Abstract

The article presents the organization of training students of a medical college during the period of industrial practice, as an element of increasing the activity of medical organizations, shows the directions of forming professional competencies, defined the structure, content, individual needs and opportunities of students during this period. As a result of the practice, the results of the questionnaire are presented, and the problems that students encounter when passing practical training at various courses are underlined. Methods for solving difficulties in the proposed professional activity are suggested.

UNDERSTANDING OF THE GROWING GENERATION ABOUT THE BASIC PRINCIPLES OF HEALTHY LIVING

Abstract

Currently, the percentage of healthy people in the country is constantly decreasing. Adult health is directly linked to the health of the younger generation. A healthy adult cannot grow out of an unhealthy teenager. And this situation is aggravated every year. When should one begin to inculcate the principles of a healthy lifestyle? The answer to this question is obvious - as soon as possible. From what and how will be incorporated in a person from early childhood, will largely depend on his lifestyle, principles of behavior in society and health. We conducted a survey of 65 students of the 1st course of the medical and pediatric faculty of the Voronezh State Medical University on awareness of the basic principles of a healthy lifestyle. According to the results of the survey, we concluded that, despite the fact that most of the respondents all the same know what a healthy lifestyle is and its basic principles, they are still not in a hurry to follow these principles in life.

Ways to improve the quality of eye care for students of Voronezh region universities

Abstract

This article analyzes the primary and General morbidity for 2011-2016 of eye diseases and its subordinate apparatus among University students of various profiles of the Voronezh region. The dynamics was formed according to official statistics and the results of medical examinations. Nosological forms of diseases are studied, their level and specific weight in structure of the studied phenomena, ways of improvement of quality of the ophthalmologic help to students are estimated. Analysis of the data showed a negative trend towards an increase in both General and primary incidence of eye diseases. Refractive errors, inflammatory diseases, strabismus, diseases of the optic nerve and retina occupy a leading place in the structure of ophthalmological morbidity of students. The increase in the number of diseases of the eye and its subordinate apparatus is associated with a high intensification of the educational process, the introduction of computerization in this process, accompanied by an increase in visual loads, as well as with an increase in the number of students entering the University with an existing pathology of the organ of vision. Special attention should be paid to the opinion of students of most universities that qualified ophthalmic care is not always available. Therefore, it is important to improve, especially the primary link that can solve the problems of prevention of eye diseases, as well as disability (with an emphasis on prenosological diagnosis and medical and social prevention) among students. An extremely important role in the implementation of this work belongs to educational organizations of various levels. During the next large-scale medical examination, a special role should be given to the student polyclinic of the city of Voronezh, which is designed to ensure the effectiveness of preventive and recreational activities at the University level.

ASSESSMENT OF THE HEALTH STATUS OF CHILDREN RESIDING IN THE TERRITORY WERE EXPOSED TO RADIATION OWING TO ACCIDENT ON THE CHERNOBYL NUCLEAR POWER PLANT

Abstract

The accident at the Chernobyl nuclear power plant resulted in radioactive contamination with long-lived isotopes of vast areas where large populations of people currently live and are exposed to varying degrees of low-intensity chronic radiation exposure. Belgorod region in this regard also belongs to the number of affected regions, where 93 settlements were subjected to radioactive contamination. It is well known that a young growing organism is most sensitive to radiation exposure. Therefore, the purpose of our study was to study the health indicators of children living in the territory of the Belgorod region, which suffered from the Chernobyl nuclear power plant. Materials of official statistics of the Belgorod region (form No. 12, 63) for 2010 and 2015 are used. The analysis of primary and general morbidity according to the appeal of the children's population to medical organizations was carried out. Results. A comparative analysis of the incidence of children in this area reveals growth in four classes of diseases - neoplasms, congenital malformations, diseases of the musculoskeletal system and individual states of the endocrine system. In the structure of neoplasms for 2010-2015 there was an increase in the percentage of malignant tumors from 12.3% to 29.2%, i.e. in 2,4 times. The conclusion. Diseases of the musculoskeletal system, digestive system, respiratory system, skin and hypodermic tissue and endocrine system prevail in the structure of the morbidity of children living on the territory affected by the Chernobyl accident. For the period under study, the obtained tendencies of growth of oncological morbidity of children are associated primarily with the improvement of diagnostics and detectability. However, it remains relevant to establish the connection of certain groups of diseases with radiation.

