RESULTS OF THE ONLINE SOCIAL AND PSYCHOLOGICAL SURVEY OF VOLUNTEERS DURING THE INITIAL PERIOD OF SELF-ISOLATION IN ST. PETERSBURG (APRIL 2020)

Abstract

The results of an online survey using the Google form of the questionnaire «Your condition and behavior during the new type of coronavirus infection (COVID-19) pandemic» were analyzed in 34 voluntary participants aged 19-53 years, 27 women and 7 men. Among the respondents, 17 people were students aged 19-27 years and 17 people working at the age of 24-53 years. The respondents were united by a single time period of the survey, most of them (82.4%, χ²=16,941, p=0.0001) did it on 09.04.2020, which is important in terms of the dynamics of the epidemic situation. The results showed that the respondents' students showed a greater interest in independently obtaining information about the COVID-19 epidemic compared to the working ones. Such characteristics as compliance with the epidemic, including respiratory regime, family relations, boredom in connection with self-isolation did not differ in working and student respondents, as well as in those who switched to remote mode and continued their activities as usual. The assessment of their mental state showed that the workers, compared to the students, had a more pronounced panic due to the possible infection of them and their loved ones with COVID-19. The emotional responses associated with the coronavirus epidemic did not differ among workers/students who were in normal and remote modes. The transition to remote mode was associated with an increase in Internet activity.

CHANGES IN THE LEVEL OF ANXIETY IN THE POPULATION DURING THE COVID-19 PANDEMIC. FACTORS AFFECTING THE DYNAMICS OF THE ANXIETY

Abstract

The article analyzes the structure of the population's anxiety based on the data obtained by the survey held online in November 2020, comparing the data obtained with similar indicators for April 2020 and with the average level of anxiety in Russia before the outbreak of the new coronavirus pandemic. The anxiety and depression of the survey participants were assessed using the HADS psychometric scale adapted to the online format. In addition, the questionnaire contains questions clarifying the attitude of the study participants to compliance with social, medical, preventive, public, and individual measures of protection against coronavirus infection, the level of compliance with state requirements and actions to protect against COVID-19, the respondents' assessment of the situation in society, and other aspects by performing the choice of an answer option or entering their own version of an answer in written form. Based on the obtained results, the level of anxiety was compared with other aspects of assessing the psychological state of the survey participants, taking into account individually significant stress factors noted by the respondents themselves. Thus, in the course of analyzing the results of the study, its goal was not only to determine the overall level of anxiety and depression within society, but also to recognize the different structure of the predicted increase in this level, as well as to compare different indicators of anxiety with various factors that presumably increase public alertness and stress potential.

CLINICAL CASE OF ACUTE POLYMORPHIC PSYCHOTIC DISORDER DEVELOPING ALONGSIDE WITH COVID-19 IN THE PATIENT WITH THE MEDICAL HISTORY OF KIDNEY TRANSPLANTATION

Abstract

According to available data, COVID-19 is often accompanied by mental disorders. The article describes one of such clinical cases. The patient was not previously followed up by psychiatrists. We thoroughly studied the case history and analysed the treatment process of a patient with acute polymorphic psychotic disorder, which developed alongside with COVID-19 diagnosed by means of a PCR test. The article details the onset of the disease, its development, and the dynamics of the results of laboratory and clinical investigation. It also describes the treatment selected considering the fact that the use of psychopharmacotherapy was limited by a serious chronic disease of the patient. The study demonstrates the necessity of timely opening of COVID hospital units at psychiatric treatment facilities.

