EXPERIENCE OF APPLICATION OF TOPOGRAPHICALLY AND ANATOMICALLY BASED, FUNCTIONALLY-ORIENTED METHODS OF PLASTY OF INGUINAL HERNIAS

Abstract

Voronezh State Medical University At the Institute of herniology named after Professor E. N. Lyubykh held research on the study of anatomy and topography in patients with inguinal hernias and healthy people. Significant changes in patients with inguinal hernias: expressed in the increase of the angle of the inguinal ligament and the shortening of the inguinal ligaments etc. The study of surgical anatomy has shown that in all patients, marked by high inguinal interval triangular shape in which impossible to complete the switching function of the inguinal canal. At the Institute of herniology named after Professor E. N. Lyubykh developed and implemented in the clinic substantiated anatomically and functionally-oriented methods of plasty of the inguinal canals for patients with unilateral (patent No. 2246904 27.02.2005) and bilateral inguinal hernias (copyright certificate №1819577 12.10.1992). They allow the correction of pathological changes in the inquind area. The results are presented in surgical treatment of 1314 gracenotea. Application of the proposed methods plastics helped to reduce the number of recurrences to 0.3%. At the same time, the methods of the Institute of herniology for the treatment of patients with inguinal hernias allow to avoid complications associated with the implantation of prostheses that can be recommended for wide implementation in practical surgery.

Abstract

Presents the results of treatment of gastroduodenal bleeding in 209 patients in emergency, the elderly and senile age, with a high degree of operational risk in a multi-hospital based on a multidisciplinary approach and the individual methods of a combined hemostasis. The developed program allowed to reduce the frequency of recurrence of hemorrhage in 2.5 and to decrease the mortality rate 2.1 times.

EXPERIMENTAL STUDY OF NEW MEANS OF LOCAL HEMOSTASIS IN THE TREATMENT OF GASTRIC BLEEDING

Abstract

We Studied haemostatic, adhesive, reparative properties and biological inertness of new hemostatic agents (polygamists, jelplast, NovoSeven) and hydrophilic granular sorbents (gelvin, unlovin, diovin, diolefin, Sephadex). The in vitro experiments was performed using native blood donor volunteers on electrocochleography N-334. The invivo experiments performed on 15 mongrel dogs. The results showed that the maximal hemostatic activity have jelplast and NovoSeven, and leading in its hemostatic activity among granular sorbents is diovin, and sephadex. The experiment has proved that the combined use of zaplatani and divine is the best combination, amplifying haemostatic effect with simultaneous acceleration of processes of regeneration in gastric ulcers.

OUT-PATIENT TREATMENT OF INTRA JOINT CHANGES

Abstract

The main task of treatment of intra-articulate fractures is prevention of development of severe post-traumatic osteoarthritis. The way of out-patient treatment by early intra-articulate introduction of chondroprotectors Zeel-T and Adgelon is presented in article. Two groups of patients on 50 people with localization of fractures in large joints of the lower extremities have been created. In control group the standard out-patient treatment was carried out. In clinical group consecutive intra-articulate introduction of chondroprotectors was added to standard therapy. When comparing results of treatment in 6 months in the studied group earlier restoration of function of a joint, fast knocking over of a pain syndrome, reduction of terms of disability was noted. These criteria demonstrated delay of destructive processes in the injured joint at patients of clinical group.

IMPROVEMENT OF THE ENDOPROSTHESIS OF THE HIP JOINT

Abstract

At present total hip replacement is one of the most effective types of surgical treatment of the patients with diseases and injuries of the hip joint. However, the incidence of complications after this operation still remains high. Dislocation and polyethylene wear complications are serious problems after surgery. The endoprosthesis of the hip joint was developed for prevention of these complications. The original endoprosthesis of the hip joint has been developed for their prevention. There is a possibility of non-invasive rotation of the liner in this implant in the postoperative period. This reduces the risk of complications and prevents possible revision surgery. It also increases the life of the implant.

CLINICAL ASSESSMENT OF A NEW LOW-INVASIVE WAY OF A REINSERTION OF A DISTAL BICEPS TENDON OF BRACHIUM

Abstract

Research was referred on a clinical assessment of a new low-invasive way of a reinsertion of a distal biceps tendon of a brachium and was randomized character. The objects of the study were 18 patients who were divided into study and control groups. In the main group the offered original way of a reinsertion of a distal biceps tendon of a brachium, and in control - its classical prototype. It is as a result established that the pain syndrome in the main group was authentically less (26,5±1,7). It is proved that the results of surgical treatment of patients with lesions of the distal biceps tendon of brachium using minimally invasive techniques offered in the majority of cases, good (30%) and satisfactory (70%), which can’t be expected from the classical methods.

Abstract

Analysis of 164 cases of fever patients at step polyclinic- emergency department - hospital was conducted to study the febrile syndrome structure. Of the 164 febrile patients with pneumonia was found in 47 cases, infective endocarditis - 37, chronic alcoholic hepatitis - in 23 rheumatoid arthritis (psevdoseptichesky variant) - in 2, systemic lupus erythematosus - in 1, polymyositis - in 2, acute pyelonephritis (or aggravation chronic) - in 15 tumors of various localization - in 37 patients. Informativeness of some blood indicators of acute inflammation, X-ray and ultrasound examination in the early diagnosis of infective endocarditis and pneumonia was analyzed.