Abstract

Modern views on preventive examination and screening in the health care system for the way of evaluation of presence of risk’ factors and(or) diseases in man. The advantages and the disadvantages of screening programs are discussed. Examples of screening programs’ realization in foreign countries are given and the comparison of those to the prophylactic medical examination of detached group of citizens in Russia was provided. There are special governmental organizations which develop the national recommendations on the screening policy in the country. Different approaches for the choice of diseases to be included in the screening list and different methods of screening’s reasonability evaluation are used. There is no country provided the screening programs on all disease and risk factors for all the citizens using all the diagnostic methods.

The article talks about the importance of adhering to ethical and deontological norms in medicine. The fundamentals of the professional behavior of medical personnel are laid during studies at the university, especially with a rich hundred-year history and traditions. Touching the heart of a person can turn his life into a service. Therefore, initially it is required to form correct ideas about the proper behavior of a specialist, to lay down fundamental ethical-deontological concepts. This lofty goal can be achieved only after identifying students' initial level of knowledge on the subject under study. The results of the survey of first-year students of medical and pediatric faculties show a good potential and correct orientation of future medical professionals.

Abstract

The article talks about the importance of adhering to ethical norms and rules, it allows you to avoid many conflict situations, unfounded claims, patient quibbles. When reducing the number of negative situations on professional grounds in a medical organization, favorable prerequisites are created for normalizing the microclimate and increasing the number of positive patient feedback. The article presents the results of a survey of nursing staff on their knowledge of the main provisions of the Ethical Code of Nurses of Russia. Possible reasons for non-compliance with the Code have been identified, on the basis of which conclusions can be drawn and corrective action plans can be drawn up. If the ethical principles of the Code are observed, the quality of medical services is improved, and, therefore, the Code can be used as a means of improving the quality of medical care.

INFLUENCE OF "HIV / AIDS Denialism" ON INCIDENCE OF HIV INFECTION

Abstract

The questions connected with the movement HIV - recusancy in Russia are considered. The analysis of awareness of youth on the problems connected with this movement is carried out and ways of prevention of its negative impact are defined. In a research it is established that the youth is insufficiently informed on existence of the movement of HIV dissidents. Modern society in general is negative to HIV dissidents and their views enough, seeing in them the threat promoting mass infection of the population and increase in mortality percentage among HIV-positive citizens. At the same time actions of HIV dissidents interfere with prevention and treatment of HIV infection and AIDS that only aggravates an epidemiological situation on this disease. Having moved to influence of adherents of the movement, many HIV-positive citizens die at working-age that, in turn, aggravates a social and economic situation in the state. Well thought over measures of implementation of educational programs, the organization of professional consultation when conducting testing for HIV infection existence, competent psychological assistance will help to reduce considerably number of those who adhere to views of HIV dissidents.

Abstract

The structure of disability owing to the tserebrovaskulyarnykh of diseases is studied in detail, regularities of formation and feature of medico-social rehabilitation of this contingent of patients are defined. Materials of addresses and results of work of the commissions of medico-social examination are studied. It is established that the level of primary disability owing to the specified pathology in the Voronezh region tends to increase in recent years. The contingent for the first time of the diseases recognized by disabled people owing to the tserebrovaskulyarnykh is characterized by prevalence of persons of retirement age, disability mainly of the III group. The trend to decrease in number of disabled people of a retirement age owing to tserebrovaskulyarny pathology and increase in number of persons of working-age is revealed.

In article are stated, the reasons found the author for which there are no desirable considerable changes in health care. All of them have subjective character. A part depends on heads, other part - on performers of medical production. Heads don't show interest in elimination of subjective approaches to assessment of employees on professional qualities and commensurable to it compensation of results of their work. Prefer to encourage "the" people, using subjective approach. Performers wish to remain equal with each other and to find other sources of welfare. These two qualities interact among themselves by the principle of synergism, strengthening each other. For a way out the author suggests to exercise will and the power, and also an experiment which results will convince of need of implementation of offers on reforming of a health care system.

