we still need to study and adjust existing approaches to create an optimal strategy. In this review article, we will try to touch on the main aspects of stabilization of newborns with RDSN, as well as demonstrate various approaches to the introduction of surfactant that are used in modern practice.

PROMISING DEVELOPMENTS OF NEW DRUGS IN THE CONTEXT OF THE COVID-19 PANDEMIC FOR PATIENTS AT RISK

Abstract

The article describes in vivo screening studies of 4 samples of partially hydrogenated pyridine derivatives synthesized at the Research Laboratory "Chemex" on a model of steroid diabetes mellitus. Steroid diabetes mellitus was modeled in 18 month old outbred laboratory rats for 13 days by intraperitoneal administration of dexamethasone at the rate of 125 µg/kg of body weight. Pharmacocorrection was carried out by intragastric administration at a dose of 1 mg/kg of body weight for 21 days of compounds of 1,4-dihydropyridine derivatives (cv140, cv-150), and 1,2,3,4-tetrahydropyridine derivatives (cv-083, jen 09 - 039). Hypoglycemic activity was noted in all 4 test compounds. In addition to hypoglycemic activity, additional pharmacological activity was found that positively affects the metabolic processes of late diabetic complications. Pharmacocorrection of metabolic and psychosomatic status was confirmed by visual observations, as well as biochemical blood analysis. The most promising for further research are compounds with laboratory codes cv-140 and cv-150.

RECOVERY TREATMENT NEEDS FOR PATIENTS WITH POST-COVID COMPLICATIONS VGMU im. N.N. Burdenko

Abstract

Currently, in clinical practice, there is very limited information on the nature and prevalence of post-COVID complications. The study was conducted on 100 patients who had previously received inpatient or outpatient treatment at medical institutions in the city of Voronezh for community-acquired pneumonia COVID-19. All study participants were assessed for physical and neuropsychic status. Fatigue associated with the underlying disease was the most common symptom, reported by 69.8% of patients. The next most common symptoms were shortness of breath (64.2%) and psychological distress (47.3%). A clinically significant reduction in physical resistance of 43.9% was noted. This is the first study in Voronezh that reports symptoms after discharge from the hospital. The results obtained allow us to recommend the rehabilitation of these symptoms and the maximum functional recovery of patients who have undergone COVID-19.

Abstract

The structure of diagnoses and age and gender characteristics of patients seeking medical care after a new coronavirus infection COVID-19 are analyzed. A cluster of the most characteristic symptoms present in patients who have undergone COVID-19 has been identified. The features of the delayed effect of the new coronavirus infection COVID-19 on the adult heart were studied on the basis of magnetic resonance imaging of the heart with contrast enhancement. An algorithm for selecting patients with suspected heart disease after COVID-19 for in-depth examination is proposed.

ASSESSMENT OF GENDER DIFFERENCES IN INCIDENCE OF COMORBID CONDITIONS OF THE CARDIOVASCULAR SYSTEM IN THE ELDERLY POPULATION OF THE CITY OF KURSK

Abstract

The probability of several pathologies occurring at once with a common pathogenetic mechanism is much higher in old age. These conditions, which named comorbidity, affect the course of the underlying disease and lead to an unfavorable outcome. The gender factor is also of great importance for the implementation of comorbid forms of diseases. Therefore, we research to assess and take into account the mechanisms of occurrence, frequency and predisposition to certain comorbid conditions in different age and gender groups. In the course of the study, a retrospective analysis of combined cardiovascular pathologies (dyscirculatory encephalopathy, aortic atherosclerosis) of the elderly population by age and gender was carried out. According to the results obtained, age has a direct effect on the likelihood of comorbid states of both parameters studied, whereas by gender it was revealed that dyscirculatory encephalopathy against the background of previous pathologies is predominantly male.

MODERN IDEA OF COXIELLOSIS IN THE VORONEZH REGION

Abstract

The article discusses the clinical and epidemiological features of the modern course and laboratory diagnosis of Q fever (QF) or coxiellosis, which is one of the zoonotic natural focal infectious diseases of the Voronezh region. Coxiellosis is a systemic natural focal zoonotic disease with a wide geographic range. Our region is officially included in the statistics of regions where the disease is registered almost annually. Difficulties in the clinical diagnosis of Q fever are due to pronounced polymorphism of manifestations of the disease and the absence of pathognomonic symptoms. The possibility of a chronic course with the development of endocarditis worsens the prognosis of the disease due to high mortality (up to 60%). This disease requires attention and careful study by modern internists. To make a diagnosis, it is necessary to know epizootological data, epidemiological history, clinical manifestations of the disease, the results of laboratory studies, which detect antibodies to Burnet's coxiella and pathogen DNA: Methods of enzyme-linked immunosorbent assay (ELISA) and polymerase chain reaction (PCR) are used.

THE ROLE OF PREVENTIVE INTERVENTIONS IN THE INCIDENCE OF VIRAL HEPATITIS "A" IN THE VORONEZH REGION

Abstract

The state of the incidence of viral hepatitis A (HAV) in the Voronezh region was analyzed against the background of measures taken to reduce the importance of the transmission waterway and immunoprophylaxis. The data of the official statistical reporting on the Voronezh region, departmental reports of the Department of Health of the region were used. As the provision of good-quality drinking water improves, the role of the transmission waterway in the incidence of HAV in the region decreases. Immunization against HAV in groups of children and adolescents, as well as vaccination in foci, during hydrological disasters, have an impact on interrupting the spread of the disease. The effectiveness of regional preventive and anti-epidemic measures can be traced in reducing the incidence of HAV to a sporadic level.