STUDY OF PSYCHOEMOTIONAL STATUS IN DOCTORS TREATING COVID-19 PATIENTS

Abstract

The article is devoted to the study of some aspects of the psychoemotional status of doctors working in the "red zones" and providing assistance to patients with Covid-19. This topic is currently very relevant, since only recently began a "decline" in the incidence of a new coronavirus infection, which allowed a number of medical scientists to suggest the beginning of the "end of the second wave" of Covid-19. The objects of this two-stage study were 100 doctors of various departments of the BUZ VO VOKB № 1, of both sexes, aged from 27 to 62 years. All of them were divided into 2 groups of 50 people each, depending on whether they provided standard medical care (group I) or treated patients in covid departments (group II). The study was based on the methods of Spielberg and Zung, aimed at determining the levels of anxiety and identifying possible depressive states. In addition, the authors developed a questionnaire, the purpose of which was to determine the main reasons that prompted doctors to work in covid departments or to refuse from it. In April and November 2020, anxiety levels were determined in all subjects, possible depressive states were identified, and changes in their dynamics were noted. The analysis of the main reasons, as a result of which doctors refused or worked in covid buildings, was carried out. This study is of interest not only for psychologists, but also for healthcare organizers and managers of multidisciplinary hospitals.

ABOUT THE PLACE OF HALOPERIDOL IN THERAPY OF PARANOID SCHIZOPHRENIA

Abstract

The efficacy of haloperidol in combination with a second-generation antipsychotic in relieving exacerbation of paranoid schizophrenia in outpatient practice was studied. Depending on the structure of the psychotic episode (paranoid, hallucinatory, or polymorphic), monotherapy with a second-generation antipsychotic or combination therapy with haloperidol and a second-generation antipsychotic was performed. The clinical and psychopathological method was used to assess the therapeutic reduction of productive symptoms. The results obtained show that haloperidol can be used to relieve exacerbations of paranoid schizophrenia in outpatient practice. The approach to choosing haloperidol may be based on the type of psychotic episode. In particular, the appointment of haloperidol is justified in combination with second-generation antipsychotics in cases of psychosis with a hallucinatory or polymorphic exacerbation structure with a progressive type of disease course.

THE INFLUENCE OF RISK FACTORS AND THE DEGREE OF LOSS OF EMPLOYMENT ON THE DEVELOPMENT OF POST-STROKE DEPRESSION AND ANXIETY IN PATIENTS WITH CVA

Abstract

Stroke is one of the most common diseases of the nervous system, which is often complicated by the development of anxiety-depressive disorders. The frequency of occurrence and features of the course of depressive disorders in patients with stroke, as well as their relationship with somatic burden, were assessed. A retrospective analysis of 40 case histories of patients with stroke was carried out, a questionnaire survey using questionnaires: Hospital Anxiety and Depression Scale (HADS), Toronto Alexithymic Scale, TAS-26. Depressive disorders of varying severity were detected in more than 70% of the examined patients with stroke. Females are more likely to develop depression and anxiety. Among patients after ischemic stroke and acute myocardial infarction, the proportion of people with moderate and severe depression was higher. It was revealed that the younger the patients who have suffered a stroke, the harder they tolerate the fact of the disease and the more pronounced their depressive state. The severity of functional deficit after stroke directly affects the risk of developing post-stroke anxiety-depressive disorders. Thus, anxiety and depression in patients who are not able to perform work, therefore, released from work duties was manifested by clinically pronounced symptoms in 74.1% of cases, which indicates a significant role of the psychological reaction to the disease.

CORRELATION OF INTERNET ADDICTION WITH CODEPENDENCY AND TEMPERAMENT

Abstract

In recent decades, the availability of the Internet has become almost ubiquitous. However, this fact has led to the fact that many experts began to notice the negative consequences of using the Internet, and therefore research in this area is particularly relevant. Our study included 57 participants, with an average age of 28.12±9.31 years, most of whom were women (45). All passed the CIAS scale for Internet addiction, the Weinhold codependency scale, the EPI personality questionnaire, as well as some questions on the social profile. After processing the data obtained, all respondents were divided into 3 groups: Internet addicts (N=9), those who are prone to forming Internet addiction (N=24), and the minimum risk of Internet addiction (N=24). When comparing the data between these groups, the following conclusions were made: codependent people are more likely to form Internet addiction, training/working off line, and younger age contributes to the formation of Internet addiction.