Abstract

Regional oncological care monitoring is an analytical system of continuous long-term observation, analysis, evaluation and prognosis of both the epidemiology of malignant neoplasms and the dynamics of quantitative and qualitative indicators of medical care. Evaluation of the effectiveness of cancer care and the determination of critical points based on large volumes of information is based on the functioning of the regional population cancer registry. The general requirements for the structure of the regional oncological monitoring system are determined, which help to optimize the management of oncological activities at all its stages. The introduction into the management process of mathematical methods for solving problems, analytical information systems ensures rational use of financial and human resources, reducing the risk of making unreasonable decisions related to information defects, as well as expanding the possibilities for timely correction of the management process. Presents the design optimization of the management of the regional cancer service, based on population-based cancer register, the algorithms developed by the regional cancer monitoring, ecommendations for use in cancer practice analysis and evaluation of the effectiveness of secondary cancer prevention in primary health care. Abstract

The organization of medical care to staff of bodies of internal affairs has features in connection with the nature of their office activity. Advantages and shortcomings of a departmental health care system are reflected in article. Advantage of the existing system of departmental health care is stable financing, timely finishing limits of the budgetary obligations under articles: salary, provision of medicines, capital and current repairs, medical and material equipping. High mobilization willingness of medical institution for actions in emergency situations at transfer to the mode of wartime and readiness for the transfer to the wartime mode is supported. The isolation of a system is also a shortcoming as staff of bodies of internal affairs, without having insurance medical policies, are deprived of a possibility of the choice of treatment and prevention facility. Issues of creation of opportunities of consecutive treatment of patients among institutions of departmental submission are resolved.

Abstract

The article presents methods for assessing the quality of medical care for patients with malignant neoplasms in the high-risk cancer area, creating the basis for developing sound operational management decisions. The formation of the information space is a necessity dictated by the need for rational management of all the processes of medical care for patients with malignant neoplasms in the primary health care system. The methodological problems of creating an effective information space for practical activities are associated both with the support and improvement of structures, and with the introduction of new information technologies. For the timely assessment and neutralization of the unfavorable oncological situation in any administrative territory, it is further expedient to differentiate the causes of cancer in the areas of cancer risk in terms of nosological forms of cancer.

Leadership in the organization, raising the level and timeliness of providing high-tech medical care to the population is the awareness of physicians on issues related to its organization in interaction with the patient, since the doctor is the first link between him and the help he provides. Our study showed a sufficiently high level of awareness of doctors, both in terms of awareness of this type of medical care, and in its application in practice. A lower level of awareness of in-patient medical staff was noted. But the most important is the desire of respondents working at all levels of medical care, both those who own information and do not own it, to expand their level of awareness.

Abstract

Questions of incidence of an urolithic disease and the urinogenital system of the population of all areas of the Voronezh region and Voronezh are studied. The incidence of an urolithic disease in the world is more than 3% of the population. The last decades the urolithic disease takes the 3rd place in the world among all renal pathology, every year it progresses and looks younger, strikes the children's population. 500 thousand people because of formation of stones in kidneys ask for medical care in the medical organizations. Costs of treatment of an urolithic disease are considerable as at treatment of this pathology hi-tech methods of treatment are required. The analysis of indicators of incidence of an urinogenital system is a necessary condition for achievement of high level of delivery of health care to the population and increase in quality of life of patients.

ANALYSIS OF THE DYNAMICS OF INCIDENCE OF GLAUCOMA IN THE VORONEZH REGION

Abstract

The article assesses the epidemiological situation associated with the incidence of glaucoma in the Voronezh Region (VO) and the Russian Federation (RF) for the reporting period of 2013-2017. An analysis of the official statistical data of the Central Research Institute for the Organization and Informatization of Healthcare of the Ministry of Health of Russia and the Voronezh Medical Information and Analytical Center was conducted over five years. An assessment of the dynamics of indicators of the overall incidence of glaucoma in the adult population of the region compared with similar indicators in adjacent areas. The result of the study was the definition of the problems of public health in a disease that threatened blindness.