PERSONALIZED APPROACH TO THE TREATMENT OF PATIENTS WITH OBESITY AND BRONCHIAL ASTHMA

Abstract

As shown by international and domestic studies, obesity is diagnosed in the majority of BA patients. At the present stage, a number of foreign and domestic scientists focus on the effectiveness of physical activity and a balanced low-calorie diet in the course of asthma. The scientific study presents data on the analysis of the effectiveness of physical activity and a low-calorie diet included in the complex treatment of patients with asthma and obesity. When using personalized therapy of patients with an emphasis on dosed physical exercises and balanced diet therapy, significantly positive dynamics of anthropometric parameters, the course of asthma, a statistically significant decrease in the severity of clinical manifestations of asthma, the achievement of optimal control over the disease and an increase in exercise tolerance were noted.

ТYPE 2 DIABETES MELLITUS THERAPY OPPORTUNITIES IN THE REPUBLIC OF BELARUS

Abstract

Dynamic updating of international recommendations for the treatment of diabetes type 2 requires timely correction of national protocols and algorithms to ensure the quality of medical care. Material and methods. An analytical review of national regulatory documents regulating the treatment of patients with type 2 diabetes in the Republic of Belarus was carried out to determine the need for their correction. Results and discussion: Treatment of patients with type 2 diabetes in the Republic of Belarus is regulated by the national clinical protocol (CP). Type 2 diabetes is included in the list of diseases that give citizens the right to free provision of medicines (MP) issued by doctors' prescriptions within the List of Essential Medicines (EML) for outpatient treatment. Free provision of drugs for inpatient treatment is provided within the limits of the Republican Formulary of Medicines (RFL). The drugs included in the CP are partially not included in the EML and RFL and are not subject to state funding. Conclusions: the inclusion of drugs in the CP should contribute to the correction of restrictive lists that regulate the financing of pharmacotherapy. Timely correction of regulatory documents that define the treatment of type II diabetes, revision of the restrictive lists of drugs, taking into account new data on the effectiveness and economic opportunities of the healthcare system, will help expand the possibilities of pharmacotherapy and improve the quality of medical care for this category of patients.

BIVALVE AORTIC VALVE AS RISK FACTOR FOR ISCHEMIC STROKE: EFFECTIVE REHABILITATION METHODS

Abstract

Acute cerebrovascular accident (ONMC) is still one of the leading causes of mortality and disability among all nosological diseases. Ischemic stroke (AI) may be the first clinical manifestation of a cardioembolic variant in cardiovascular pathology (CCC). Cardiac embolism accounts for about 1/4 of AI. Strokes due to cardioembolism are usually severe and are prone to early and distant relapses. Cardioembolism can be suspected by clinical signs, but is often difficult to diagnose. Magnetic resonance imaging (MRI), transcranial dopplerography, echocardiogram, Holter and electrophysiological studies can help establish the source of cardioembolism. The bicuspid aortic valve (DAK) is by far the most common congenital heart disease (PRS). Remaining asymptomatic for a long time, it can eventually be complicated by the development of aortic stenosis or aortic insufficiency. DAC is also associated with aortic aneurysm and its dissection. Given the high prevalence of this defect and the severity of the complications, it is believed to be responsible for more deaths than all other congenital heart defects combined. Therefore, it is relevant to determine the characteristics of patient management taking into account comorbid pathology and the formation of a multi-disciplinary team (MDB) for an individual approach to the rehabilitation and treatment of patients with DAK. The article details the clinical case of a 47-year-old patient with AI and newly diagnosed DAK in the absence of obvious clinical signs of heart disease. The importance of routine assessment of cardioembolic sources of all AI development options is emphasized. As well as detailed consideration of the modern possibilities of conservative treatment of AI and effective rehabilitation methods.

ANTIVIRAL ANTIBODIES AND IMMUNE STATUS OF PATIENTS WITH ANXIETY-DEPRESSIVE DISORDERS

Abstract

A study was performed on the presence of antibodies against Epstein-Barr viruses, herpes simplex, cytomegalovirus in persons suffering from anxiety-depressive disorders. Pronounced changes in immunological parameters in this contingent from the level of practically healthy volunteers are shown. The dependence of quantitative and qualitative variations of immunological indicators, their correlation relationships on the type of carrying antiviral antibodies by patients has been established. It has been established that in patients with anxiety-depressive disorders, the carriage of antiviral antibodies against EBV occurs in each of the patients we studied, the combination of EBV, HSV and CMV antibodies is 70.5%, the combination of antibodies to EBV and HSV in 85.3% of cases, the combination of antibodies to EBV and CMV in 82.4% of cases.

ETHNIC FEATURES OF CYTOCHROME P 450 POLYMORPHISM IN RESIDENTS OF THE VORONEZH REGION

Abstract

The paper analyzes the frequency of polymorphism of enzymes of the cytochrome P 450 (CYP) family in regional conditions in residents of the Voronezh region. As a result of studies performed in 2021 in 69 residents of the Voronezh region during genetic testing of CYP 2C19 isoenzymes, the frequency of occurrence of "slow" metabolizers (15.8% of patients), "fast" metabolizers (2.8% of patients) was established. To assess the relationship between the phenotype and genotype of patients, a Born aggregogram was performed, which reflects the functional activity of platelets. The aggregation ability (aggregation ≥ 40%) of platelets was observed in patients who had a decrease in the metabolic rate of CYP 2C19.