EFFECTIVENESS OF VORTIOXETINE IN THE TREATMENT OF DEPRESSIVE DISORDERS IN HYPOTHYROIDISM

Abstract

The article presents the data of a retrospective study of the effect of combined therapy with vortioxetine and L-thyroxine on the severity of anhedonia in patients with mild and moderate depressive episodes in patients with hypothyroidism. The severity of depressive symptoms was directly related to the severity of anhedonia in patients with hypothyroidism. There is a higher prevalence of anhedonia in patients with hypothyroidism than in the population of patients with depressive disorder without indication of hypothyroidism. Vortioxetine in combination with levothyroxine showed high efficacy and safety in the treatment of depressive disorders in patients with hypothyroidism, the proportion of respondents in relation to depressive symptoms was 83.3%. The pronounced effect of combined therapy with vortioxetine and L-thyroxine on the manifestations of anhedonia, assessed on the Snight-Hamilton anhedonia assessment scale, in patients with hypothyroidism and a mild to moderate depressive episode was shown.

INFLUENCE OF DISTANCE LEARNING ON THE PSYCHOEMOTIONAL STATE OF STUDENTS

Abstract

This article examines the psycho-emotional state of students and the level of their anxiety during the period of distance learning. With the help of a questionnaire, an assessment of the psychoemotional state of students of 4 universities of St. Petersburg was carried out. The study revealed that distance learning has an ambiguous effect on the psychoemotional state of students. Most of the students are satisfied with the distance learning regime and have adapted well to it, but at the same time, many of them developed concerns about a possible decline in academic performance and a lack of practical skills. When assessing the level of anxiety using the integrative anxiety test (ITT), it turned out that the situation of self-isolation and distance learning did not become as traumatic for students as expected. Most universities still continue to be in remote mode, so at the moment it is difficult to predict how the psycho-emotional state of students will change in the future.

FEATURES OF MENTAL ADAPTATION OF LIBRARY STAFF

Abstract

The results of research of mechanisms of psychological protection in library employees are presented. Many studies suggest that library staff experience significant emotional and intellectual overload, like other specialists whose work is related to communication with people. In this regard, these employees are subject to the development of states of maladaptation and the development of emotional burnout. The study of the features of mental adaptation will make it possible to draw up psychoprophylactic programs to prevent the development of a state of maladaptation and deterioration of mental health among library staff. The state of mechanisms of psychological protection of employees of libraries of Chuvashia was studied depending on the length of work, level of education, place of residence, family status and subjective assessment of the level of their own competence. The study shows that most library staff have good mental adaptability. On the other hand, library staff most often use psychological protection mechanisms that can reduce their adaptability by creating difficulties in understanding intra-personal problems. Lower levels of education and staff experience for more than 10 years may be risk factors for staff maladaptation.

MANIFESTATIONS OF EATING DISORDERS IN HIGHER EDUCATION STUDENTS

Abstract

The purpose of the research is to analyze the characteristics of eating behavior among students of a higher educational institution. It performed the study of the characteristics of eating behavior of 570 students, including 146 boys (25.6%) and 424 girls (74.4%) by means of a survey using the developed questionnaire. It was found that out of 570 surveyed 435 students (76.3%) regularly overeat. At the same time, daily episodes of overeating were noted by 90 people (20.7%). Almost one third - 139 (32.0%) of the respondents reported that they overeat several times a week, and 159 students (36.6%) reported overeating once a week. 47 respondents (10.7%) reported the frequency of overeating once a month. More than two thirds of students (298 people, 68.5%) reported that these episodes can occur in the presence of other persons, while 137 respondents (31.5%) reported that during the moments of overeating, none of the outsiders was present. In more than 50% of cases, the consumption of chicken meat (51.7%), dairy products (56.6%), products from cereals (52.3%), carbonated water (54.0%) is noted. A small proportion of respondents - 10.8% - reported eating large amounts of vegetables and fruits. A high frequency of episodes of overeating among students of medical universities was established, which indicates the presence of signs of eating disorders. For the vast majority of students, these episodes can occur in the presence of other persons. A multidisciplinary approach is needed to study and correct eating disorders and assess its relationship to various social factors, patient characteristics and environmental influences.