Currently asthma is an urgent public health problem. In the pathogenesis of this disease an important role is played by the imbalance in prooxidant-antioxidant system with formation of oxidative stress, which justifies the use of antioxidants in treatment of patients with asthma. In addition, in the therapy of BA is a widely used non-pharmacological methods. The application of pulsed non-polarized red light reduces the imbalance in prooxidant-antioxidant system and promotes the improvement of asthma control.

The anti-phospholipid syndrome falls into with one of the actual low-studied nosological problems in a neurology today. NEW TREATMENT OPTIONS FOR VERTEBRAL DORSOPATHIES USING THE PREPARATION OF L-LYSINE AESCINAT

Abstract

The article describes a clinical study conducted to evaluate the efficacy of parenteral angioprotectors direct anti-edema effect of L-lysine aescinat in complex treatment of patients with acute pain in the cervical spine. The study involved 60 patients with dorsalgia. Details are set out the study design, clinical characteristics of the study and control groups. According to the results of the clinical study the efficacy of L-lysine at aescinat dorsopathies was confirmed. It was concluded that the feasibility of using this drug in the treatment of acute pain syndromes with local edema component in vertebral pathology.

GENETIC ASPECTS OF MUTATIONS, ASSOCIATED WITH AN ISCHEMIC STROKE AT THE PERSONS OF YOUNG AGE

Abstract

Ischemic stroke (IS) in young age is a heterogeneous disease. Among multiple risk factors, significant role belongs to polymorphisms in genes encoding the hemostatic system, fibrinolysis, immune response and vascular tone. Studies of frequencies of these polymorphisms in genes among young patients with ischemic stroke are rare and their role in development of the disease has not been fully determined. This study included 15 young adults diagnosed with "ischemic stroke". The control group consisted of 15 healthy young adults and 15 elderly patients diagnosed with "ischemic stroke". Analysis of polymorphisms in genes controlling hyperhomocysteinemia, immune response, vascular tone was performedwith polymerase chain reaction. It has been established that mutations in genes that control hyperhomocysteinemia, immune response, state of vascular tone play important role in pathogenesis of Ischemic stroke in young age. The results indicate the expedience of inclusion genetic research to standard diagnostics of Ischemic stroke among young adults, which will optimize the diagnostic methods and help in developing measures in preventing recurrence of the disease.

USE OF NEUROPROTECTORS IN THERAPY OF THE CAROTID ISCHEMIC STROKE

Abstract

Cerebrovascular disease and stroke take one of the first places in the structure of overall morbidity in the world. Out of the total mortality in the Russian Federation acute cerebrovascular accident (CVA) in second place after coronary heart disease and up to 21.4%, and disability after stroke reaches the figure of 3.2 per 10 thousand. Population, ranking first among all causes permanent disability. Thus, the widespread, high mortality and disability in the population due to cerebrovascular disease and stroke allows us to consider them not only as medical, but also social and economic problem. Due to the above the question of adequate and effective treatment of an ischemic stroke, allowing to reduce terms of treatment of the patient is actual.

Abstract

Traditionally, it so happened that the study of the internal picture of the disease (UNHCR) conducted mostly in line with the Russian clinical psychology. The physician in any specialty, taking into account the WKB particular patient has an opportunity to more fully reveal the etiology, pathogenesis, disease clinic and choose the best algorithm for therapeutic and diagnostic system, guided by the characteristics of the patient's personality. Improving rehabilitation and medical-social examination of patients who underwent a traumatic brain injury (TBI) in young and of working age, it can be achieved by adequate maintenance of these patients, not only in acute but also in subsequent periods after suffering head trauma. This requires a correct assessment of clinical prognosis and rehabilitation, taking into account characteristics of pathogenic effects of trauma and subjective (personal) relationship of man to the situation. The study of WKB when TBI is a significant area not only for health care workers and psychologists, but also becomes a central element in the analysis and understanding of the patient's personal perspective of neurology, neurosurgery, psychiatry, rehabilitation. It requires a holistic comprehensive assessment of WKB in young patients undergoing CCT as subjective picture of the disease begins to act as a magnifying glass, allowing a glimpse into the inner world of the patient.

Voronezh regional clinical hospital №1. Abnormal uterine bleeding is a severe complication in operative obstetrics. Reinfusion proper scour erythrocytes has found wide application for blood replacement. However, the processing hardware modifies proper red-blood cells characteristics. This fact has a negative influence on oxygen delivery process to the tissues. We investigated auto erythrocytes condition after the processing hardware by Cell-saver 5. Significant variation of erythrocytes deformability have been registered. While morphological indexes and heterogeneity of red-blood cells are kept previous. Given case supports optimal conditions for oxygen delivery to the tissues.

The gender structure of a trauma, distribution of patients on age, the mechanism of receiving the combined craniofacial damages, communication of weight of a craniocereberal trauma with damages of face zones is studied. The algorithm of diagnostics and treatment of a kraniofatsialny trauma, practical recommendations are developed.