ANALYSIS OF CARDIOVASCULAR INCIDENCE AND MORTALITY OF ADULT POPULATION OF THE VORONEZH REGION

Abstract

In work it is established that in the territory of the Voronezh region, according to data of official statistics for 2009-2016, cardiovascular diseases (CD) mortality tends to decrease, but exceeds an average value of mortality on the Russian Fed-eration (RF). Dynamics of incidence and mortality from CD in the territory of the Voronezh region for the analyzed period is studied, it is established that the greatest contribution to the general structure of incidence and mortality from CD bring the diseases which are characterized by the increased arterial blood pressure, cere-brovascular diseases and coronary heart disease. For the studied period in the Vo-ronezh region there was a decrease, both the general death rate, and the share falling on CD. The last can be connected as with development of scheduled maintenance on detection of CD at early stages, improvement of quality of delivery of health care, development of network of the cardiovascular centers, and with changes of the principles of a wording of the final clinical and pathoanatomical diagnoses conforming to requirements of the International classification of diseases of the 10th revision (ICD-10). The received results will form a basis for further researches in the field of improvement of prevention of CD in the territory of the Voronezh region.

CRITERIA FOR THE IDENTIFICATION OF SITES OF RISK AND HIDDEN FOCI OF MALIGNANT NEOPLASMS

Abstract

Approaches to optimization of management of oncological care processes on the basis of information technologies are substantiated. Developed algorithms territories adverse oncologic situations across managed and unmanaged factors, and risk and risk management. Identification of areas of cancer risk and foci of latent incidence of malignant neoplasms allows the priority mode to use the available resources. Currently, the information basis for the management of secondary cancer prevention processes is medical and statistical reporting, population cancer registry. However, the information obtained does not allow timely assessment of environmental problems and taking measures to minimize the impact of negative factors. Since the lack of the necessary resources to carry out a full range of activities that affect the cancer situation in the administrative territory is insufficient, the task of their "redistribution", in which the impact would have been on the factors that could have the greatest impact on improving the health of the population, becomes urgent.

ORGANIZATION OF SOCIAL AND HYGIENIC MONITORING AT THE REGIONAL LEVEL

Abstract

The organization of monitoring of environmental factors and population health indicators at the territorial level is shown. The data of the regional information fund describing the health of the population, its habitat, socio-economic indicators are given. The data on the contribution of environmental factors to the complex anthropotechnogenic load, priority pollutants are presented. The analysis of the assessment of the impact of pollutants of drinking water, atmospheric air on the health of the population. Unsuccessful territories were identified according to the risk of the formation of environmentally-related diseases. Information and analytical publications on the results of socio-hygienic monitoring are noted.

ON THE PROBLEMS OF IMPS IN THE VORONEZH REGION

Abstract

In the Voronezh region in medical organizations over the past 3 years, from 258 to 306 cases of infections associated with the provision of medical care are registered annually. The greatest number of cases of ISMP is registered in children's hospitals and maternity institutions by 32% - 34% of the total number of cases, in surgical hospitals is from 19 to 20.4 %, in outpatient clinics-from 3 to 4.9%. In the General structure of ISMP, acute intestinal infections account for no more than 20%, purulent-septic infections (GSI) of newborns - from 32.9% to 38.7% for other infectious diseases (SARS, chickenpox, etc.) - from 12.4% to 22.8%. From 3 to 7 cases of hepatitis B and C are registered annually. The share of postoperative infectious complications is not more than 2.6-4.2%, single cases of urinary tract infection are registered.

Abstract

The problems of research of non-food products for compliance with technical regulations related to the conditions of modeling of air and water extracts are studied. It is established that the test modeling conditions are an important stage of qualitative and quantitative chemical analysis and directly affect the results, and therefore require a clear methodological support. The lack of uniform approaches to modeling conditions in the research of non-food products can lead to incorrect conclusions about the degree of safety of products and goods for children, light industry products, personal protective equipment. The estimation of conditions and methods of modeling established by various normative documents is carried out. The necessity of their synchronization and methodological support is established.