ON THE IMPORTANCE OF PREVENTION OF DISEASES OF DEPENDENCE AMONG STUDENTS DURING AN ADVERSE EPIDEMIOLOGICAL SITUATION

Abstract

The article is devoted to the study of the attitude of teachers of a medical university to "bad habits" among their students, as well as an attempt to determine the frequency of adherence to them by the students themselves, including to identify changes in the dynamics of alcohol consumption, smoking, etc. with the onset of the period of distance learning caused by the unfavorable epidemiological situation caused by Covid-19. The objects of this two-stage study were 100 teachers of a medical university, men and women of different ages, divided into 2 groups of 50 people, depending on the course of study of students: 1-3 courses - 1 group and 4-6 courses - 2 group. And also 400 students of pediatric and medical faculties, divided into 2 groups of 200 people, depending on the course (2 and 5). Subsequently, they were divided in each group into 2 subgroups depending on their location during the distance learning period: at home with relatives or alone in a hostel. With the help of anonymous questionnaires, the very attitude of teachers to the "bad habits" of students and the frequency of preventive conversations were studied. Among the students, persons with "similar habits" were identified and the dynamics of the frequency of "use" was analyzed with the beginning of the distance learning regime. At the final stage, the attitude of future doctors to preventive conversations conducted by teachers was determined. The article is of interest not only for psychologists, but also for all teachers of higher education.

FUTURE - ORIENTED PARADIGMS OF THE TREATMENT OF ANXIETY AND DEPRESSION

Abstract

This article investigates future - oriented paradigms of the treatment pf anxiety and depression. The review looks into associations between neutrally mediated (glutamatergic, GABA - ergic, hypophyseal portal) and regulatory (endocrine, immune, central nervous) systems. Data of studies show that there appears a neuroinflammation after stress influence. Also, there was an immunosuppression, improved by inflammatory factors’ and glucocorticoids’ rush. Based on these data, this review sees into experimental and innovative strategies of anxiety’s and depression’s treatment. The article sets down such new ways as an influence of Ketamine and immunocorrectors on anxious and depressive symptoms. The information about link between glucocorticoids and depression and anxiety was discussed. Therefore, the aim of executed review is investigation of association between anxiety, depression and its immunological and neurally mediated aspects of anxiety and depression pathogenesis; and identification of the most promising treatment strategies.

TOPICAL ISSUES OF PREVENTION OF MENTAL DISORDERS AT THE STAGES OF MEDICAL EVACUATION

Abstract

The issues of prevalence of emergency situations in Russia, peculiarities of mental reactions to stress are considered. Theoretical and statistical methods are used from research methods. In the formation of mental disorders in emergency situations, temporary periods are distinguished. Specialists of emergency medical care, emergency rescue, emergency psychological formations face the first period of formation of mental disorders, the main task is to isolate persons with destructive forms of behavior maladaptive reactions. Adaptive reactions were noted in 34% and maladaptive reactions of the hyperkinetic and hypokinetic variants in 66%. In the acute period, the intensity of reactions decreases, which contributes to the social adaptation of most of the people who have suffered a disaster. In some cases, anxiety-phobic reactions are transformed into neurotic conditions, the main task is their active detection and hospitalization in this period. In a distant period, the development of post-traumatic stress disorders is possible in both victims and specialists of rescue formations and medical workers with a long time of work. Carrying out work to increase the psychological literacy of specialists of emergency rescue formations, their testing of first aid techniques, correct organization of work in emergency situations, clear teams and confident competent actions of employees contributes to the prevention of the development of mental disorders.
